Arthroscopy of the patello-femoral joint.

Arthroscopy of the knee joint was performed in 126 patients complaining of symptoms in the patello-femoral joint. In about one-third of these cases the arthroscopic findings in the patello-femoral joint were normal, whereas the remainder had varying degrees of chondromalacia or arthrosis. The classification of changes agreed well with an independent classification carried out in conjunction with surgery. In one-third of the cases there were additional findings which could explain the knee symptoms, more than half of these being degenerative changes reminiscent of an early gonarthrosis located in the femuro-tibial joint. There were no complications arising from the arthroscopy procedure which was well tolerated by the patients. It is concluded that arthroscopy is a helpful tool for differential diagnosis in patients with patello-femoral pain and for classification of degenerative changes in this joint.

Anterior displacement of the tibial tuberosity in chondromalacia patellae.

Sixty-eight knees in 62 patients wiht degenerative changes of the patellar joint surface, chondromalacia or arthrosis, were operated on with anterior displacement of the tibial tuberosity. Complications were encountered in no less than 22 joints and the results appeared to be somewhat related to these complications, particularly in women, even if a direct relationship between complications and remaining symptoms could not be demonstated. In a follow-up examination an average of 13 months after operation, approximately 90 per cent of the patients had improved, men as well as women. In an additional follow-up up, 30 months after operation, the men had maintained or even somewhat improved their good results whereas in women only about 70 per cent could not be classified as improved or free from symptoms. When those patients, who were not as regards all variables improved by surgery, were analysed separately, there was in this group a preponderance of women and particularly older women. Also, the patients with complications belonged to this group. Modifications of the surgical procedure did not seem to change the results nor were the results different in those patients in whom a superficial shaving of the articular cartilage of the patella had been added. The conclusion of the study is that anterior displacement of the tibial tuberosity has a lasting beneficial effect on patiens with degenerative changes of the patello-femoral joint.

Netilmicin in urinary tract infections.

30 urological patients with normal renal function were treated with netilmicin because of urinary tract infections, mostly recurrent and due to predisposing factors. the drug was given in doses of 4 mg/kg/day for 4-9 days. All but 5 patients had resolution of their signs and symptoms, and the bacteria were eliminated in 23 patients, with recurrence in 8 and reinfection in 3. The bacteria persisted in 1 patient and the results were indeterminate in 6 patients. No signs of hematopoietic, renal and hepatic toxicity occurred, and vestibular and auditory functions were undisturbed. Regular assays of serum drug concentrations revealed no signs of accumulation. Netilmicin given in doses of 4 mg/kg/day is an efficient antibiotic with low toxicity in the treatment of urinary tract infections.

EORTC protocols in prostatic cancer. An interim report.

Two parallel prospective randomized studies have been undertaken by the EORTC Urological Group in previously untreated patients with prostatic cancer in order to compare low dose Stilboestrol versus Cyproterone acetate versus Medroxyprogesterone acetate in the first trial, and Stilboestrol versus Estracyt in the second trial. Although the follow up is still short, no superiority of the other drugs over Stilboestrol had appeared so far with regard to either objective response or significant side effects apart from gynaecomastia. In the third trial, patients with advanced disease no longer responsive to hormonal treatment were randomized to either Adriamycin or Procarbazine. Toxicity and early death were particularly frequent in Procarbazine treated patients, whereas most patients progressed in both treatment groups.

Degradation of mecillinam in aqueous solution.

The hydrolysis of mecillinam in aqueous solution (37 degrees) was studied at pH 2-10. The degradation products observed by TLC and NMR were identified and quantified. Several of these compounds were synthesized. Mecillinam and the key degradation product, (6R)-6-formamidopenicillanic acid, underwent reversible 6-epimerization in basic solution. Some of the thiazolidine derivatives formed epimerized at position 2. In contrast to penicillins, the degradation pattern of mecillinam becomes more complex with increasing pH. Rate constants for some processes are given.

Content and synthesis of nucleic acids in the cartilage in chondromalacia patellae.

The content and the synthesis of nucleic acids in chondromalacian, osteoarthritis and normal cartilage was compared. The chondromalacian cartilage differed from osteoarthritis in that the content of nucleic acids was less. Also, the cell density was less in chondromalacian than in normal cartilage as opposed to previous findings in osteoarthritis. The synthesis of DNA was greater in chondromalacian than in normal cartilage but less than in osteoarthritis. With regard to the RNA synthesis, however, the chondromalacian cartilage showed a higher rate than both normal and osteoarthritic cartilage.

Fluorescein angiography especially of the upper extremities.

The technique used in sequential fluorescein angiography of the upper extremities is described. The sensitivity and reproducibility of the method are exemplified and discussed in regard to its applicability in diagnosis of organic arterial obliterations, e.g. in Raynaud's phenomenon as well as to its usefulness in evaluation of reconstructive arterial surgery and medical treatment with respectively thrombolytic (Brinase, Astra) and microhaemorheologic agents (Venoruton, Zyma; Arwin, Knoll) or those with an influence on systemic haemodynamic factors as in induced hypertension. The importance of standardized experimental conditions is emphasized, particularly with reference to vasodilatation. The method represents a new possibility of obtaining clinically a functional over-all information on the nutritional blood flow of rather extensive skin areas. Also, it can illustrate the interaction between macro- and microcirculation. Segmental macrovascular occlusions without concomitant damage of the corresponding terminal vascular area mainly cause a prolongation of the fluorescence appearance time. Obstruction of the microvascular blood flow reveals itself as more definite changes in the fluorescence pattern of the affected skin areas.

Late results from mucosal denudation for urinary bladder papillomatosis.

The study comprises 16 patients who during the period 1965-1972 underwent mucosal denudation (stripping) for multiple, highly differentiated, non-invasive bladder papillomas. No major complications to the operation were seen. In 3 of the 16 patients histological examination of the operative specimen revealed a more malignant tumour than found on preoperative biopsy. During the observation period, which averaged 4 years, the disease recurred in 10 of the 16 patients. In 4 of them, repeated biopsy showed a more malignant tumour than did the operative specimen. The study moreover demonstrated that following mucosal stripping, pronounced bladder contracture occurred in all the patients and dilatation of the upper urinary tracts in half of them. These serious complications mean that even if the disease does not recur, a reasonably normal life for the patient is unattainable. The method, consequently, cannot be recommended.
