Measurement of secretin in the mucosa of digestive tract by radioimmunoassay.

Radioimmunoassay for measurement of mucosal secretin revealed that HCl was more efficient than distilled water for the extraction of secretin from canine duodenal mucosa. The rapid degradation of secretin within untreated mucosal specimens observed at room temperature could be effectively prevented by the storage at 4 degree C. In dogs, immunoreactive secretin distributed most abundantly in the upper and middle portion of the duodenum with a tendency to decrease gradually to the anal side. Secretin was not detected, however, in the esophagus, stomach, lower ileum, colon, liver and pancreas. The effect of intraduodenally instilled 0.1 N HCl on plasma and mucosal secretin was also investigated in conscious dogs. Plasma secretin rose promptly after the start of infusion and reached the maximum at 10 min, while mucosal secretin responded later than plasma secretin.

Plasma secretin level and exocrine pancreatic secretion in response to intraduodenal infusion of 1-phenyl-1-hydroxy-n-pentane and HCl by using a new four lumen tube.

We have designed a four lumen tube which has enabled us to aspirate duodenal juice while infusing endogenous stimuli into the lower part of duodenum. Exocrine pancreatic secretion and plasma immunoreactive secretin (IRS) levels have been simultaneously observed in response to intraduodenal infusion of 1-phenyl-1-hydroxy-n-pentane (PHP) and 0.1 N HCl in normal subjects, patients with diabetes mellitus and patients with chronic pancreatitis by use of this tube. (1) Pancreatic flow rate and bicarbonate concentration were found to increase to twice those of basal levels and plasma IRS levels rose significantly after infusion of PHP or HCl in normal subjects. The same tendency was observed in patients with diabetes mellitus. (2) Exocrine pancreatic secretion did not change after infusion of PHP or HCl in patients with chronic pancreatitis. Pancreatic flow rate, bicarbonate concentration and amylase output were significantly low in patients with diabetes mellitus. (3) Pancreatic flow rate and bicarbonate concentration incresed to about twice those of basal levels after both PHP and HCl infusion. On the other hand, although pancreatic amylase output increased significantly after infusion of HCl as compared with basal secretion, it did not change after infusion of PHP. It appeared reasonable to conclude that PHP is a morespecific substance for releasing secretin than HCl.

Rheumatoid arthritis with systemic necrotizing arteritis.

An autopsy case of rheumatoid arthritis with active polyarthritis, systemic necrotizing arteritis, pleuritis, pericarditis, rheumatoid nodules in a few organs and a healing gastric ulcer was reported. Histologically, systemic necrotizing arteritis was characterized by vascular changes of the following three types: Granulomatous arteritis with a characteristic arrangement of mesenchymal cells forming a palisade around coagulation necrosis of media and some of them formed a rheumatoid nodule-like lesion in the wall (RA type); Fibrinoid arteritis very similar to the Kussmaul-Maier type periarteritis nodosa (PN type); and chronic arteritis with endarterial proliferation (Ep type). Although it is hard to distinguish arteritis of PN type from the Kussmaul-Maier type periarteritis nodosa, arteritis of RA type with rheumatoid nodule-like lesion in the wall may be interpreted as an extremely developed form of vasculitis in rheumatoid arthritis.

Viral placentitis - a case report.

A case of viral placentitis, in a 20 year-old pregnant woman suffering from chronic glomerulonephritis, was presented. In light microscopic findings of the placenta, many necrotic foci with intranuclear inclusion - bearing cells revealed in chorionic villi and by electron microscopy numerous immature and mature virus particles, probably herpes simplex were proved in intranuclear inclusion-bearing cells.

Experimental rheumatoid arthritis-like features induced by prolonged sensitization with focal antigens.

Prolonged sensitization with emulsion of an autologous or isologous subcutaneous abscess of Arthus type induced by injection of hen egg-white was performed in 34 rabbits which were divided into (high responder and intermediate responder groups (H- and M-groups) according to individual difference of immune responses. The development of a rheumatoid factor-like substance (RFLS) was demonstrated after 30 experimental days and subsequently observed in 21 out of 33 rabbits. There was no significant difference in the incidence of RFLS between both groups. As to the relation between the development of RFLS and types of focal antigens, the group of the autologous W-substance showed a higher incidence of RFLS than the N-substance. Acute and/or chronic synovitis was demonstrated in 13 of 33 rabbits and inflammatory changes were more intensive and extensive in the later period of experiment. Presence of RFLS in the affected synovial tissues, chiefly in the cytoplasm of plasma cells and mononuclear cells, occasionally in a free state was revealed by immunofluorescent study, and depositions positive for IgG and beta 1C were observed in the wall of blood vessels and fibrinous thrombi in the affected synovial tissues.

Fibrous long spacing type collagen fibrils in the glomeruli of experimental amyloidosis in rabbit.

Fibrous long spacing type collagen fibrils were observed in the glomeruli of experimental amyloidosis induced by either injections of casein or prolonged sensitization of Arthus type tissue antigens in the rabbit. These fibrous long spacing type fibrils were admixed with amyloid fibrils which accumulated both in nodular and diffuse deposition, but they were observed also in irregularly thickened capillary basement membranes and mesangial areas, where amyloid did not deposit. The periodicity of these fibrous long spacing fibrils was approximately 1200 , and nine symmetrical intraperiodic bands were obviously discerned within each period.
