Extrarenal and urothelial Wilms tumor.

Two patients with Wilms tumor are presented. One patient had a bilateral Wilms tumor with invasion of the right renal pelvis and proximal ureter. A second patient presented with a large Wilms tumor originating separate from and inferior to the left kidney. The histologic, therapeutic, and prognostic implications of these radiographic presentations are explored.

Demonstration of unsuspected malignant disease of the pericardium by computed tomography.

Massive pericardial effusion associated with mediastinal neoplasm was first diagnosed by findings at computed tomography (CT) in two patients. In each case the CT findings prompted urgent pericardiocentesis for relief of pericardial tamponade. One of the patients was subsequently found to have a widely disseminated renal papillary adenocarcinoma, and the other had fibrosarcomatous mesothelioma of the pericardium. The role of CT in the diagnosis of pericardial disease is discussed.

The distribution of load across the knee. A comparison of static and dynamic measurements.

An anteroposterior radiograph of the leg to include the hip and ankle, taken with the patient standing, provides an estimate of the line of load-bearing at the knee. Gait analysis may be used to determine the way in which the load in the knee is shared between the medial and lateral compartments during normal walking. A comparison of the results from the two methods, carried out on a group of 47 patients, led to the conclusion that both calculations are required for the successful outcome of a tibial osteotomy or a total arthroplasty.

Natural history and associations of dissociated vertical divergence.

One hundred patients with dissociated vertical divergence (DVD) have been followed up for an average of 7.3 years. In contrast to some other reports, none of them has worthwhile binocular vision. There has been a constant association with nystagmus, often manifest, and the DVD has always been noted later than the nystagmus. In no case has the degree of DVD decreased significantly with age. The dissociation has only rarely caused a serious cosmetic problem, and then it has been markedly asymmetrical and has been improved by combined superior rectus recession and Faden procedure. DVD can simulate inferior oblique overactions, but is more commonly associated with an A phenomenon, and can be distinguished by careful cover/uncover testing. The common occurrence of associated congenital defects, especially of the central nervous system, is noted.

Radionuclide evaluation of a communicating bronchopulmonary foregut malformation.

A boy with a communicating bronchopulmonary foregut malformation is presented. Since arteriography failed to demonstrate systemic or pulmonary blood supply to the lesion, 99M Tc-macroaggregated albumin was injected into the aorta at the conclusion of the arteriogram. This procedure documented the existence of a systemic blood supply which at surgery arose from the ninth left intercostal artery. Administration of 99M Tc-pertechnetate prior to surgery demonstrated gastric mucosa in the lesion.

Lability of odor pleasantness: influence of mere exposure.

Subjects judged the pleasantness of various odorants both before and after intensive exposure to a pleasant, a neutral, or an unpleasant odorant, or a short period of relaxation. Intensive exposure comprised a 30 min task of intensity discrimination. The outcome implied that exposure to an odorant can modify its own pleasantness readily, but not so readily that of other odorants. Exposure to the pleasant lemon-smelling substance citral reduced its subsequent pleasantness, whereas exposure to the unpleasant rancid-smelling substance isobutyric acid reduced its subsequent unpleasantness. The results were compatible with the notion of affective habituation. Variability of the pleasantness judgments was uniform throughout the hedonic continuum. That is, subjects agreed as much about hedonically neutral odors as about extremely pleasant and unpleasant odors.
