[Clinical aspects and pathogenesis of infarction of the brain stem].

Clinical and hemodynamic studies were carried out in 75 patients with stem brain infarction in the acute period, including repeated infarctions. The total rupture of the autoregulatory mechanisms of the brain blood flow was discovered, leading to the development of different phenomena of "brain steal" determining the clinical course of stem brain infarction. In the given pathology, the acute period is characterized by the development of the vicious circle of hemodynamic disorders, which is coupled with pathological dissociation in coagulation hemostasis. The progression of the hemodynamic changes requires pathogenetic correction of central and cerebral circulation.

[Emergency surgery of carotid and vertebral arteries in acute cerebral ischemia].

The efficacy of emergency surgical interventions on the carotid and vertebral arteries in patients with acute disorders of cerebral circulation is discussed. An organizational structure of rendering aid is suggested, which unites emergency ambulance service, neurology, and vascular surgery in one therapeutic institution. A clinical classification of the severity of acute cerebral ischemia and the optimal terms for operations were elaborated. In a group of 21 patients who underwent emergency operations 14 showed excellent and good clinical results.

[Clinical picture and diagnosis of subdural hematomas in children].

Thirty-six children, aged from 5 months to 14 years, with subdural haemorrhages are discussed. In 31 the hemorrhages were of a traumatic character, in 5 their origin was unknown. The authors draw the conclusion that the course of subdural hematomas in children is marked by peculiarities which often make the clinical diagnosis difficult. Examination of the cerebrospinal fluid, echo-encephalography, and angiography helped in all these cases to identify the character of the process and its localization properly. All children underwent operation with a satisfactory outcome.
