[Small cell carcinoma of the urinary bladder in a young man. A case report and literature review].

Small cell carcinoma of the bladder is a rare entity characterized by an aggressive clinical behaviour with a high incidence of systemic metastases. We report a case of small cell carcinoma of the bladder in a young man. The primary local tumour was treated by radical surgery, pelvic radiation therapy and polychemotherapy according CDDP protocol. The patient died six months after surgery because disease progression. We also review and update the literature concerning this infrequently tumour.

[Study of the angiogenesis as prognostic factor of pT1G3 bladder tumours].

INTRODUCTION AND OBJECTIVES: Angiogenic activity has been considered like prognostic factor in several solid tumors. This activity can be analysed by two ways: immunohistochemical determination of molecules that activate/inhibit angiogenesis or quantitive measure of microvascular density (MD). Our objective is to determine the prognostic value of Vascular Endothelial Growth Factor (VEGF) and Microvascular Density (MD) in pT1G3 bladder tumours. MATERIAL AND METHODS: We have studied retrospectively 83 patients with pT1G3 tumors treated by TUR + endovesical instillations with a follow up of 3 years at least. We analysed VEGF expression monoclonal antibody No. 360P. To determine MD we have marked vessels with FVIII antibody and detected "hot spots" areas. The number of microvessels is quantited by a digital image analyser excluding those that have more than 50 micras of diameter. We established the correlation of these findings with recurrence, progression and survival by using Chi-square test and Kaplan-Meier curves (log-rank). RESULTS: Average follow up was 58 +/- 28 months. We have established like cut-off 50% of tumor cells (VEGF) and 30 microvessels/fields (MD). Chi-square test did not show correlation with survival neither recurrence but it was positive for progression p(VEGF) 0.048 and p(DM) 0.021. Kaplan Meier curves determined significative differences only for free of progression time respect to MD (p 0.038). CONCLUSIONS: We did not find statistically significant value for recurrence nor survival. Just MD reached prognostic value for progression. More studies and multivariant analysis are required to determine the clinical utility of MD, specially in order to make more aggressive therapeutic options in this kind of patients.

[Mucinous adenocarcinoma of the bladder].

Mucinous adenocarcinoma is a rare entity within the group of primary adenocarcinoma of the bladder which represent 0.5-2% of all malignant epithelial bladder tumours. In spite of the rarity of this tumoral type; it is a poor prognosis entity mainly due to its diagnosis especially in advanced stage of the disease. There is no general agreement on the treatment of adenocarcinoma of bladder. Not withstanding surgery would be the only curative treatment, although unfortunately, it is curative in just a few cases. We report six cases with mucinous adenocarcinoma of the bladder attended in our Department in the last ten years (january 1991-december 2001). In one of them a radical cystectomy was performed, while transurethral resection with or without adjuvant treatment was practiced in the other one. Only one patient is alive today, namely, the one where the tumour not invade the muscular tissue. These findings show the discouraging results of this entity closely intertwined with the pathologic stage.

[Simple thyroid cyst].

Simple or true thyroid cysts are very rare. We only have found one case in which colorless and acellular transparent fluid has been found by means of the fine needle aspiration puncture (FNAP). We report the case of a 33 year old female who first presented with a rapidly growing thyroid nodule. The thyroid scintigraphy revealed the existence of a cold nodule and the ultrasonography showed a lesion having a cystic appearance. The FNAP was performed and was both diagnostic and therapeutic in this case.

[Mucinous prostate adenocarcinoma initially presented as an intestinal subocclusive picture].

Mucinous prostate adenocarcinoma is an infrequent tumour, with no more that 50 cases described to date. This paper contributes a new case of the entity. Rather than on the rarity of the tumour, the interest in our case focuses in the unusual nature of its clinical presentation. It initially appeared as a presumable rectoanal neoplasia but the pathoanatomical examination discovered the prostatic origin of the tumoration.

[Comparative study of pure epidermoid carcinoma of the bladder and transitional cell carcinoma with squamous or mixed differentiated foci].

INTRODUCTION: The epidermoid bladder carcinoma is an infrequent tumour in our environment accounting for 5-8% of total vesical tumours, but is not so uncommon to find foci of squamous differentiation within a transitional carcinoma. We conducted a comparative study between these two histologic forms and analyzed the pattern of their clinical behaviour. MATERIAL AND METHODS: Retrospective study on a total of 32 cases of epidermoid carcinoma diagnosed between 1986-1996 that following histological confirmation were divided into two groups; plain epidermoid carcinoma (PEC), 11 cases; and transitional carcinoma with foci of squamous differentiation (MEC), 19 cases. After further review 2 cases were excluded. RESULTS: PEC appears in older patients. Both tumoral forms are more frequent in males, the male/female ratio being lower in PEC. Presence of irritative factors is significantly associated to PEC (P < 0.01) and smoking to MEC. Presentation signs are similar in both groups, and haematuria is the most frequent sign. Most usual location in both groups was the trigone, which caused uni/bilateral hydronephrosis in over 50% patients. At the time of diagnosis all PECs were invasive versus only 86% in the MEC group. TREATMENT: PEC group underwent the following procedures: 4 radical cystectomies, 2 cystectomies plus radiotherapy, 3 palliative TUR and 2 TUR with radiotherapy. In the MEC group there were 3 radical cystectomies, 2 cystectomies plus radiotherapy, 1 cystectomy with previous chemotherapy, 8 palliative TUR, 2 TUR with endovesical chemotherapy, 1 TUR with systemic chemotherapy, 1 TUR with radiotherapy and 1 systemic chemotherapy. Mean survival was 12.5 months (0-43) in PEC and 26.55 (0-96) in MEC. Disease-free time in PEC and MEC was 5 months (0-37) and 10.25 (0-70) respectively. Only one patient achieved 5-year survival. CONCLUSION: Epidermoid carcinoma both plain and mixed is an aggressive tumour with poor prognosis and short survival. 100% PECs in our series are invasive at diagnosis, and no statistically significant differences were found in survival and disease-free time between both histological variants. Cystectomy remains the choice therapy.

[The diagnostic value of liver aspiration puncture with echographic control].

OBJECTIVE: To correlate liver fine needle aspiration (FNA) diagnosis with histopathological diagnosis and clinical evolution. DESIGN: Retrospective analysis of liver FNA validity, evaluating sensitivity, specificity and predictive value of the method in correlation with the clinical evolution and biopsy results. RESULTS: Malignancy was diagnosed in 46 cases (20 hepatocarcinomas and 26 liver metastases), and benign lesions in 24 patients. Ten additional cases were excluded on the grounds of insufficient material or the impossibility of establishing clinico-histological correlation. There were 6 false-negatives and one false-positive. The overall accuracy of the method was 90% with a sensitivity of 88.2% and specificity of 94%. The positive and negative predictive values of the method were 97.8 and 75%, respectively. CONCLUSIONS: Liver FNA is efficient in cases of suspected liver tumoration. The technique proved particularly useful in the present series in diagnosing liver metastases, often allowing the establishing of tumor origin.

[Giant cell tumors of maxilla].

Ten cases of giant cell tumor of maxilla are reported, the patients being children or young adults and of principally female sex. In three cases another affection was associated: Recklinghausen's disease, Francescatti syndrome and hyperthyroidism of adenomatous origin. Histology of all tumors was similar: giant cells, fibroblastic tissue and numerous vessels: the course was benign in all cases with frequent recurrences but no metastases. The pathology of these tumors is still the subject of wide debate, although research is extensive in this field: electron microscopy, histoenzymology, monoclonal antibodies are some aspects studied but results have sofar failed to elucidate the true nature and histoprognosis of these tumors.

[Retrorbital metastasis an initial manifestation of prostatic cancer].

Orbital metastasis is an unusual localization within tumoral dissemination of prostatic cancer. Similarly, it is rare that orbital metastasis might be responsible for the clinic manifestations that determine the initial diagnosis of neoplasia. We illustrate the case of a patient suffering from prostatic adernocarcinoma that displayed alterations of facial sensitivity and right eye exophthalmos. We describe how the final diagnosis was reached and the patient's response to the suppressive hormonal treatment. The patient's rate of survival has proved to be longer than the rest of cases documented, with over 30-month follow-up.

[Hypertrophic osteoarthropathy in a patient with nasopharyngeal carcinoma].

A clinic-pathological study of a 12 year-old male diagnosed of non-differentiated nasopharyngeal carcinoma (NPC) was carried out. The patients presented secondary hypertrophic osteoarthropathy prior to the development of lung metastases. Hypertrophic osteoarthropathy is a clinic-radiological syndrome characterized by a neoformation of periosteal bone, fundamentally located in long bones and distal ends of metacarpal bones. Polyarticular and synovial involvement is also observed. Association between NPC and hypertrophic osteoarthropathy is uncommon and appears to suggest the existence of metastatic spread (probably to the lung), particularly in young patients.
