Selective arterial embolization for control of hemorrhage in pelvic malignancy: femoral and brachial catheter approaches.

Selective transcatheter embolization with Gelfoam pledgets was used to control intractable pelvic hemorrhage in two patients with gynecologic malignancy. There were no complications or sequelae associated with the procedure, and no further bleeding occurred. This method requires only Seldinger catheterization techniques under local anesthesia and should be considered in patients whose medical condition precludes major surgery or in whom operative ligation cannot be performed.

Incidentally detected thrombosed vein during 67Ga citrate scanning.

A patient with stage IVA Hodgkin's disease developed thrombosis of the right cephalic vein at the site of an indwelling catheter. A few weeks later, 72 and 96 hr scans after injection of 67Ga citrate revealed an increased linear uptake where the thrombosed vein was palpated. As far as we know, this is the first case showing abnormal uptake of this agent in a thrombosed vein. It is postulated that residual chronic inflammatory cells are responsible for 67Ga citrate uptake in this case.

Coarctation of the abdominal aorta with multiple aneurysms. Operative correction.

An 11-year-old asymptomatic girl with hypertension was found to have coarctation of the abdominal aorta with four aneurysms in or adjacent to the hypoplastic segment of aorta. Operative treatment included insertion of a thoracoabdominal aortic bypass graft and resection of the aneurysms. The association of abdominal coarctation and aneurysm formation is reviewed and the rationale for therapy discussed.

Hepatic Venography and Hemodynamics in Patients with Alcoholic Hepatitis.

Fourteen patients with alcoholic hepatitis were evaluated serially by free (FHV) and wedge (WHV) hepatic venogrpahy and corrected hepatic wedge pressure (CWP). Cardiac output was determined and liver biopsy performed before and after 30 days of medical therapy in 11 and 6 patients, respectively. The CWP and FHV correlated well with the severity of hepatitis. Sinusoidal morphology, as determined by WHV, was difficult to interpret and varied on serial studies, although there were no significant changes in the liver histology or FHV. The authors conclude that FHV and CWP are useful parameters of the severity of alcoholic hepatitis, while WHV is helpful in determining patency and direction of blood flow in the portal vein.

Effect of nickel chloride on primary antibody production in the spleen.

Mice immunized intraperitoneally with sheep erythrocytes were treated with nickel chloride, a common particulate air pollutant. Primary antibody production in the spleen was examined using a hemolytic plaque technique. A negative linear dose-response relationship (p is less than 0.05) was observed between the logarithm of (plaques/10(6) cells) and the nickel concentration administered. Mice injected with 3.09 mug Ni2+/g body weight displayed lymphocyte function similar to that of control mice. However, injection of 9.26-12.34 mug Ni2+/g caused significant immunosupression (p is less than 0.05).

Technique for differentiating particles that are cell-associated or ingested by macrophages.

Phagocytosis is a two-step process involving attachment and ingestion of particulate material. It is often difficult to determine under a light microscope whether the particles are actually ingested or are merely attached to the cell. A more accurate, easy to perform technique with the use of xylene has been developed for determining the difference between the attachment and ingestion of polystyrene latex spheres. The xylene treatment dissolves the extracellular spheres, leaving only the intracellular spheres to be counted by the experimenter to obtain a more accurate phagocytic index. This technique also allows an investigator to get an ingestion index, an attachment index, or both.
