In vitro lecithinase activity and sensitivity to 22 antimicrobial agents of Clostridium perfringens isolated from necrotic enteritis of broiler chickens.

Viable Clostridium perfringens ranging from 10(5) to 10(8) g-1 was detected in all of 88 intestinal content specimens of necrotic enteritis in broiler chickens. In vitro lecithinase activity and sensitivity to 22 antimicrobial agents were determined for the 88 isolates. The activities of lecithinase in the culture filtrate of isolates were estimated to be 0.5 to 4.0 AE ml-1 as alpha-antitoxin equivalent. With reference to antimicrobial activity penicillins and cephazolin showed excellent activity and no resistance; peptides, of the agents used as growth promoters, showed that all except bacitracin had low minimum inhibitory concentration levels (1.6 micrograms ml-1 or less) against this organism; polyethers of monensin, salinomycin and lasalocid were generally adequate in low concentrations while there was a high level of resistance to three tetracyclines in 90 per cent of the strains and all isolates were insusceptible to streptomycin of the aminoglycoside antibiotics.

Morphological clues for the diagnosis of small hepatocellular carcinomas.

Histological features of 44 cases of small hepatocellular carcinoma (HCC) were examined and compared with those of large regenerative nodules. The highly differentiated type of HCC most often occurred in nodules which were less than 2 cm in diameter. Noticeably, in 9 out of 15 such cases (60.0%), tumour cells were arranged in trabeculae of almost normal thickness (normotrabecular pattern). These trabeculae, however, showed variable nuclear crowding, occasional microacinar formation, and increase in cytoplasmic basophilia. It is emphasized that the presence of this triad may be a very reliable indicator for the histological identification of early HCC, especially in examining limited material such as a biopsy specimen. However, cellular and structural atypia becomes more prominent in nodules which are larger than 2 cm.

Combined hepatocellular and mucinous carcinoma.

A rare autopsy case of combined liver cell and bile duct carcinoma (CLBC) occurring in a 51-year-old male with alcoholic liver cirrhosis is presented. Histologically, while the primary lesion was solely composed of well differentiated hepatocellular carcinoma (HCC), intrahepatic metastases consisted of a variable admixture of HCC and cholangiocarcinoma (CC) with excessive mucin production. Interestingly, the tumor cell cluster showing a trabecular growth pattern produced both bile and mucin, thus converting from HCC to mucinous CC. It is concluded that this liver malignancy is principally HCC with a marked tendency to transform into CC. The importance of the findings, especially the simultaneous production of bile and mucin within the same cell cluster, is emphasized in terms of the classification of CLBCs.

Pathologic features of small hepatocellular carcinoma.

Twenty-one nodules of small hepatocellular carcinoma (HCC) were examined. Histologically, the nodules often presented formation of plump trabeculae, marked nuclear atypism, or aggressive growth comprising capsular invasion, vascular invasion, and replacement of adjacent pseudolobules. Aside from these characteristic findings of HCC, it was important to reveal the following features for the diagnosis of well differentiated type of small HCC: variable thickening or distortion of trabecular structure in association with nuclear crowding, acinar formation, selective cytoplasmic accumulation of Mallory bodies, nuclear abnormalities consisting of thickening of nucleolus, hepatic cords in close contact with bile ducts or blood vessels, and hepatocytes growing in a fibrous environment. During the invasive growth, the tumor cells may well be subtly blended with benign hepatocytes, giving rise to a pattern of "mixed cellularity". It is also emphasized that connective tissue septa of pseudolobules could be a route of rapid tumor spreading.

Diagnosis of small hepatocellular carcinoma: correlation of MR imaging and tumor histologic studies.

Magnetic resonance (MR) images of the liver were used to study 43 patients with relatively small hepatocellular carcinomas (HCCs) and 36 with other hepatic mass lesions. In 27 HCC patients, histologic findings were available. All focal lesions detectable by CT without contrast media were delineated with greater contrast by MR imaging. The rate of detection depended on tumor size, being 97.5% for HCCs greater than 2 cm in the longest axis and 33.3% for those less than 2 cm. MR imaging demonstrated the ring sign characteristic of encapsulated HCC twice as frequently as CT scans. Inversion recovery (IR) images depicted the internal structure of the HCC better than T2-weighted spin-echo images. Lesions were classified into four patterns of intensity: low, iso, high, and mixed. The latter three were relatively characteristic of HCC and related closely with steatosis of cancer tissue. HCCs with fibrosis tended to have long T1 values; those with steatosis had short T1 values. T1 and T2 relaxation times were useful in the differential diagnosis.

[A study of constructional activity in patients with cerebrovascular disorders].

Three experiments were conducted to examine the differences of constructional disabilities between left and right hemiplegic patients with cerebrovascular disorders. In Exp. I, 32 left and 30 right hemiplegics were given the Block Design Test. In Exp. II, the Visual Cognition Test, consisted of 10 subtests to measure the abilities which were presumed to be essential for the process of block design construction, was performed. In Exp. III, the patients with constructional disabilities were given various aids to facilitate the constructional activities. The main results were as follows: Constructional disabilities were more frequent and severe in left than right hemiplegic patients. There were qualitative differences in construction between the two groups. The factors underlying constructional disabilities were responsible to visuoperceptual disorder and programming disorder of behavior for both groups. However, visuoperceptional disorder in left hemiplegics and programming disorder in right hemiplegics manifested relatively closer relation to constructional disabilities than others.

[Two cases of Kallmann's syndrome].

Herein we report two cases of hypogonadism with anosmia or hyposmia (Kallmann's syndrome), a 23-year-old single man (case 1) and a 34-year-old single man. (case 2). Micropenis and small testes were seen in both cases. Central anosima and central hyposmia were found in cases 1 and 2, respectively. Their karyotypes were normal (46, XY). The plasma levels of LH and FSH were low and the reaction to LH-RH test was poor in both cases. After repetitious LH-RH administrations, a good response of plasma LH was recognized in both cases except for a poor response of plasma FSH in case 1, while plasma testosterone remained low in both cases. Their testicular biopsies revealed immature testes without any developed Sertoli and Leydig cells.

Survival of patients with tumors of the renal pelvis and ureter: a retrospective study.

A review of 27 patients with renal pelvic tumors and ureteral tumors admitted to the Tsukuba University Hospital was performed retrospectively. Nineteen cases of renal pelvic tumors and eleven cases of ureteral tumors were submitted to the study. These patients were treated mainly by surgery. The rates for 1-year, 3-year, 5-year and 10-year survival were 88.5%, 57.6%, 40.3% and 20.2%, respectively. No obvious relationship between prognosis of renal pelvic tumor and that of ureteral tumors was seen. Pathologically, there was a correlation between the grade and the stage of the tumor. Prognosis was related to the stage of the tumor, but not to the grade of the tumor. Clinically, the patients with positive urinary cytology had a poorer prognosis. It was considered that no relationship existed between the preoperative clinical stage and the postoperative pathological stage. Consequently, the former is not considered a prognostic factor. We have been performing mainly radical nephroureterectomy with regional lymphadenectomy since 1975. The patients who received the operation with regional lymphadenectomy since 1975. The patients who received the operation with regional lymphadenectomy or with adjuvant chemotherapy had a better prognosis. It is concluded that lymphadenectomy is a valuable operation for improving prognosis.

[The effects of cue stimuli on the reaction times of mentally retarded children: an examination of the temporal factor and the spatial factor (author's transl)].

To investigate the covert behavior performed by retardates during the warning intervals of simple RT task, cue stimuli consisting of a combination of the mode of the temporal presentation (regular vs. irregular) and the mode of spatial presentation (regular vs. irregular), were introduced in the warning interval. Retarded and normal children performed under these conditions. For retardates the spatial regularity was more effective than the temporal regularity. For normals there was little RT difference among the four experimental conditions. Retardates had minimized RT under the condition in which the cue stimuli were presented regularly in the spatial order. These results suggested the difference in the covert behavior performed by retardates and normals.

[The effects of feedback signal and model signal on heart rate changes (author's transl)].

An attempt was made to modify heart rate by presenting the Ss: E1-visual feedback and model signals, E2-feedback signals, E3-model signals, or C-no signals as a control group (6 Ss in each group). Model signal was a weak light which flickered at the rate of 20% above or below the pre-stimulus base IBI. All groups were required to either raise or lower HR alternately and E1 group was instructed to synchronize the feedback signal with the model signal. Significant bidirectional HR changes were found in 3 experimental groups and most effective changes were obtained in E1 group. In the second study, extended training to one S with E1 condition resulted in HR increases of 16 bpm and decreases of 9 bpm.
