Treatment of hirsutism by oral cyproterone acetate and percutaneous estradiol.

Twenty hirsute women were treated with 50 mg cyproterone acetate orally, administered from the 5th to the 25th day of the menstrual cycle, along with 3 mg 17 beta-estradiol administered percutaneously from days 16-25. Percutaneously administered 17 beta-estradiol was used rather than ethinylestradiol in order to avoid the side effects of oral administration of synthetic estrogens. From a clinical point of view there was a dramatic improvement of hirsutism after 3-6 months of treatment. Biologically, plasma testosterone decreased markedly (P < 0.01) from 64.6 +/- 24.2 ng/dl (n = 20) to 25.2 +/- 11.8 (n = 20), 26.1 +/- 16.6 (n = 16), and 13.3 +/- 10.8 ng/dl (n = 14) after 3, 6, and 9 months of treatment. There was also a significant decrease in delta 4-androstenedione from 251.0 +/- 110.2 ng/dl to 129.9 +/- 66.5, 114.2 +/- 45.8, and 62.0 +/- 21.5 ng/dl after the same periods. From these results it may be assumed that this therapeutic combination has an antigonadotropic effect, as confirmed by the decrease in plasma estradiol, FSH, and LH and the absence of a significant progesterone level in all cases. Plasma and urinary cortisol, lipids, and hepatic tests remained normal. The good clinical and biological tolerance of this treatment makes it interesting to consider for use in the management of hirsutism.

[Deviation in cortisol metabolism induced by rifampicin. Therapeutic consequences in adrenal failure (author's transl)].

Deviation of cortisol metabolism in favour of its 6 beta-hydroxylated derivative was demonstrated in two patients with adrenal failure receiving substitution corticosteroid therapy and rifampicin. The existence of a frank increase in the metabolic clearance antipyrin was in favour of an hepatic enzyme induction. After rifampicin treatment was stopped, the 24 hour urinary excretion of 6 beta-OH-F returned to normal, demonstrating the responsability of the drug. This enzyme induction results in a need to increase the dose of hydrocortisone substitution therapy in patients with Addison's disease treated with rifampicin.

[The hormonal basis of discontinuous progestational contraception (author's transl)].

This study reports the changes in the principal biological parameters seen during the human menstrual cycle (plasma FSH and LH gonadotrophins, oestradiol and progesterone) and provoked by the discontinuous administration of a progestational agent. Fourteen women aged from 25 to 40 years, volunteers, received this treatment. In 9 women, 10 mg of lynoestrenol were administered from the 10th to the 25th day of the cycle. In 5 others, the progestational agent was given at the same dose from the 5th to the 25th day of the cycle. In all cases, there was a rapid and durable fall in gonadotrophins and ovarian steroids in the plasma. These results suggest a marked antigondadotrophic effect of the progestational agent used, which would provide effective contraception. The possibilities offered in the area of human contraception by the dicontinuous administration of progestational agents are discussed in relation with the indications of these substances, in particular during the premenopausal phase and in cases of luteal progesterone insufficiency.

[Polycystic ovary syndrome (author's transl)].

Recent developments have occurred in the understanding of the physiopathology of polycystic ovary (Stein-Leventhal) syndrome. The authors describe in particular the present bio-chemical definition of so-called type I polycystic ovary syndrome: very high and anarchical secretion of LH by the pituitary, explosive response of LH during the LH-RH test, contrasting with normal levels of FSH under basal conditions and after stimulation with LH-RH. In the polycystic ovary, positive and negative feedback exerted by oestrogens at the level of the hypothalamus are intact. In particular, the administration of clomiphene results, in the majority of cases, in ovulation in patients with this disorder. The mechanisms (ovarian hyperproduction of androstenedione, increased transformation of androstenedione into testosterone then dihydrotestosterone and into androstanediol) responsible for the development of hyperandrogenism during polycystic ovary syndrome are also analysed.

[12 cases of polycystic ovary type 1 (Stein-Leventhal syndrome): complete hormonal study (author's transl)].

Twelve patients with clinic and anatomic features of polycystic ovary syndrome type 1 were investigated for gonadotropic and androgenic functions. Basal LH level was 3 times higher than in normal women in the beginning of follicular phase (16,2 +/- 3,0 mUI/ml). After LH-RH stimulation (100 microgram), peak LH level was excessively high (70 +/- 10 mUI/ml) whereas FSH was normal. Clomiphen citrate (100 mg X 5 days) was followed in all cases by temperature ascension and biologic evidence of luteinisation. Four pregnancies were obtained. Plasma androstenedione was elevated in most cases (320 +/- 30 ng/ml) with elevated urinary androstanediol (80 +/- 20 microgram/24 h.) These elevated levels were strikingly reduced in all cases after ethinyl-estradiol, 50 microgram X 20 days. Physiopathologic hypothesis consistent with these results are discussed.

[The treatment of benign pathological conditions of the breasts with progesterone and progestogens. The results according to the type of breast condition (260 case records) (author's transl)].

This work reports the clinical results which were obtained after treating 260 women who had pathological conditions of the breasts using a combination of a progestogen which was given by a generalised route (orally or by injection) and progesterone which was applied locally so that its effect would be percutaneous on the breasts. These results were evaluated according to the effectiveness of the treatment according to the different symptoms or groups of clinical symptoms and radiological findings which had been noted at the time of the first examination. A better therapeutic effect was found in cases of early lesions where oedema was the dominant factor (mastodynia) and glandular hyperplasia (diffuse polyadenomatosis). On the other hand, longstanding lesions (sclerocystic mastopathia) where sclerosis was marked showed a much more variable result of treatment with progestogens. Therefore its seems that benign conditions of the breasts should be treated early and particularly in the stage of mastodynia if development into irreversible sclerotic conditions is to be avoided. It also has to be carried on for a long time, especially if there are other risk factors for cancer which are associated with conditions in the breast. The prophylactic role of this treatment in regard to cancer of the breast is discussed.

[Hirsutism (author's transl)].

Hirsutism may arise from two different causes: an increased production of active androgens by the adrenals and/or an increased utilization of circulating androgens by the target cells of the skin i.e. idiopathic hirsutism. Plasma testosterone and androstenedione levels are the best indexes of androgen production. Testosterone 5 alpha-reductase capacity in the skin and urinary 3 alpha-androstanediol reflect androgen utilization. In most hirsutisms, the only plasma testosterone level give sufficient information: it is normal with normal ovulatory cycles in idiopathic hirsutism, it is slightly elevated in ovarian dystrophy, the most frequent causes of hirsutism. More sophisticated investigations are only needed when testosterone levels appear to be over 1.50 ng/ml. In these cases, one could expect an uncommon etiology such as virilizing tumor on a delayed onset of congenital adrenal hyperplasia.

[Benign breast diseases: hormonal studies in 125 cases (author's transl)].

The results of exploration of the corpus luteum in 125 patients with benign breast disease are analysed. In 109 patients, plasma oestradiol and progesterone were measured at various times during the hyperthermic phase of the menstrual cycle. The results obtained were compared with those obtained in normal women explored during the same phase of the cycle. The average daily values for plasma progesterone obtained in this group of 109 patients was significantly lower than that found in the normal women, whilst there was no change in oestradiol levels. In addition, 16 patients were studied daily throughout the period of hyperthermia. In 8 cases, there the hyperoestrogenism, with or without progesterone insifficiency of the corpus luteum. In all, secretory imbalance of the corpus luteum with a tendency to relative or absolute hyperoestrogenism was a constant finding in women with benign breast disease.

[Is progesterone insufficiency carcihogenic?].

Estradiol is a natural estrogen which favors cellular hyperplasia at the level of its target organs. As for progesterone, its activity may be synergic or antagonist of estradiol. In the first case, progesterone has generally a secretory activity upon the target cells "prepared" by estrogens, in particular the cells of the female genital tract (uterus, breast). In the second case, progesterone may be an antiestrogen not only at the level of genital cells but also at the level of other receptors such as vascular system, etc. If an unbalance occurs in the quantitative and chronological ratio between estradiol and progesterone production (which determines the number of target cells and their secretory activity), a cellular dysplasia may be observed. This unbalance is generally characterized by an insufficiency in progesterone secretion which may be physiological (puberty, menopause) or pathological. Such a progesterone insufficiency may play a role in the pathogenesis of genital cancers (endometrial and possibly mammary). Experimental data which give some support to such an hypothesis are reported. They emphasize the importance of treatment of the progesterone insufficiency of the premenopausal period in women with the aim of preventing cellular dysplasia of the female genital tract.
