Contralateral medial temporal lobe damage in right but not left temporal lobe epilepsy: a (1)H magnetic resonance spectroscopy study.

BACKGROUND: Proton magnetic resonance spectroscopy (MRS) of the hippocampus is useful in lateralising the epileptic focus in temporal lobe epilepsy for subsequent surgical resection. Previous studies have reported abnormal contralateral MRS values in up to 50% of the patients. OBJECTIVE: To identify the contributing factors to contralateral damage, as determined by MRS, and its extension in patients with temporal lobe epilepsy. METHODS: Single voxel MRS was carried out in the hippocampus and lateral temporal neocortex of both hemispheres in 13 patients with left temporal lobe epilepsy (LTLE) and 16 patients with right temporal lobe epilepsy (RTLE). All patients had mesial temporal lobe epilepsy with hippocampal sclerosis. Controls were 21 healthy volunteers of comparable age. RESULTS: Consistent with previous studies, the NAA/(Cho+Cr) ratio was abnormally low in the hippocampus ipsilateral to the focus (p < 0.0001), and there were lower values in both patient groups in the ipsilateral temporal neocortex (p < 0.0001). Patients with RTLE had left hippocampal MRS anomalies (p = 0.0018), whereas the right hippocampus seemed to be undamaged in LTLE patients. CONCLUSIONS: Unilateral mesial temporal lobe epilepsy is associated with widespread metabolic abnormalities which involve contralateral mesial and neocortical temporal lobe structures. These abnormalities appear to be more pronounced in patients with RTLE.

[Leg heaviness and stasis microangiopathy. Preventive and therapeutic approaches].

Venous insufficiency of the legs is a frequent and insidious disorder which, through its consequences' cascade, lead to some cutaneous microangiopathy with relative ischemia. Chronic leg ulcer is the late issue of the process accompanied by varied medical and socio-economic aspects. The pathogeny of the cutaneous lesions is directly related to alterations in hemorrheology and leukocyte adhesion inside capillaries. Elastic contention and some venotropic drugs exert preventive effects on the development of leg ulcers. They also bring some therapeutic improvement of the symptoms of leg heaviness.

Coating of fertilizers by degradable polymers.

The conventional agriculture leads to some important pollution of ground water (particularly, by nitrates). The solution is the coating of fertilizers by degradable polymers. In this work, we have studied the water vapour and liquid diffusion through polymer films detached from their support. Therefore, we may classify polymers as a function of their properties like water vapour and liquid barrier. We may choose the best polymer(s) for coating.coated fertilizers by chosen polymer(s) with mechanical techniques such as fluidised bed and pan coating. Moreover, the electron microscopy used to see the quality of the wall has showed the presence of pores due to the rapid evaporation of solvent. A drying in air current and an annealing could be done to avoid this problem.followed the ions release of fertilizers immersed in distilled water by conductimetry. The more interesting result was obtained with fertilizers coated by polylactic acid. In effect, the total release reached three weeks.

[An asymptomatic aneurysm of the popliteal artery treated with endoprosthesis].

Stent-graft is considered as a possible alternative to traditional surgery in the correction of popliteal aneurysms but only a few cases have been reported and these are difficult to compare. This new case highlights the feasibility and problems of an endovascular approach. The size of the stent was determined on the basis of preliminary arterial scanning and a measurement guide wire. Strong recommendations to the patient to avoid posture-related kinking of the artery and/or the stent (during occupation-related or other activities) could prevent delayed thrombosis of the stent. The lifetime of the stent can be prolonged by a simple radiosurgical clearance procedure.

[How I explore ... A cutaneous scleroderma using noninvasive methods].

There exists several clinical types of scleroderma. Most of them show cutaneous manifestations. Skin involvement may be isolated (morphea) or be part of a multi-organ systemic disease. Objective non-invasive functional methods are useful in order to better assess the severity and evolution of the disease and the effects of treatments. Measurements of the thickness and mechanical properties of the skin as well as the visualization of the cutaneous blood flow prove to be informative.

[How I explore ... an orphan metastasis from melanoma].

A presumed melanoma metastasis is sometimes found without any evidence for the primary neoplasm. Such an orphan metastasis merits a complementary exploration. Searching for the primary melanoma must then be performed using non invasive and clinically oriented methods. Beside a thorough skin examination, otorhinolaryngological, ophtalmic and gynecologic searches are recommended. In addition, paraclinical investigations are conducted according to the metastatic location, the gender and the functional complaints of the patient. The prognosis of these orphan metastases is not more dramatic than when the primary lesion is identified. Therapies are similar to those proposed in the classical presentation of the disease.

[DHEA and rejuvenating intracrinology? Between reason and magic].

Slowing down ageing is a goal for a large part of the population. Some mercantile claims hold out bright prospects to DHEA. The effects of this hormone manifest themselves after intracrine conversion. In the skin, sebaceous glands enlarge and seborrhoea increases in post-menopausal women. No other effect is clinically discernable on the skin. At this level, DHEA does not show efficacy comparable to that of other anti-ageing compounds.

[Skin and its daily chronobiological clock].

Some cutaneous functions of importance are governed by periodical variations according to the nycthemeral timing. Similarly to other organs, the skin is under the influence of a coherent organisation of circadian and nythemeral rhythms modulating various biological cycles which usually display a wide amplitude. The cutaneous biorhythms in humans suggest that during daylight the skin boosts diverse protective functions with regard to environmental threats. In the evening and at night, the skin increases its renewal and diverse metabolic processes. Biorhythms in the skin which are in line with the organism's needs, merit to be recognized. Chronophysiology, chronopathology and clinical chronopharmacology may help reach optimal therapeutic decisions.

Outgassing behavior and composition of comet C/1999 S4 (LINEAR) during its disruption.

The gas activity of comet C/1999 S4 (LINEAR) was monitored at radio wavelengths during its disruption. A runaway fragmentation of the nucleus may have begun around 18 July 2000 and proceeded until 23 July. The mass in small icy debris (</=30-centimeter radius) was comparable to the mass in the large fragments seen in optical images. The mass budget after breakup suggests a small nucleus ( approximately 100- to 300-meter radius) that had been losing debris for weeks. The HNC, H2CO, H2S, and CS abundances relative to H2O measured during breakup are consistent with those obtained in other comets. However, a deficiency in CH3OH and CO is observed.

Allergic reactions to decorative skin paintings.

BACKGROUND: Dye mixtures containing henna, P-phenylenediamine (PPD), and indigo are applied to the skin in various parts of the world for religious purposes, social recognition or fun. AIM: To identify eventual allergens responsible for contact sensitization to skin decoration in three young children. Lesions remained strictly limited to the site painted during the summer holiday. None of the children had a previous history of contact dermatitis, but two of them suffered from severe seasonal allergic rhinoconjunctivitis. FINDINGS: Patch testing revealed severe contact hypersensitivity to PPD in all three patients, and one child presented a positive prick test to 1% henna in water. The eruptions resolved following spontaneous elimination of the dyes and topical applications of corticosteroids, but left residual depigmentation. CONCLUSION: PPD is the main allergen identified in allergic reactions to decorative skin paintings.

[Liminal perception threshold of cutaneous distension].

BACKGROUND: Cutaneous sensorial perception is complex and proves to be difficult to quantify. Skin sensitivity to mechanical distension is largely unexplored. We have developed an original method aimed at quantifiying these physiological parameters. VOLUNTEERS AND METHODS: The study was performed on 24 adult volunteers aged 20 to 50 years. A Cutometer SEM 474 equipped with 2 and 4 mm diameter probes was used to exert a progressive or a steep suction on the cheeks. The intensities of both the suction and the skin deformation observed at the earliest moment the traction was perceived by the volunteers were recorded. RESULTS: The liminar sensorial detection under progressive suction force was 1.5 times lower when using the 4 mm probe than the 2 mm probe. Suction necessary for liminar sensation was 7 times lower under steep suction than in response to the progressive mode. The skin deformation corresponding to the liminar sensorial perception was almost constant (C=0.5 p. 100) for each subject. By contrast, a large inter individual heterogeneity unrelated to age was present. COMMENTS: Sensitivity of facial skin to stretching can be explored using a precise suction method. Perception is more related to the rate of traction than to the surface area submitted to suction. Cutaneous deformation, nearly constant for each individual, appears to be the main parameter. Steep and progressive suctions appear to stimulate distinct types of mechanoreceptors. The presently described method can be applied in cosmetology and in functional investigation of various diseases such as diabetes, acromegaly and atopic dermatitis among others.

[The skin, cold and winter sports].

Winter sports are responsible for various dermatoses which could be often avoided by simple preventive procedures. Both the severity and duration of cold exposure combined with wind speed, altitude and environmental hygrometric value govern the potential types of cold injuries.

[Uterine artery embolization for symptomatic uterine fibromas. Informed consent of the patient].

It would seem necessary that an information sheet and written consent should be exchanged and signed by the physician and the patient, especially when interventional radiology procedures are involved. A comprehensive interview with full explanation is also mandatory if the patient is to be adequately informed. The existence of a specific document provides two benefits: For the patient: an understanding of the procedure and of the associated risks and benefits tends to reduce the natural anxiety which stems from ignorance and better prepares her to make an informed decision on whether to agree (i.e. give consent) to the procedure or decline it. For the physician: the document represents a formal way of demonstrating that he or she has fulfilled his or her duties in the matter of patient information and consent. A written form would seem to be the best solution to meet these requirements. A document of this type is proposed in this case in the form of a series of questions and answers about embolization of the uterine arteries as a modality for the treatment of symptomatic uterine fibroids.

[Image of the month. A hidden melanoma hidden in a tennis player].

A man presented with a malignant melanoma of the scalp. The lesion reached a large size before being disclosed. Repetitive sunburns during tennis matches could be responsible for this cancer. Indeed, hair protects only partially the skin from ultraviolet damages.

[Severe pustular and polymorphous vasculitis caused by losartan].

INTRODUCTION: We report a case of severe pustular vasculitis most probably due to losartan (Cozaar) which is an antihypertensive drug inhibiting the receptors for angiotensin II. CASE REPORT: A 64-year-old woman, suffering from hypertension, developed a widespread and rapidly evolving bullous and pustular eruption without any other clinical sign. Leucocytoclastic vasculitis and junctional blisters were present with deposits of both IgA and IgG at these sites. Withdrawal of losartan, which was the only drug taken by the patient, and tapered corticotherapy cleared the lesions. The rash left a prominent mottled hyperpigmentation. It did not recur during the following months despite the presence of antineutrophil cytoplasmic antibodies. DISCUSSION: So far only a few cutaneous side effects have been reported following losartan intake. This drug was the likely culprit in the present case. Vasculitis was extremely severe and polymorphous.

An appraisal of 18-fluorodeoxyglucose positron emission tomography for melanoma staging.

BACKGROUND: Positron emission tomography (PET scan) using fluorodeoxyglucose (FDG) is increasingly recognized as a reliable diagnostic method to detect metastases of malignant melanoma (MM). OBJECTIVE: To compare the diagnostic performance of 18-FDG PET scan to that of conventional imaging. METHODS: A total of 28 assessments were conducted in 24 patients at risk of metastatic MM. RESULTS: The diagnostic accuracy was over 80% and similar for PET scan and conventional imaging. CONCLUSION: Both the specificity and sensitivity of PET scan are high although not perfect. Confrontation of data with anatomical location and clinicopathological findings remains mandatory.

Anisotropy and multiple scattering in thick mammalian tissues.

A dual-channel Mach-Zehnder interferometer using heterodyne detection allowed us to measure simultaneously parallel and perpendicular polarization components through various mammalian tissues at a wavelength of lambda = 633 nm. By contrast with liver tissue, squeletic muscles of a few millimeters thickness exhibit strong anisotropic properties that change the direction of the linear polarization of the light. This rotation of the initial plane of polarization is to be distinguished from the depolarization that is due to the multiple light scattering that goes along with large temporal fluctuations. Complementary photos under linearly polarized light illustrate the behavior difference between liver (isotropic medium) and muscle (anisotropic medium).
