Acute cardiomyopathy with recurrent pulmonary edema and hypotension following heroin overdosage.

An 18-year-old man developed acute pulmonary edema following heroin overdose. Two days after initial improvement, there was recurrence of hypotension and pulmonary edema with severe hypoxemia refractory to mechanical ventilatory support utilizing positive and end-expiratory pressure. Cardiac catheterization revealed elevated pulmonary capillary wedge pressure suggestive of left ventricular failure. The use of digitalis and diuretics resulted in prompt clinical improvement and ultimate recovery. Evidence is presented indicating that this patient represents an uncommon but important syndrome of acute cardiomyopathy with left ventricular failure which complicates the clinical course of certain cases of heroin overdose. Its physiologic diagnosis is of obvious importance in the choice of proper therapy, thereby increasing the patient's chances of recovery.

Emergence of overt diabetes in offspring of rats with induced latent diabetes.

A single subdiabetogenic dose of alloxan administered to the weanling rat induces a persistent state of latent diabetes which progresses to fasting hyperglycemia by the seventh generation. Initial descendants of alloxan-treated animals have hyperinsulinism which progresses to insulinopenia in later generations. Later generation animals develop ketoacidosis when challenged with a dose of alloxan that has no effect on control animals. The significant sex difference in glucose tolerance rates disappears as the animals become more diabetic and decreased fertility and parity become apparent. One explanation for this data remains the hypothesis of paramutation, induced by alloxan, affecting regulator gene activity. Light microscopy of diabetic animals shows no pathology.

Variable X chromosomal abnormalities in patients with stigmata of Turner syndrome.

Four cases with suspected sex chromosomal abnormalities and clinical features and endocrine data typical of Turner syndrome are presented. Chromosome preparations made from skin fibroblasts and peripheral blood and multiple banding techniques employed to map the genes of their X chromosomes showed variable results. A review of the literature also revealed conflicting findings regarding the mapping of genes on the X chromosome. Despite different cytogenetic findings, the clinical features of the four cases presented were quite similar.

Alterations in plasma amino acid levels in children with asthma: a preliminary investigation.

Concentrations of plasma amino acids and cortisol were measured in 27 children with status asthmaticus, moderate, or mild asthma and in 7 controls without lung disease. Individuals with conditions potentially altering amino acid levels were excluded. Measurements were made at 8 A.M. and 4 P.M. on the same day. Of the 24 amino acids analyzed, threonine, serine, citrulline, tyrosine, proline, alanine, cystine, isoleucine, ornithine, lysine, and arginine were decreased at 8 A.M. or 4 P.M. and plasma cortisol was elevated at 4 P.M. in patients with status asthmaticus when compared with the other groups (p less than 0.05). Normal diurnal variations of amino acids and cortisol were preserved in all groups. Sequential daily determinations in eight patients with status asthmaticus showed progressive increases in total and many individual amino acids as symptoms subsided. These studies demonstrate a decrease of certain gluconeogenic amino acids in the plasma in status asthmaticus. This diminution could reflect the need for maintenance of normal glucose concentrations in status asthmaticus. The decrease in gluconeogenic amino acid levels may play a role in the severity of asthma.

A comparison of outcome scores in os calcis fractures.

BACKGROUND: Of over 20 different scoring systems to evaluate outcome following calcaneal fracture, the Maryland Foot Score has broad current acceptance. A general health survey, the Short Form 36 (SF 36) has also been used. AIMS: The study compared two scoring systems for assessing the outcome of open reduction and internal fixation of displaced intra-articular calcaneal fractures. METHODS: Over a four-year period, 31 displaced intra-articular calcaneal fractures were categorised by the Sanders classification and treated by open reduction and internal fixation. Outcome was assessed by the Maryland Foot Score and by the Short Form 36 (SF 36) general health questionnaire. RESULTS: Thirty-five per cent of fractures were Sanders class 2, 57% were class 3 and 8% were class 4. The average follow-up was 2.5 years (range 1-4 years). Using the Maryland Foot Score, seven fractures had an excellent result, 13 had a good result, six had a fair result and one was a failure. There was a correlation between pain (coefficient = 0.780, p < 0.001) and physical function (coefficient = 0.638, p < 0.001) with the appropriate sections of the SF 36. CONCLUSION: The Maryland Foot Score measures what it attempts to measure and therefore it has content validity for pain and physical function.

Overlooked sources of ethanol.

The case of a 55-year-old female who presented to the emergency department with acute ethanol intoxication and suicidal ideation is reported. After initiating routine management, we discovered that her serum ethanol levels remained persistently elevated as a result of the patient's secretly ingesting mouthwash. This occurred after she was searched and allowed to retain personal hygiene products. Alcohol-dependent patients may consume ethanol products that are not manufactured for ingestion. These products include cosmetics, cough and cold remedies, and personal hygiene products. The ethanol content of these nonbeverage ethanol (NBE) products exceeds that of many conventional alcoholic beverages. Financial constraints and ease of availability are factors leading to their consumption. This report serves as a reminder to be aware of the existence and popularity of NBE in order to avoid potential morbidity and even mortality associated with its use.
