Results of IVF attempts in patients with unexplained infertility.

From January 1986 to July 1987, 143 patients with unexplained infertility (UI) following 217 IVF attempts were studied and randomly assigned for statistical analysis to be compared with 434 tubal infertility (TI) patients undergoing 748 IVF attempts. The age of patients, previous pregnancy history and stimulation protocols were identical in both groups. In comparison with tubal patients, IVF attempts on the UI group were characterized by the same rates of cycle failure, mean number of oocytes retrieved per cycle, a lower fertilization rate (45.7% UI/59.8% TI) (P less than 0.01) and no difference in cleavage and nidation rates. However, a decrease in the pregnancy rate/attempt (13.8% UI/19.5% TI) tended towards a significant value (P = 0.06). Although the semen parameters were found to be in the normal range during the previous fertility screening in both groups, the incidence of at least one abnormality (count less than 20 X 10(6)/ml, and/or total motility less than 30% and/or abnormal forms greater than 75%) on the day of insemination was found to be significantly higher in UI (20%) than in TI (11%) patients. Moreover, 25% of UI patients did not fertilize any oocytes inseminated, whatever the number of oocytes retrieved. This rate of failed fertilization was significantly lower (9%) in tubal patients. The oestrogen response profiles were similar in both groups, analyzed according to the stimulation protocols.(ABSTRACT TRUNCATED AT 250 WORDS)

[Congenital mesoblastic nephroma (Bolande's tumor). Prenatal diagnosis].

The authors report a new case of the antenatal ultrasound diagnosis of a congenital mesoblastic nephroma (also called Bolande's tumor) at 33 weeks of amenorrhea. This tumor was found to be associated with acute hydramnios involving premature labor and fetal death by respiratory embarrassment. Congenital mesoblastic nephroma is a very rare entity but is the predominant renal neoplasm in the immediate neonate period. The differential diagnosis between Bolande's tumor and nephroblastoma (Wilm's tumor) is now clear since histopathological features have been precisely described by Bolande. Clinical and pathological features of the renal disease are discussed. The prognosis is excellent for most cases requiring only surgical treatment (nephrectomy).

[Prostaglandins and the uterine cervix. Immunofluorescence study].

The authors, using immunofluorescence, studied the effect of different prostaglandins (F2 alpha, E1, dimethyl PGE1) on cervical connective tissue. They analysed 80 biopsies which were carried out before and after the prostaglandins had been applied locally, both in pregnant and in non-pregnant women. The method showed that there were changes in the collagen fibres but not in fibronectin. On the other hand, there does not seem to be any difference in the collagen effect with the methods used: 1) between pregnant and non-pregnant women, and 2) between the different types of prostaglandin that were studied.

[Fetal blood sampling for karyotype using echoguided puncture of the cord. Study of 103 pregnancies].

Subcutaneous ultrasound guided puncture of the umbilical cord was carried out in order to obtain samples of blood in 103 fetuses. This was done to get the result of the karyotype quickly, i.e. in 72 hours. The indications were for diagnosis, particularly of malformations (63 cases) or intra-uterine growth retardation (24 cases). A small proportion of the indications were those for failures to carry out amniocentesis early or for checking on mosaics that were obtained after the culture of amniotic fluid cells. In 88% of the cases it was possible to obtain a karyotype and the results were checked with the sex of the fetus and the karyotype that had been obtained from amniocentesis. There were 11 abnormalities diagnosed. The chief indication of chromosome abnormality is fetal malformation. Because the result are obtained so quickly cordocentesis should replace late amniocentesis, at present when looking for fetal abnormalities.

[Benign liver tumor during pregnancy. Review of the literature and management (apropos of a case)].

The authors report a case of a tumour of the liver in a pregnant woman who had taken oral contraception for six years. Since oral contraceptives have become far more widely used the two benign tumours of the liver, namely a hepatocellular adenoma of the liver and focal nodular hyperplasia, have become far more common. The increased blood supply to the liver can make such a tumour symptomatic and increase the risk of it rupturing intraperitoneally, especially at the onset of labour. A course of management has been suggested according to the size of the tumour and the duration of the pregnancy.

[RU 486 and induction of labor in 3d trimester pregnancy interruptions. Preliminary clinical studies].

The authors report 12 cases of induction of labour which was carried out with the help of RU 486 in the 3rd trimester of pregnancy (mean duration of the pregnancy 34.2 weeks). Nine cases had malformed fetuses and 3 cases had normal infants. In 6 cases out of the 12 delivery took place within 48 hours after RU had been administered by itself and in 3 cases induction with Syntocinon was helped when RU was given beforehand. In 3 cases the live-born children showed no secondary ill effects.

[Pregnancies occurring after a perinatal death. Preparation of a psychological risk index].

The authors have interviewed 21 women who were attending the gynaecological and obstetrical service of the hospital for pregnancies occurring after a perinatal death. They show that anxiety is characteristic of these situations, but that the intensity of anxiety varies from woman to woman. They describe, after analysing the interviews, the different variations that can be found as these pregnancies progress according to the type of anxiety. By using a statistical analysis they can separate out the most important criteria and they can work out a method of illustrating a threshold which can easily be used by obstetricians in everyday practice. They suggest a method of handling the patients according to the level of anxiety found.

[Action of Parlodel on the preovulatory peak of estrogens and the ovulatory response of patients with abnormal ovulation and normal blood prolactin].

Bromocriptine, a dopamine agonist, is well known for its inhibitory action on prolactin secretion. Its action on other hypophyseal secretions, particularly on the gonadotrophins FSH and LH can indicate use of Parlodel as a stimulatory agent for induction of ovulation even in cases of normal prolactinemia. 129 normoprolactinemic patients with a defect of the preovulatory estrogen rise have been treated over a period of 3 months. 63% of the patients exhibited a positive response to bromocriptine, either because a pregnancy started or because preovulatory E1+E2 became normal. The rate of "responders" is increased among those patients who had conservation of spontaneous menses at the start of therapy. Conclusions of the study are that Parlodel has a place among treatments of ovulation defects. Its genuine efficacity must be confirmed by a controlled study against placebo and other confirmed treatments like antiestrogens.

[Free tumors of the peritoneum. Review of the literature apropos of a case].

Free bodies in the peritoneum are observed infrequently, particularly large solid tumors as in the present report. Of slightly more frequent occurrence are detached epiploic appendices or cystic tumors. Symptomatology varies greatly according to whether organs are compressed or irritated by the free body. Diagnosis is always preoperative, and the postoperative course simple because the tumor is delivered without dissection or ligature. Prognosis is good since these tumors are benign. Histopathology demonstrates origin and histologic nature of free body, and operation for a "chronic uterine fibroma" in the present case revealed a large free tumor. Operative findings (absence of right appendage) and histology suggested a diagnosis more likely to be of ovarian fibroma with a ruptured pedicle from chronic torsion. Despite the advanced age of the patient (96 years) no postoperative complications were reported.

[Evaluation of the clinical and biologic means of assessing fetal prognosis in pregnancy toxemias. Apropos of 223 cases].

Two hundred and twenty-three cases notes of patients with toxaemia of pregnancy were analysed in order to try to establish the ability to predict through clinical and biological assessments of the mother the prognosis for the fetus. Statistical analysis methods were chosen to work out the classification of the parameters and to lead to an index of severity that could be established by a multifactorial discriminating analysis. This study confirms that there is an inversion in the circadian rhythm of arterial blood pressure, with an increase in the evening and at night. It shows how bad prognostically blood pressure instability is. On the other hand oedema of the legs is of no significance. Contrary to the view that has been held for a long time, an increase in weight is directly proportional to fetal parameters. As far as parameters are concerned as a whole (both clinical and biological) a rise in blood urea is more discriminating at every stage of the illness and precedes an increase in the platelet count, the changes in creatinine clearance and night blood pressure. Maternal changes are better correlated with fetal variability before the 35th week of the pregnancy whereas after that date the prognostic value of these readings is of little importance.

Immunohistochemical detection of the estrogen-regulated 52,000 mol wt protein in primary breast cancers but not in normal breast and uterus.

An estrogen regulated glycoprotein of molecular weight 52,000 is released by metastatic human breast cancer cells in culture. In order to detect this protein directly in human tissues, several high affinity monoclonal antibodies were produced against the 52,000 mol wt protein. Frozen sections of human breast cancer samples were stained by the peroxidase-anti-peroxidase method using these antibodies. In 20 of 25 samples, specific immunoperoxidase staining was observed in the cytoplasm of epithelial cells with six monoclonal antibodies to the 52,000 mol wt protein. The 5 samples that were not stained contained no detectable estrogen receptor. Epithelial cells were not stained in 6 normal mammary glands collected during reduction mammoplasties and in 9 normal uteri, whether tissues were collected during the follicular or luteal phase. The demonstration that the 52,000 mol wt estrogen regulated protein is present in the cytoplasm of some primary breast cancers but absent in normal mammary tissue and uterus indicates its possible use as a tumor marker.

[Veralipride and menopause].

Twenty post-menopausal women (10 natural and 10 surgical menopauses) with climateric manifestations consisting mainly in hot flushes were given veralipride. Overall satisfactory results obtained in two-thirds of cases evidence the value of tis non-hormonal therapy, especially in cases of natural menopause which responded consistently. Clinical tolerance of veralipride proved satisfactory. Biological investigations (total and differential blood cell counts, glucid and lipid metabolism, BUN, transaminases) before and after three twenty-day courses of veralipride showed no modifications. Hormonal assays done in four patients showed no action of veralipride on FSH and LH.

[Postpartum cardiomyopathy attributed to the use of beta-mimetics].

A case of post-partum cardiomyopathy probably due to salbutamol is described. The patient was a young primipara without any cardiovascular disease. She presented a sudden left ventricular failure after Caesarean section for twins: salbutamol had not been given for a week. After 10 days' treatment with diuretics and vasodilators, the patient was cured, no sequellae remaining. It seemed the mechanism was multifactorial: direct myocardial injury, a reduction in the number of receptors, peripheral haemodynamic changes.

[Probability of requests for desterilization. Prospective study of 50 cases].

The reasons for 50 requests for reversal of tubal sterilisation were studied prospectively. Our results agree with those in the rest of the literature and show that within a population of women who have been sterilised there is a group who have "a strong likelihood to ask for reversal of sterilisation". These results make one think of the role of the doctor and can help by using scores that have been worked out previously to improve the indications for sterilisation in case legislation will be forthcoming.

[Ovarian tumor biology].

The authors studied endocrine ovarian tumor secretions capable of causing a clinic and/or biological syndrome. The plasmatic hormonal determinations and the in-vitro incubation methods revealed that the mesenchymal tumors discharged a full range of steroidsid hormones. Their feminizing or masculinizing clinic aspects depend on each respective hormonal ratio and on the field where they are acting. Ultrastructural studies show that steroid cells derive from the mesenchymal cells and that they often have an ambiguous structure (male or female). Tumors with functional stroma capable of producing steroïds appear to be frequent if correct plasmatic determinations and histochemical studies for their diagnosis are used. The polypeptidic hormonal secretions are much more uncommon. Aside chorioepithelioma and carcinoïde tumors in which the hormonal secretion is in agreement with the histological aspect, ACTH, TSH, PRL, para-hormone or insulin secretions responsible for endocrine paraneoplastic syndromes, may be found as well.

[Carcinoembryonic antigen (C.E.A.). Its value in the care of malignant tumours of the ovary (author's transl)].

The authors report on 466 serial levels of carcinoembryonic antigens (C.E.A.) carried out with the aim of surveying 44 patients who had malignant tumours of the ovaries : - one third of the cases had a positive level more than 10 ng per ml; - the presence and the serial level depend more on the histological nature of the tumour than the stage of its evolution; - when a serial level has been negative before treatment it has never become positive in in a definite and continuous manner when the tumour progresses. On the other hand, when the test is positive before any therapy is started then there is a good correlation between its clinical evolution, its response to treatment and the serial variations. Relying on their experience and on the results given in the literature, the authors suggest a scheme of practical use of levels of C.E.A. in the care of malignant tumours of the ovary.
