[Early carcinoma of the gastric remnant: pathological features relating to the interval between the initial and second surgery].

Twenty cases of early carcinoma of the gastric remnant have been treated at our hospital and examined clinico-pathologically. In 11 cases, the interval after the initial gastrectomy, due to the gastric carcinoma, was done was short compared with length of the interval of 9 cases after a gastrectomy for a benign gastric disease. There were two types of gastric remnant carcinomas in cases in which the interval extended 10 years. The first type developed at the site of the gastro-intestinal anastomosis, especially when the anastomosis was found to be joined by the Billroth II method, and the carcinoma was associated with a gastritis cystica polyposa (GCP). The other type showed a polypoid appearance except for the stomal site.

Weber-Christian disease associated with multiple perforations of the ileum and colon.

Panperitonitis developed in a 57-yr-old Japanese man during the course of Weber-Christian disease (WCD). Emergent operation disclosed three perforations of the ascending colon and two of the distal ileum. Histological study of the surgical specimen showed that all perforations were located at the centers of acute subserosal panniculitic foci. No vasculitis was discerned. In the previously reported three cases of WCD with intestinal perforation, perforation was solitary and seen in the small intestine. The cause of perforation was considered to be vasculitis in two cases. In the remaining one, panniculitis around the perforation was in chronic granulomatous stage. This was the first case of WCD in which acute subserosal panniculitis caused multiple ileocolonic perforations.

[A papillary-cystic neoplasm of the pancreas, a calcifying-inverted variant].

A calcifying-inverted variant of a papillary-cystic neoplasm (PCN) is reported, presenting presented the same histological and cellular features, and similar immunohistochemical characteristics and the ultrastructure of a papillary-cystic neoplasm of the pancreas (PCN) that usually afflicts younger women. It took the form of a partiality of a capsule, a common trait for a PCN, and proliferated on infiltrating the pancreatic parenchyma. The case involves a 51-year-old woman, whose PCN was detected by an abdominal X-ray that showed a calcified shadow in a health examination, though there was no palpable abdominal tumor. Usually, a PCN is detected by palpation tumor. Thus, the author demonstrates the existence of PCN variants, characterized by intrapancreatic infiltrating proliferation and an incomplete cyst formation or intratumoral calcified foci, and reports on variants of the usual PCN.

[Histogenesis of gallbladder cancer with special reference to metaplastic changes and distribution of various mucins and CEA].

In order to study the histogenesis of gallbladder cancer, metaplastic changes and dysplasia in the mucosal epithelium were investigated in 30 cases of gallbladder cancer and 300 cases of chronic cholecystitis. Intestinal metaplasia was observed more frequently in the cases of cancer, both in cancerous and non-cancerous tissues, than those of chronic cholecystitis. In addition, CPS III type of mucin, which is preferably demonstrated in the pyloric glands, was observed in the tumor cells of 50% of cancers. Thus, gastric metaplasia as well as intestinal metaplasia seems to be important as a predisposing lesion to gallbladder cancer. By means of reconstruction method carried out on the specimens of cancer, multifocal gradual transition among metaplasia, dysplasia and cancer was observed and dysplasia is an important step in cancer development. As for mucin secretion, the rate of sialomucin-containing cells was notably high in the lesions of dysplasia and cancer, increasing in intensity in this order, accompanied with positive CEA. The results of the present study support the hypothesis that cancer arises from such pre-existing mucosal lesions as metaplasia and dysplasia.

Intraoperative irrigation and primary resection for obstructing lesions of the left colon.

The treatment of choice for left sided colonic obstruction is still controversial. We tried primary resection and anastomosis with the aid of intraoperative irrigation of the dilated proximal colon. Twenty-one cases of obstructive carcinoma of the left colon and four cases of sigmoid colon volvulus were managed using this approach. After the antegrade or retrograde intraoperative irrigation, primary resection and anastomosis without colostomy was performed in all 25 cases. There was only one case with anastomotic leakage. The satisfactory results of our present study and of the previous reports of intraoperative irrigation lead us to recommend intraoperative irrigation and primary anastomosis for left sided colonic obstruction except when the general condition of a patient is very poor or when the prognosis is expected to be poor due to the advanced spread of carcinoma.

[An immunohistochemical analysis of extramammary Paget's disease compared with mammary Paget's disease].

Fourteen cases of extramammary Paget's disease and five cases of mammary Paget's disease have been studied using stereomorphometric, histochemical, and immunohistochemical methods. The antibodies were anti-CEA, anti-EMA, 115D8 and DF3. Intracytoplasmic mucin was positive in Paget's cells, and both 115D8 and DF3 were detected intensely in all of Paget's cells. 115D8 and DF3 were also positive in the normal apocrine sweat gland, the intercellular canaliculi of the eccrine sweat gland, and the intraductal carcinoma of the breast. Therefore, we have concluded that extramammary Paget's disease is an intraepidermal adenocarcinoma originating in the apocrine sweat gland.

[A case of signet-ring cell type cancer of the colon].

A rare case of a primary signet-ring cell carcinoma of the sigmoid colon in a 66-year-old man is reported. Endoscopically, a sessile polyp, 2 cm in diameter, was found in sigmoid colon, consisting mainly of a well differentiated adenocarcinoma and, partially of a signet-ring cell carcinoma. A residual polyp was found at the same site in the sigmoid colon, sixth months after surgery. The resected polyp was histologically confirmed as being a signet-ring cell carcinoma, which had infiltrated into submucosal space by scirrhous invasion. After a polypectomy, a partial colectomy was performed and no carcinoma cells have been identified in the resected colon. We have speculated that this signet-ring cell polyp could develop into Borrmann type 4 advanced colon cancer.

[Histochemical analysis of uterine cervical adenocarcinoma with reference to mucosubstances and distribution of CEA, CA125 and CA19-9].

Epithelial mucin and immunohistochemical localization of carbohydrate antigens (CA125, CA19-9) in normal endocervical glands and adenocarcinoma of uterine cervix were examined histochemically. The materials were obtained from 33 surgical cases with adenocarcinoma and 9 control cases. Serial sections were examined by the following procedures: 1) Digestive PAS reaction, 2) High iron diamine-Alcian blue pH 2.5 staining, 3) Modification PAS and Thionin Schiff reaction to differentiate sialic acid, 4) Immuno-peroxidase method. Histologically, these adenocarcinomas were classified into four subtypes, endocervical, intestinal, endometrioid and serous adenocarcinoma. Endocervical adenocarcinomas were differentiated into the endocervical-intestinal group and serous-endometrioid group by mucin profiles and distribution of carbohydrate antigens. In endocervical and intestinal types, sialomucin was predominant, this mucin being characterized by O-acetylated sialic acid. Immunohistochemically, CA19-9, CA125 and CEA were localized in this group. The latter group of tumors were serous and endometrioid adenocarcinomas, characterized by surface coat type sulphomucin and non-acetylated sialic acid, but CA19-9 and CA125 were not detected in these cases. Adenoma malignum was classified in endocervical type adenocarcinoma.

Specular microscopic findings of lattice corneal dystrophy.

A Japanese family with 4 members suffering from lattice dystrophy of the cornea is reported. Their central corneas were affected leaving the peripheral zone clear. Corneal lesions were found in the superficial stroma of younger cases and in the deeper stroma of older cases. The proband was a 75 year-old woman who underwent a penetrating keratoplasty. Her corneal specimens revealed amyloid deposits stained with Congo red in the stroma. In specular microscopy, the probands central cornea had a crateriform appearance which had not been reported in the literatures. A few fine short branching lines and crateriform lesions were shown in the superficial stroma of the proband's daughter and only crisscrossing lines could be seen in her granddaughter's cornea.

Peutz-Jeghers polyposis associated with carcinoma of the digestive organs. Report of three cases and review of the literature.

Three cases of Peutz-Jeghers polyposis with carcinoma of the digestive organs are studied. Although mucocutaneous pigmentation was not present in two of the three patients, the features of intestinal polyposis are consistent with those of Peutz-Jeghers syndrome. One patient had a carcinoma of the pancreas and the other two had carcinomas with colonic Peutz-Jeghers polyps. Previous reports on carcinomas associated with Peutz-Jeghers syndrome are reviewed. An unusual location in the gastrointestinal tract, together with occurrence at an early age, characterize the carcinoma in Peutz-Jeghers syndrome. In Japanese patients, the large bowel is the site of the greatest number of carcinomas. On the other hand, Western patients showed a relatively even distribution. A possible surveillance protocol for early detection of gastrointestinal carcinoma in patients with Peutz-Jeghers syndrome is discussed.

Metastatic calcification in the stomach demonstrated by a bone scan in Bence Jones lambda myeloma.

A 53-year-old male with Bence Jones lambda myeloma developed hypercalcemia and acute renal failure (calcium 14.4 mg/dl, BUN 40 mg/dl, creatinine 3.0 mg/dl) after initial response to chemotherapy. A 99mTc-MDP bone scan revealed unusual isotope accumulation in the left hypochondrium. Extensive calcium deposition was confirmed in the gastric mucosa in the postmortem examination. Detection of gastric calcification by a bone scan is very rare. Only two cases of gastric calcification visualized on bone scans can be found in the literature, one with multiple myeloma) and one in Hodgkin's disease).

Colonoscopic polypectomy as a tool for management of colonic polyps and detection of new lesions.

Over the past 14 years 1,000 polyps were removed via colonscope and histologically examined. There were 869 neoplastic polyps including 125 carcinomas. Non-neoplastic polyps included 32 inflammatory polyps, 32 metaplastic polyps, 27 juvenile polyps and 17 Peutz-Jeghers type polyps. Ninety-eight focal carcinomas were adequately treated by polypectomy only, whereas 9 out of 26 invasive carcinoma required further operations with one residual carcinoma in the pararectal tissue and colonic wall respectively. The criteria requiring additional surgery in invasive carcinoma are 1) lymphatic permeation of the submucosa, 2) poorly differentiated carcinoma or 3) massive invasion close to the cut end. Not only polypoid adenomas but also flat adenomas were found to exist and seem to play an important role in the adenoma-carcinoma sequence. It became clear that colonscopic polypectomy was a useful tool for the management of colonic polyps.
