[The combination of ulcer disease and pancreatitis in surgical practice].

Results of treatment of 1203 patients, admitted to the hospital for an acute pancreatitis and its aggravation, were analyzed. Earlier 414 (34.5%) patients were operated for ulcer disease (UD), in 126--suturing of perforated ulcer was performed, in 159--selective proximal vagotomy with pyloroplasty, in 129--resection of stomach. After clinical examination UD of stomach was diagnosed in 141 (11.7%) patients, UD of duodenum--in 1062 (88.3%). Diagnosis of pancreatitis and UD was established basing on clinical data, instrumental, biochemical, laboratory, ultrasonic and other methods of investigation. Complex therapy was effective in 998 patients. For destructive pancreatitis 78 patients were operated, of them 9 (11.5%) died. For complication of UD on background of pancreatitis (perforation of ulcer--in 71 observation, hemorrhage--in 39, the gastric outlet stenosis--in 19) 127 patients were operated, 12 (9.5%) died. Application in complex therapy of lymphosorption and hemosorption using xenospleen had promoted the lowering of postoperative lethality.

[Twenty year experience in the treatment of peritonitis, multiple organ dysfunction syndrome in perforative ulcers].

The results of treatment of 573 patients, operated on the clinic for the period from 1981 to 2000 year for perforative gastroduodenal ulcer were analyzed. In 24 (4.2%) of them the necessity of relaparotomy performance for peritonitis, polyorgan insufficiency had occurred. In all patients complex intensive therapy, intestinal decompression, flowing peritoneal dialysis were conducted, in 10--the governed laparotomy. In 9 patients plasmapheresis was performed, in 8--drainage of ductus thoracicus with lymphosorption, in 7--xenosorption. Application of complex treatment equally with active detoxication in patients with peritonitis, polyorgan insufficiency have allowed to achieve recovery in 66.6% of them.

[Surgical treatment of recurrent inguinal hernia].

The experience with surgical treatment of 89 patients with recurrent inguinal hernia is presented. A method for hernioplasty with the use of the pyramidal muscle transferred for covering the inguinal space is described. In 37 patients, no hernia recurrence and injury to the femoral vessels were revealed. This was indicative of the effectiveness of the method suggested.

[The surgical treatment of large ventral hernias in middle-aged and elderly patients].

Changes of intra-abdominal pressure, the degree of abdominal wall stretching, the strength of connection of the tissues and the quality of the operative wound, central hemodynamics, and the indices of external breathing were studied depending on the type of hernioplasty in 44 animals (dogs) and 420 patients with large ventral hernias. The authors suggest a method for surgical treatment of ventral hernias by twisted aponeurotic sutures applied under control of intraabdominal pressure. With the use of this method the number of complications in the main group reduced 3.5 times. With an increase of intraabdominal pressure by more than 1.5 times as compared to the initial value, slackening incisions of the anterior walls of the sheath of the rectus abdominis muscle are recommended. The length of the incisions is controlled by the values of intraoperative tensiomanometry. The number of recurrences with the of the suggested method was 6%.

[Tubular gastrectomy with selective vagotomy in the treatment of peptic ulcer].

The authors generalize the material on the surgical treatment of 151 patients who were operated on for complicated gastroduodenal ulcers by means of an improved method of tubular resection of the stomach with selective vagotomy, subtotal excision of the lesser curvature, and the formation of a ++post-colonic rectangular gastroenteroanastomosis on a short loop with oblique division of the jejunum. The late-term results were studied in follow-up periods of up to 4 years in 110 patients (72.8%) and appraised according to Visik's scale. The results were excellent in 78 (70.9%), good in 25 (22.7%), satisfactory in 6 (5.5%) patients, and poor (recurrence of the disease) in one (0.94) patient.
