Precipitating antimitochondrial antibodies in Japanese patients with primary biliary cirrhosis.

The prevalence of three precipitating antibodies to mitochondria A (M-A), mitochondria B (M-B), and mitochondria C (M-C), reacting with the antigens in the mitochondrial fraction of sonicated rat liver was studied in Japanese patients with primary biliary cirrhosis and other liver diseases. Antibodies to M-A and M-B were found in 12 of 22 (54%) and 11 of 22 (50%) patients, respectively. Antibodies to M-C were found in only one of 22 (4%) patients. The titers of antibodies to M-A and M-B correlated with the titers of mitochondrial immunofluorescence staining on unfixed mouse kidney section (r = 0.71 and 0.81, respectively). These antibodies were not present in liver cirrhosis (20 patients), chronic active hepatitis (20), acute viral hepatitis (10), and hepatoma (10). However, the titers of these antibodies did not correlate with amount of immunoglobulin, immune complexes, and the severity of disease. This work confirms that the antibodies to M-A and M-B are also marker antibodies for Japanese patients with primary biliary cirrhosis.

Differential reactivity of alpha-fetoprotein with lectins and evaluation of its usefulness in the diagnosis of hepatocellular carcinoma.

The reactivity of alpha-fetoprotein (AFP) with concanavalin A (Con A) and Lens culinaris agglutinin (LCA) was studied by crossed immuno-affinoelectrophoresis of the serum samples of 146 patients from three groups (groups I, II and III). Fifty-one patients with benign liver diseases were included in group I, 83 patients with hepatocellular carcinoma in group II, and 12 patients with carcinoma metastatic to the liver from digestive organs in group III. In group I, the percentage of Con A-reactive species of AFP was high (97 +/- 5%, mean +/- SD), but that of LCA-reactive species was very low (3 +/- 5%). The percentage of LCA-reactive species of AFP in group II (45 +/- 33%) was higher than that in group I (P less than 0.001), while the Con A binding pattern of AFP in this group, as demonstrated by immuno-affinoelectrophoresis, was similar to that of group I. The percentage of Con A-reactive species of AFP in group III (55 +/- 24%) was much lower than that in group II (97 +/- 5%) (P less than 0.001). The above results indicate that measurement of the reactivity of AFP in serum samples with Con A and LCA is useful for the differentiation of AFP species which are found in association with benign liver diseases, hepatocellular carcinoma and carcinoma metastatic to the liver.

Nephropathy associated with Charcot-Marie-Tooth disease.

In this report, we describe a case of 14-year-old girl with Charcot-Marie-Tooth (CMT) disease and the nephropathy which was characterized by heavy proteinuria and microscopic hematuria. She progressed to renal failure with clinical duration of 4 years from the onset of disease. Renal biopsy specimens revealed the features of focal segmental glomerulosclerosis (FSGS). The patient has also a bilateral sensorineural deafness. Although the clinical features show similarities to those of the Alport syndrome, electron microscopic examination did not disclose the glomerular basement membrane changes which were characteristic of the Alport syndrome. The association of nephropathy with CMT disease is discussed, as compared with previous report.

Hepatitis B immunoglobulin (HBIG) efficacy in the prevention of perinatal transmission of hepatitis B virus (HBV) infection.

Twelve out of 13 infants born from mothers having both HBsAg and HBeAg developed HBV carrier state within 4 months after birth. On the other hand, no babies from mothers having HBsAg and anti-HBe developed HBV carrier state. In order to prevent perinatal transmission of HBV, HBIG was administered into 14 babies born from mothers with positive HBeAg three or four times during 6 months after birth. During 12 months or more of observation period 5 out of 14 infants who received HBIG acquired active anti-HBs response after discontinuation of HBIG (passive-active immunization). However, 3 out of 14 infants unfortunately developed persistently positive HBsAg antigenemia at 12th, 14th and 14th month respectively after birth. Remaining 6 babies still have no virus markers, indicating not infected. These results indicates that HBIG administration was extremely effective for prevention of perinatal transmission of HBV. However, additional preventive measures with active immunization (HBV vaccine) seems to be necessary to prevent completely the perinatal transmission of HBV.

[Hepatitis and hepatocellular carcinoma].

Most of Hepatocellular carcinoma (HCC) in Japan are accompanied with liver cirrhosis (LC) or fibrosis and histopathological investigation based on materials disclosed the gradual transition from hepatitis to LC and then HCC. In recent years, it was clarified that, using sensitive assay methods for Hepatitis B Virus (HBV) markers, most of sera from HCC patients had HBV markers, indicating that HBV might play an important role in the progress and/or establishment of chronic liver diseases and HCC. In this paper, our experiences of the transition from type B hepatitis to HCC confirmed by serial liver biopsies were presented mainly, then I explained about the clinicopathological studies of NonA NonB hepatitis, of which virus markers were not yet found, in relation to HCC.

Inhibitory effect of alpha-fetoprotein on protein synthesis in a reticulocyte lysate cell-free system.

The inhibitory effect of human alpha-fetoprotein on the protein synthesis (incorporation of [3H]leucine into the total protein fraction) weas observed in a rabbit reticulocyte lysate cell-free system. The incorporation was inhibited to 50% of the control level by 14-16 microM alpha-fetoprotein, and decreased to about 15% at a concentration higher than 40 microM. An almost identical dose dependence was obtained between fetal alpha-fetoprotein and hepatoma-derived alpha-fetoprotein. This inhibitory effect on the protein synthesis was due to the interference of alpha-fetoprotein to the 40 S initiation complex formation from the ternary complex (eukaryotic initiation factor 2.GTP.Met-tRNAfMet). In contrast human serum albumin purified from fetal cord sera did not exhibit this inhibition under the same conditions. These results indicate that alpha-fetoprotein may function as a regulator of the protein synthesis in the fetal stage.

Therapeutic effects of tiopronin on chronic hepatitis: a double-blind clinical study.

The efficacy of tiopronin (2-mercaptopropionyl glycine) 600 mg daily in the treatment of chronic active or chronic persistent hepatitis has been assessed in a double-blind controlled clinical trial of 12 weeks involving 165 Japanese patients with histologically proven disease. Treatment with the drug was associated with a significant improvement in abnormalities of serum transaminase and gamma glutamyl transpeptidase, with reversion towards baseline values on stopping the drug. Improvement was independent of the histological classification of the disease, or HBsAg status. The drug was well tolerated with few side-effects. The results of this short-term study indicate that tiopronin may be of value in the treatment of chronic hepatitis.

Fibronectin in human hepatocellular carcinoma.

Human liver specimens with hepatocellular carcinoma were studied for distribution of fibronectin by immunofluorescence staining. Fibronectin was detected in the cytoplasm and/or on the cell surface in certain cases, which is in contrast to the observations in vivo for hepatocytes of normal and other fibrotic livers. The present finding is also in contrast to those for various other carcinomas so far reported.
