[Campylobacter pylori and diseases of the stomach].

In spite of extensive clinical studies the pathogenicity of Campylobacter pylori in various diseases of the stomach is still not fully elucidated. In peptic ulcer Campylobacter pylori has not provided a reasonable explanation for the localized and solitary development of ulcer lesion which is one of characteristic features of this clinical entity. Our previous clinical study revealed that relapse occurs mostly at or near the scar of the preceding ulcer lesion and occurs more often in red scar than in white scar in peptic ulcer. Therefore, the scar was chosen to investigate whether there is a correlation between Campylobacter pylori and relapse. Twenty patients with healed gastric ulcer were biopsied in duplicate at 4 different sites, namely at the center of and near the scar of the preceding ulcer lesion, in the body and pyloric antrum. One of the duplicate specimen was stained, and the other cultured. Number of the colony observed on the plate was highest at the scar center followed by near the scar or in the body, and the pyloric antrum showed the lowest. Campylobacter pylori was detected in 46.7% of red scar, and 20% of white scar. Campylobacter pylori may play a pathogenic role in some patients of peptic ulcer, but not in all. Namely Campylobacter pylori is one of factors which may cause in imbalance between offensive and defensive powers at gastric mucosa and may lead to development of ulcer lesion. In case of positive bacteriological test, therefore, as to presence of Campylobacter pylori, the preventive measure for relapse such as bismuth preparation may be considered.

[Correlation between gastric emptying time and both plasma gastrin and pancreatic polypeptide in streptozotocin diabetic dogs].

Correlation between gastric emptying time and both plasma gastrin and pancreatic polypeptide (PP) levels after meal was studied in 9 normal and 5 streptozotocin (STZ) diabetic dogs. Moreover, the effects of exogenous insulin at doses of 0.2 and 0.4 U/kg on gastric emptying and plasma hormone levels were also studied in STZ diabetic dogs. The time required to reach the peak of plasma acetaminophen level, which is the indirect indicator of gastric emptying rate, was significantly delayed in STZ diabetic dogs than in control dogs. Plasma gastrin response was delayed slightly in STZ diabetic dogs, but the plasma levels were not significantly different between the two groups. Plasma PP levels both in the basal and after postprandial state were significantly higher in STZ dogs than in normal dogs. Infusion of insulin did not affect plasma levels of acetaminophen and gastrin, while it produced a dose-dependent suppression of postprandial PP rise in STZ diabetic dogs. The foregoing data indicate that high level of plasma PP observed in STZ dogs did not correlate with gastric emptying but with insulin deficiency.

The effects of cadmium, copper or zinc on formation of embryonic chick bone in tissue culture.

Femurs from 9-day-old chick embryo were cultivated for 6 days by the roller-tube method in the presence of Cd, Cu or Zn. Cd (5.0 microM and above) and Cu (2.5 microM and above) caused a decrease in collagen content of both diaphysis and epiphysis, mainly due to inhibition of collagen synthesis. In addition, Cd and Cu each showed a tendency to inhibit an increase in Ca content of diaphysis, where intraperiosteal ossification could be observed. Alkaline phosphatase (ALP) activity was decreased by Cd (5.0 microM and above) or Cu (10 microM and above) in diaphysis. On the other hand, Zn at 50 microM and above inhibited an increase in Ca content of the diaphysis with a remarkable elevation of ALP activity in the medium. At this time, Zn did not decrease the collagen content of the diaphysis so strongly. Histological observations revealed that Cd and Cu each decreased both calcified and uncalcified osteoid tissue at 2.5 microM, while Zn at 100 microM decreased calcified tissue but increased uncalcified osteoid tissue. As Zn accumulated particularly in diaphysis and deposited at the edge of calcified tissue, it was suggested that Zn inhibited calcification physicochemically. It was concluded that Cd or Cu would induce bone damage represented by osteoporosis, whereas Zn would induce osteomalacia.

Role of zinc in protection against cadmium-induced toxicity in formation of embryonic chick bone in tissue culture.

To clarify a possible mechanism of zinc (Zn)-induced tolerance to cadmium (Cd) toxicity on bone formation, femurs from 9-day-old chick embryos were cultured for 6 days by the roller-tube method in the presence of Cd (2, 4 or 9 microM) and/or Zn (60 microM). Zn prevented a decrease in bone growth caused by Cd at 4 and 9 microM. An increase in calcium (Ca) content of diaphysis was inhibited by Zn in both the presence and absence of Cd. Histologically, Zn protected a Cd-induced degenerative change of mesenchymal cells in the periosteum and that of osteoblasts around the trabecula at each Cd level. At 60 microM Zn, Cd accumulated less in the bone at 2 microM but more at 9 microM. From these results, it was concluded that Zn prevented Cd-induced toxicity in the process of ossification except calcification in a culture system by two different mechanisms, i.e. a decreasing Cd accumulation at a low level of Cd and probably an induction of metallothionein (MT)-like protein at a high level of Cd.

Plasma paracetamol concentrations measured by fluorescence polarization immunoassay and gastric emptying time.

Radionuclide labeled gastric emptying was compared with plasma paracetamol (acetaminophen) concentration measured by fluorescence polarization immunoassay. A significant correlation was found between both gastric emptying half-time of radionuclide (t1/2) and % isotope remained in the region of the stomach and paracetamol absorption kinetic parameters. The strongest correlation was found between t1/2 and the time required to reach the peak plasma level of paracetamol (r = 0.680, p less than 0.01). This study indicates that simple paracetamol marker method has enabled an indirect quantitative measurement of gastric emptying by sampling at varying time intervals.

Quantitative study on phagocytosis of human tonsillar adherent cells by the flow cytometric and immunocytochemical methods.

An in vitro phagocytosis assay of adherent cells of 12 palatine tonsils (7 children's and 5 adults' cases with persistent angina) was performed by use of flow cytometry (FCM), using fluorescent latex particles (FLP). The mean value of the percentages of phagocytic cells in the children's group (81.4%) was higher than that in the adults' group (64.0%). In addition, separate immunocytochemical stainings with each antibody--Anti-Leu-M5, OKDR, OKT6, S-100, and lysozyme--were made on smear preparations of a child's and an adult's tonsillar adherent cells after incubation with FLP. By use of the light microscope, the percentages of phagocytic cells in relation to positive cells for each antibody were calculated. The lysozyme positive cell proved to have the highest percentage of phagocytic cells. Then, the average number of phagocytized FLP in one positive cell for each antibody were calculated. With regard to the lysozyme positive cell, this cell proved to have the highest average number of FLP.

Effect of trimoprostil on gastric secretion in man.

The effect of trimoprostil, an analogue of prostaglandin E2, on gastric secretion was studied in healthy volunteers in comparison with that of cimetidine. Graded doses of 50, 250, 500 and 750 micrograms were given p.o. for trimoprostil and 200 mg for cimetidine. Basal secretion was significantly depressed at doses larger than 500 micrograms of trimoprostil but not by 200 mg of cimetidine probably due to the method of gastric analysis adopted in the study. In amyl-oxy-carbonyl (AOC)-tetragastrin stimulated gastric secretion, 500 micrograms of trimoprostil depressed acid output significantly, but did not affect pepsin output which was depressed, however, at 750 micrograms. On the other hand cimetidine markedly inhibited AOC-tetragastrin stimulated acid and pepsin secretion. Some difference was also recognized between trimoprostil and cimetidine concerning the mechanism of inhibitory action. Serum concentration of trimoprostil measured at 60 min after oral administration varied dose-dependently and antisecretory effects were also found to be dose-related. Serum gastrin and pepsinogen I were not affected either by trimoprostil or cimetidine. These results indicate that trimoprostil showed inhibitory effect on gastric secretion at doses larger than 500 micrograms for acid, and at doses larger than 750 micrograms for pepsin.

Disseminated visceral giant cell arteritis.

Autopsy findings of disseminated visceral giant cell arteritis in a 78-year-old man who was in chronic renal failure for two years were presented. Arteritis had not been suspected clinically and definite diagnosis was made after the autopsy. Only small arteries and arterioles were involved. The arteritis was characterized by granulomatous lesion with exclusively multinucleated giant cells. Fibrinoid necrosis with polymorphonuclear leukocyte infiltration was found only in one arteriole of the stomach. The arteritis was distributed in the heart, lungs, kidneys, gastrointestinal tract, liver, gallbladder, pancreas, prostate, and lymph nodes, but not in the aorta and brain. This peculiar type of arteritis was reported only by LIE and the entity was designated as disseminated visceral giant cell arteritis.

Correlation between gastric emptying time and the response of pancreatic polypeptide to test meals in peptic ulcer diseases.

In the present study, pancreatic polypeptide (PP) levels both in basal state and after stimulation by meal were determined in 21 controls, 28 patients with gastric ulcer and 21 patients with duodenal ulcer and their correlation with gastric emptying was examined. In patients with gastric and duodenal ulcers, basal levels as well as stimulated of plasma PP during 95 min of observation period were higher than those in controls except for the values at 5 min. As no correlation was seen between integrated PP response (IPPR) to the meal and T1/2 for gastric emptying in any of three groups, it was indicated that gastric emptying has only a minor role in postprandial PP response.

Effects of endogenous and exogenous secretin on plasma pancreatic polypeptide concentrations in dogs.

The effects of exogenous and endogenous secretin with or without intravenous glucose infusion upon islet hormone secretion were studied in four conscious mongrel dogs fitted with a duodenal fistula. Intravenous infusion of secretin for 1 h at doses of 0.5 and 4 U/kg raised plasma secretin concentrations to physiological and pharmacological levels respectively, without affecting plasma insulin and pancreatic polypeptide concentrations. In contrast, bolus injections of secretin at high concentrations produced significant increases of plasma insulin at 0.5 U/kg and 4 U/kg and of pancreatic polypeptide at 4 U/kg. Plasma glucagon did not change during intravenous infusion of low dose secretin (0.5 U X kg-1 X h-1), but decreased during infusion of 4 U X kg-1 X h-1 or bolus injection of secretin (0.5 U/kg). Intravenous infusion of glucose together with secretin (0.5 U/kg and 4 U/kg) did not affect plasma insulin, glucagon, or pancreatic polypeptide levels significantly compared with the changes caused by glucose infusion alone. Intraduodenal instillation of HCl, which produced plasma secretin concentrations similar to those evoked by intravenous infusion of secretin (4 U X kg-1 X h-1), led to a rise in plasma pancreatic polypeptide. It is concluded that the stimulatory effects of secretin on insulin and pancreatic polypeptide and the inhibitory effect on glucagon are pharmacological, and that increase of plasma pancreatic polypeptide after intraduodenal infusion of HCl is not mediated by endogenous secretin.
