A new rabbit antiserum-based assay was designed and evaluated for the assessment of the allergen potency of latex medical gloves, since total protein measurement by modified Lowry method remains unsatisfactory and the human IgE-inhibition method is limited by the use of sera from type I allergic patients. Four rabbit sera against a nonammoniated latex extract were shown by immunoblotting to have similar binding patterns to those obtained by human IgE. One rabbit serum was used to develop a competitive immunoassay for antigenic latex proteins (CIALP). The same nonammoniated latex extract was used for coating and calibration. The lower detection limit of the method was 0.085 microgram/g of glove. Antigenic proteins measured by CIALP for 77 latex glove extracts (33 pairs of surgical gloves and 11 exam gloves) showed positive correlation with those of the modified Lowry method (r = 0.4; p < 0.001). For 36 extracts made from the right and the left hand of 18 out of the 33 surgical latex gloves tested, inhibition of human specific-IgE results did not correlate with the modified Lowry method but did with the CIALP results (r = 0.8, p < 0.0001). The CIALP, which is well correlated with the human IgE-inhibition test, enables the assessment of the allergenicity of latex medical gloves by measuring the antigenic proteins.
Mizolastine, a new second-generation H1 receptor antagonist with additional anti-allergic properties, was compared with loratadine in 61 patients suffering from severe chronic idiopathic urticaria (CIU). In this double-blind study, patients were randomly allocated to receive either mizolastine 10 mg (n = 26) or loratadine 10 mg (n = 35) once-daily for 28 days. Both compounds were well tolerated, safe and efficacious. The reduction in the number of episodes per week (5. 6+/-16.3 and 6.4+/-12.4 for mizolastine and loratadine, respectively) and the reduction in the symptom severity score, measured using a Visual Analogue Scale (VAS), were comparable (30.2 +/- 39.0 mm and 30.5 +/- 28.5 mm for mizolastine and loratadine, respectively). Mizolastine had a positive effect on angioedema (85% CI 95% [0.69-1.00]) of patients improved compared with 75% (CI 95% [0.59-0.91]) of the loratadine group and the differential reduction of the mean total duration of episodes in the mizolastine group was higher when compared with the loratadine group (from 13.7 +/- 33.5 hours on day 0 to 5.1 +/- 9.0 hours over the treatment period and from 8.2 +/- 8.8 hours on day 0 to 5.1 +/- 7.8 hours over the treatment period for mizolastine and loratadine, respectively). Prick test analysis demonstrated that both drugs caused a significant decrease of histamine-induced wheal and flare with no development of tolerance, with a significant superiority of mizolastine over loratadine for some histamine concentrations. Mizolastine and loratadine both proved very efficacious and safe. In addition mizolastine demonstrated a superiority in prick tests, beneficial effects on angioedema and seemed to provide a faster onset of action.

BACKGROUND: Vibratory angioedema is a rare form of physical urticaria. This entity can be an hereditary autosomal dominant disorder or an acquired sporadic disease. Immediate and delayed forms have been reported. CASE-REPORT: A 34-year-old woman was referred to us for a vibratory angioedema induced by mountain biking. Despite her handicap, she could provoke a clinical tolerance with disappearance of pruritus but not of erythema and edema if she continued her physical effort. Diagnosis was confirmed with a vibratory stimulus using a Vortex for 5 minutes. The realization of a controlled vibratory stimuli among 20 volunteers induced a positive response in 7 cases (35 p. 100). DISCUSSION: These results raise the problem of the specificity of the test but also point out the possible physiological response of the body to intense vibratory stimulations.

The authors review 26 cases of multiple endocrine tumours with presence of a carcinoid tumour. In 44% the carcinoid was localised to the bronchus, in 20% of cases to the small intestine or duodenum. In 76% of cases during multiple endocrine tumours, the carcinoid appeared in embryological derivatives of the anterior part of the intestine. No carcinoid syndrome was reported. The difficulty of histological distinction has led to the description of carcinoid islet cell tumours. These facts fit in with the APUD concept.

Numerous physiopathological processes such as diurnal variations sex, age, pregnancy, muscular effort, diseases and drugs modify the Achilles reflexogram. Their multiplicity explains the difficulty of general interpretation of changes in the reflexogram which bring into play the energy reserves of the muscle cell and its potassium content.

Total eosinophil count (TEC) was done in 13 normal subjects, in 8 subjects before and 90 mn after a "French" breakfast and in 29 asthmatic patients. The geometric mean is higher (p less than 0,001) in asthmatic than in normal subjects: 304 (231--391) versus 144 (101--197). The breakfast does not interfer with the result of the TEC. In asthmatic subjects, a significant inverse correlation appears (r = 0,57, p less than 0,01) between the logarithm of TEC and the quotient: formula; see text. This correlation persists whatever may be the subject's age, the duration of the asthma or age at which he first contracted the illness. Asthma appeared after 40 years in 4 of the subjects: their TEC was higher (p less than 0,01) than in subjects who had become asthmatic before the age of forty. The respiratory function was lowered more in these 4 patients than in others who had been ill for the same time (p less than 0,001). The TEC is not related to results of skin testine with usuel inhalant allergens (grass pollen, house dust, cat's or dog's dander) or by result of a provocation test (by inhalation of allergen). Eosinophilia and bronchial obstruction are connected by one (or more) factors that may be inhalant allergens in extrinsec asthma and circulating immune complexes in intrinsec asthma.
