Organic solvents are debated as a cause of chronic toxic encephalopathy. The pathogenesis is not completely known. There are some indications of structural changes in the central nervous system. Epidemiological studies are saddled with potential faults and methodological problems, but in total the epidemiological evidence for a causal association between solvents and encephalopathy is sound. The first step in the diagnosis is to demonstrate encephalopathy by neurological and neuropsychological methods. The next step is to estimate the probability of solvents being the cause of this encephalopathy. Misjudgement of diagnosis is often due to uncertainty in the exposure evaluation. We discuss elements of the exposure assessment, and outline neuropsychological testing as part of the diagnostic process.
Energy conservation, insufficient ventilation and introduction of new building materials are accompanied by increasingly frequent complaints of increasingly frequent mucosal irritation, headache and lethargy among occupants, commonly called "sick building syndrome". Infections and toxic allergic reactions may be caused by infected humidifiers. The article includes a review of symptoms and causes. Many of the complaints can be prevented. Combined efforts by medical and technical personnel are necessary in diagnosing and solving indoor climatic problems.

A group of 51 male children, average age 9.1 years, presenting at a child psychiatric clinic because of attention and conduct problems were investigated for reading and phonetic spelling ability, vigilance, and intellectual level. Each child was given a DSM-III diagnosis prior to being tested. The Boder Test of Reading and Spelling patterns was used to obtain scores for reading age, number of syllables read correctly in the word list at the child's reading level, and number of phonetically correct syllables spelled in the known and unknown word lists, at and just above the child's reading level. A factor analysis with varimax rotation produced two significant factors, a Reading factor with high loadings for verbal and reading scores, aand a second Vigilance factor. Phonetic spelling ability loaded separately from reading into the Vigilance factor. The Vigilance factor was significantly related to the diagnosis of moderate and severe attention deficit disorder with hyperactivity (ADDH). The correlation of short-term working memory with vigilance in moderate to severe ADDH children is discussed.
Controversies in the use of central nervous system stimulant medications in children with attention deficit hyperactivity disorder are discussed. Diagnostic issues, age of optimal use, side effects, effects on learning and ethical considerations are current issues. An animal model for the effects of chronic long-term high dose regimes is proposed.

The effect of conditioned media obtained from in vivo or in vitro asbestos-transformed rat pleural mesothelial cells on growth of normal mesothelial cells was studied. It was found that both conditioned media led to a reduction of cell proliferation as shown by a decrease in the following parameters: cell number, tritiated thymidine incorporation and percentages of S cells. Similar effects were obtained with EGF (Epidermal Growth Factor).

Several transformed cells have been demonstrated to secrete growth factors. We studied the effect of conditioned medium from neoplastic rat pleural mesothelial cells on normal and neoplastic mesothelial cell growth. The results showed that the concentrated conditioned medium stimulated neoplastic mesothelial cell growth but inhibited reversibly normal mesothelial cell growth.

Nine patients with Paget's disease were treated with 200 U (15 nmol) synthetic salmon calcitonin (sCT) intranasally (in)/day for 12 months. Five of them had received im or in sCT therapy for 1-4 yr up to 0.5-5 yr before this study. Low titer antibodies to sCT were detected in the serum of three of these five patients, but not in the four patients who had not received prior sCT therapy. After 2 months of in sCT administration, four of the former group, but none of the latter group, had antibodies to sCT. After 12 months of treatment, antibodies to sCT were found in all patients who had received sCT earlier and in three of the four patients who had not. The half-maximal inhibition of [125I]sCT binding ranged from 44-284 pmol/L sCT. In a cultured human breast cancer cell line (T47D) cAMP production was stimulated by sCT (EC50, 70 pmol/L). cAMP production stimulated by sCT (5 pmol/L) was reduced to 6-20% of the control value in the presence of serum from patients which inhibited [125I]sCT binding by more than 50% in a dilution of 1:50 or greater. In patients with lower titer antibodies cAMP production was not inhibited. Serum alkaline phosphatase activity was transiently lowered to 79 +/- 6% (+/- SE) of basal levels in the patients who had earlier received sCT (P greater than 0.1), while sustained reduction to between 66 +/- 2% and 84 +/- 6% of basal levels (P less than 0.05) occurred in the patients who had not been treated with sCT previously. In conclusion, reexposure to sCT of five patients with Paget's disease caused secondary antibody responses and clinical resistance.

The American family income distribution now lies at the center of several controversies. Some observers argue that the American middle class is vanishing, but U.S. census income statistics show income inequality has not changed appreciably since 1947. A second controversy involves whether average living standards have risen or fallen since the major oil price increase of 1973-74. These controversies can be partially resolved by understanding the sharp slowdown in the growth of workers' wages which occurred after 1973 and the demographic trends which kept per capita living standards rising despite stagnant wages, including more working women and low birthrates.
