[Postoperative chemotherapy of perforating appendicitis in children; with special reference to latamoxef therapy].

We employed latamoxef (LMOX) as antibiotic therapy subsequent to the performance of appendectomies in children. Fifteen patients, consisting of 8 cases of catarrhal appendicitis and 7 cases of gangrenous appendicitis, were administered LMOX at 50 mg/kg/day, while 13 patients with peritonitis due to a perforated appendix were treated with a dosage of 90 mg/kg/day. Both of these dosages were administered in 3 equally-divided doses, as one shot intravenous injections. All of the cases of catarrhal and gangrenous appendicitis showed good, steady recovery after the surgery, and there were no instances of complications. With regard to the 13 cases of perforating appendicitis, a clinical efficacy of at least "good" was obtained in 12 patients, for an efficacy rate of 92.3%. In addition, bacteriologically, 35 out of 36 strains that were isolated initially could no longer be detected after the LMOX therapy, indicating an eradication rate of 97.2%. Complications consisted of 1 case of dehiscence of the wound, and 1 case of adhesive ileus. Comparison of the values of the laboratory tests before and after the LMOX therapy revealed no development of abnormalities. Moreover, with regard to side effects, with the exception of 1 case of transient diarrhea, there were no side effects which presented clinical problems. It was found that the causative microbes involved in these cases of appendicitis--especially in the perforative appendicitis cases--were Gram-negative bacilli such as E. coli, and an anaerobic bacterium, B. fragilis.(ABSTRACT TRUNCATED AT 250 WORDS)

GK(Ca)-dependent cyclic potential changes in the sensory nerve terminal of frog muscle spindle.

Spontaneous cyclic hyperpolarizations along the sensory nerve terminal of frog muscle spindles were observed during the application of 1-9 nA depolarizing currents across an air-gap on which the axon was bridged. An increase in the current intensity increased the amplitude and duration of the cyclic changes. Upon subthreshold depolarization, single or repetitive hyperpolarizations could be elicited after a brief electric pulse or during stretch of the receptors, respectively. The threshold was decreased in higher Ca2+, Sr2+ or Ba2+ solutions. The cyclic changes were reversibly blocked by K+- or Ca2+-blockers and quinine. These results suggest that the changes are due to GK(Ca). The site of origin of the changes was at the branching node in the capsule, as confirmed by the following results: (1) the cyclic changes were abolished upon inactivating the node by UV-irradiation; (2) in normal Ringer's solution, the rate of afferent impulses, which reflects the membrane potential at the encoding site along the non-myelinated filaments, was unmodified by the cyclic changes and was independent of the intensity of the polarizing currents within a certain range; however, it was sensitively dependent on this intensity after treatment with K+-blockers; (3) the amplitude of the impulses reaching the branching node was attenuated during the cyclic changes, but not after GK-blockade.

Effects of anions on calcium component in sensory nerve terminal of frog muscle spindles.

An increase in the Ca2+ component following individual sodium spike in spontaneous discharges was observed in the isolated sensory terminal of the frog muscle spindle perfused with isotonic sodium solution with a group of anions, of which the size in the aqueous solution ranged from 0.74 to 1.32 times that of hydrated sodium ion. The effects of these anions was counteracted with divalent cations. It is hypothesized that anions similar in size to hydrated sodium ions may form an anion-cation complex at the interaction site of the Na+ carrier, whereby the Na+ may enter the Ca2+ channel. This may be inactivated by the divalent cations.

Prenatal diagnosis of metachromatic leukodystrophy: a diagnosis by amniotic fluid and its confirmation.

Late infantile metachromatic leukodystrophy (MLD) was successfully diagnosed in utero by demonstrating the absence of arylsulfatase-A in amniotic fluid using diethylaminoethyl-Sepharose column chromatography. Diagnosis by amniotic fluid using an ion-exchange column is more rapid and reproducible as compared with those reported previously. The diagnosis was confirmed by the absence of arylsulfatase-A in fetal brain, liver, and kidney tissues as well as by the marked accumulation of sulfatide in kidney. The kidney is the most appropriate organ for the demonstration of sulfatide accumulation in fetal tissues in MLD.

Reversible inhibition by human serum lipoproteins of cell proliferation.

Normal human serum or plasma was studied for the presence of inhibitors of cell proliferation by assaying inhibition of incorporation of labeled thymidine into acid-insoluble fraction using human FL cells. Lipoprotein fraction obtained by gel filtration through Sepharose 4B and by KBr density gradient centrifugation was found to play a major part of the inhibitory activity of the serum. It was also shown that the inhibitory activity resides in low-density lipoprotein (LDL). The addition of the lipoprotein fraction to growing FL cells caused an early decrease in the transport of uridine and thymidine across the membrane. This change in the permeability of membrane was followed by the preferential inhibition of DNA synthesis and a reduction in the percentage of mitotic cells in the cell population. The inhibition of the growth was reversible and was observed in various types of cells irrespective of species.

Alkaline phosphatase isoenzyme of colonic carcinoma in Wistar-Furth rats.

Wistar-Furth (WF) strain rats have been shown to have a high incidence of spontaneous colon cancer. We have compared the alkaline phosphatase (AP) isoenzymes in colonic carcinomas of these inbred WF strain rats with the isoenzymes in the non-carcinomatous colon of the same strain and the normal colon of Wistar strain rats, from which the WF strain was derived. On electrophoresis of AP specimens partially purified by acetone fractionation, the non-carcinomatous colon was found to have three main isoenzymes, while the normal colon had two. Colonic carcinomas gave one band which migrated slower than any bands from either normal or non-carcinomatous colon. The electrophoretic mobility of AP from colonic carcinomas was retarded by neuraminidase treatment. This did not influence the isoenzymes from non-carcinomatous and normal colon. The inhibition patterns of the enzyme from colonic carcinoma by amino acids, inorganic phosphate and urea were different from those of the isoenzymes in the other two tissues. AP of colonic carcinoma was also the most heat-labile. There were no significant differences in the enzymic properties of the isoenzymes from non-carcinomatous and normal colon. The enzymic properties except for electrophoretic mobility were found to be the same between APs of colonic carcinoma and the placenta.

Calcium spike induced by electrical stimulation to the sensory nerve terminal of the frog muscle spindle.

A calcium spike in the sensory nerve terminal of the frog muscle spindle could be elicited by electrical pulses, which were given across an air-gap on which the first myelinated segment of the parent axon outside the spindle capsule was bridged, after the sodium spike had been blocked by treatment with tetrodotoxin (TTX). The threshold current was 1.1-2.4 nA higher than that for the sodium spike, suggesting that the calcium channels distribute distally to the site of afferent impulse initiation, or that the density of calcium channels on the encoding site may be less than that along portions distal to the site.

Effects of calcium blockers on the discharge pattern of frog muscle spindle.

Application of calcium blockers, such as MnCl2, MgCl2, EGTA and verapamil, resulted in a decrease in the dynamic response of afferent discharges during ramp-and-hold stretch of the frog muscle spindle, causing mainly the reduction of the peak rate at the completion of the stretch. As calcium spikes in the spindle terminal have been known to superimpose preferably at the completion of ramp stretch, it is suggested that a part of the dynamic response of afferent discharges is possibly contributed by the calcium spike in the terminal.

Control of the variability of the afferent discharge rate in frog muscle spindle by potassium blockers.

Blockage of the K+ channels in the sensory terminal of frog muscle spindle by the combined application of BaCl2 and other K+ blockers makes it possible to reversibly change the sustained and irregular discharges into non-sustained regular discharges. As a similar change in the discharge pattern is also made by application of CoCl2 alone, though it is irreversible, the irregular and sustained discharge may be due to [Ca2+]i-activated potassium conductance.

Histidinemia: biochemical parameters for diagnosis.

A highly specific and sensitive procedure for determining histidase activity, with labeled histidine as the substrate, that requires only 1 to 2 mg of stratum corneum epidermidis has been developed. Methods for determining ratios of urocanic acid to histidine (U/H) in stratum corneum epidermidis and for measuring excretion of histidine and its metabolites by thin-layer chromatography were developed. Application of this triad--stratum corneum epidermidis histidase determination, measurement of stratum corneum U/H, and analysis of excretion of histidine metabolites--should facilitate evaluation of the condition of infants and young children with suspected histidinemia. Moreover, differentiation between patients with the classic form and genetic variants of the disorder should now be possible.
