[Problems in the diagnosis and surgical care of thoracoabdominal wounds].

The article presents an 11 years experience in work of a specialized department of thoracoabdominal trauma with an analysis of treatment of 292 wounded. The main causes of the thoracoabdominal wounds (TAW) were knife stabs (96.2% of the patients). The diagnosis of injuries to vital organs of the chest and abdomen is known to be very difficult. The X-ray analysis is one of major diagnostic methods for TAW. Emergency thoracoscopy is of great significance in diagnosis of intrathoracic injuries and laparocentesis and laparoscopy are of great value for intraperitoneal wounds. The sequence of measures and surgery volume must be determined by the wound type and the severity of injuries of the internal organs which gives better results of treatment of such patients. Postoperative complications were noted in 9.1% of the wounded. Lethality was 5.1%.

[The etiology, pathogenesis and clinical picture of nonlactational mastitis].

The authors analyze an experience with treatment of 2750 patients with different forms of nonlactational mastitis. Special attention is given to peculiarities of the clinical course of the disease, importance of early diagnosis of the inflammation in the mammary gland. Pathogenetical treatment of different forms of nonlactational mastitis is substantiated. Dissection of all pathogenetically altered tissues is thought to be necessary in operative treatment of patients with purulent forms with the following histological investigation.

[The possibilities of surgical treatment in injuries to the major blood vessels today].

Under analysis were results of surgical treatment of 47 patients with injuries of the aorta, major arteries and veins. Higher lethality was noted in people with injuries of major veins, especially vena cava. Specific operative methods are described for various kinds of injuries of the vessels of the thoracic and abdominal cavities. Lethality was 25.5%.

[The surgical treatment of liver injuries].

The article sums up an experience with treatment of 342 patients with injuries to the liver at a specialized unit for thoracoabdominal trauma. Under analysis are problems of diagnosis, classification, surgical policy and causes of lethal outcomes.

[Therapeutic choices in spontaneous pneumothorax].

Questions of conservative and operative treatment are considered on the basis of studying 259 patients with spontaneous pneumothorax. In most of the patients the lung was spreaded by a single or permanent active aspiration of air from the pleural cavity. However, due to great amount of recurrences after the conservative treatment it should be more often followed by operative methods of treatment. Indications to operation are: failure of conservative spread of the lung, repeated collapse, recurrent pneumothoraxes and detection of subpleural bullas during thoracoscopy.

[Endogenous toxemia in surgery].

An analysis of the mechanisms of the development of endogenous intoxication was used to consider its role in pathogenesis of most serious surgical diseases and traumas and their complications. The role of endogenous intoxication is shown in the development of multiple organ insufficiency. Methods of extracorporal hemocorrection in treatment of endogenous intoxication are substantiated. Potentialities of the present-day methods of extracorporal hemocorrection and prospects of development are discussed.

[Photomodification of autologous blood in surgery].

The use of the method of autologous blood photomodification in surgery is discussed on basis of generalized experience of the Leningrad Research-Practical Center of Blood Photomodification (3,000 procedures conducted in over 2,000 patients). The current state of the problem, the main mechanisms of the therapeutic action of the procedure, and the clinical efficacy of the method in various surgical diseases are dealt with. The possible complications and the means of their prevention are deal with. The methodological aspects of research are discussed. The authors define the main problems which must be solved in order to raise the efficacy of using photomodification of autologous blood in surgery.

[Rehabilitation treatment of patients with uncomplicated fractures of the spine at a hospital rehabilitation center].

The authors make an analysis of treatment of 188 patients with noncomplicated compressive fractures of the vertebral column in the thoracolumbar part performed at the stationary rehabilitation center. The course of restorative treatment was as long as 31-40 days and included individual and group trainings of exercise therapy, massage, hydrokinesotherapy, thermo-, electro-, photo- and magnetotherapy. Results of the treatment were followed in 81 patients. Excellent and good results were obtained in 43 patients (53%), unsatisfactory--in 7 patients (8.6%). The period of follow-up observation was from 1 month to 1 year.

[Injuries of the heart and pericardium].

The work presents data of 25 patients with wounds of the heart and pericardium. Clinical diagnostics, tactics and results of treatment are described. Attention is called to the importance of the method of thoracoscopy in determination of the character of injuries. To improve results of the treatment of this group of patients, the work of the emergency aid must use resuscitation-surgical teams, admittance of the patients to specialized departments for thoracal traumas.

[Blood reinfusion in injuries of the chest and abdomen].

Reinfusion of blood used in 78 patients with injuries of the chest and abdomen has shown the method to be clinically effective and expedient including treatment of injuries of hollow organs of the abdominal cavity. The technique of blood reinfusion for different injuries is described. Data are presented which allow to suspect that the reinfused blood has curative properties in treatment of injuries of the chest and abdomen.

[Complications of the urachus in adults].

The authors observed 7 patients aged from 19 to 74. Five of them were operated upon. One patient died from anaerobic neclostridial phlegmon of the anterior abdominal wall which appeared after an injury of the not noticed urinary duct. Laparotomic accesses passing aside form the area of possible localization of the urinary duct were more justified. When operating on the paraumbilical area it is expedient to make a revision for the detection of the urachus and associated abnormalities on the anterior abdominal wall.
