[Therapy of carcinoma of the breast. Surgery. Present state of knowledge and problems (author's transl)].

For breast cancer of a limited size a tendency away from the radical Halsted-mastectomy can be noticed. A number of studies conducted during the last few years demonstrate that procedures less than radical mastectomy do not necessarily jeopardize the primary goal, namely maximal survival without recurrence. Lumpectomy, axillary staging and curative irradiation can be applied to small breast cancers without clinical evidence of diseased axillary nodes. The quality of the rather difficult radiation therapy and of the follow-up are of paramount importance.

[Cranial mucormycosis in a patient with a transplanted kidney].

Mucormycosis is a very serious complication of debilitating diseases, and particularly of diabetes. Presently the treatment of choice is amphotericine B. A patient is described who, like most other renal transplant patients with mucormycosis, had cranial localization of this disease and diabetes. The clinical findings were classical and the diagnosis was confirmed histologically.

[Blood transfusion and kidney transplantation].

The non-transfusion policy for potential recipients of kidney allografts that has been followed in recent years has not proven successful. There is now ample clinical evidence of improved transplant function in the transfused recipients category. This has also been confirmed in recipients of a first cadaver kidney in Basel and Geneva. If the conditions valid for human kidney transplantation are matched as closely as possible in animal experiments, prolongation of graft function rather than accelerated rejection is usually observed after blood transfusions. Potential kidney graft recipients should be more liberally transfused and experimental evidence ought to be accumulated in order to establish a rationale for optimal host conditioning, while avoiding the hazards of blood transfusions to the greatest extent possible.

[Mammary carcinoma in Switzerland (author's transl)].

Combined surgical, radiotherapeutic, and sometimes chemotherapeutic treatment of early stages of operable mammary cancer results in 5-year survival without recurrence in a similar percentage of cases as modified radical mastectomy does. This is demonstrated with 1296 patients of two French and one American groups using comparable indication and therapeutic principles. No controlled studies of long-term results using these principles are thus far available. Preliminary results of a Swiss adjuvant polychemotherapy study after mastectomy show a significant favorable effect only in patients with negative axillary lymph nodes.

[Diagnostic laparotomy in Hodgkin's disease: indication, management, results (author's transl)].

Till December 31st 1976, diagnostic laparotomy was carried out in 58 patients with Hodgkin's disease. Of these, 23 cases (40%) revealed abdominal involvement. The tumor stage found by conventional diagnostics before surgical treatment had to be altered in 16 patients (27%). It had to be increased in ten and lowered in six cases. Of 13 splenic Hodgkin infiltrations twelve had not been supposed preoperatively; four out of 43 sound organs had been suspected preoperatively because of splenomegaly. Subdiaphragmatic histological findings involving a rather unfavorable prognosis were seen in nine patients (16%). In Hodgkin's disease, risk groups cannot be selected with confidence, as no definite correlation between the frequency of subdiaphragmatic affection and the primary manifestations, histology or general symptoms was observed. Since operation risk of diagnostic laparotomy is low, operable stage I-III cases with Hodgkin's disease should undergo diagnostic laparotomy prior to treatment planning.

[Hemodialysis--the arteriovenous fistula, distal to the wrist joint].

The arterio-venous fistula distal to the proc. styloides radii in the "tabatière" offers a most useful additional and very peripheral vascular access for chronic hemodialysis. It in no way compromises the further construction of arteriovenous anastomoses in be forearm in fistula failure. The results of 26 such fistulas are reported.

Hyperlipidemias in patients with kidney transplants.

Serum lipids in 58 renal transplant recipients were related to duration of follow-up, relative body weight, steroid medication, proteinuria and graft performance. Hyperlipidemia was observed between the 4th month and the end of the first year after transplantation in 83% of the patients. Thereafter, the frequency of hyperlipidaemia appeared to decrease: at 4 to 7 years only 61% of the subjects continued to exhibit abnormal high serum lipids. Three mechanisms leading to hyperlipidaemia were identified: 1) overweight, 2) steroid mediation, 3) proteinuria. A forth apparent mechanism was impaired transplant function.

Clinical and psychological effects of intravenous diazepam related to plasma levels. A controlled, double-blind, cross-over study in oral surgery involving local analgesia.

In a controlled, double-blind, cross-over study with diazepam (DZ) and placebo (PL) various clinical, psychological, and pharmacological parameters were studied in 33 patients having identical impacted third molars removed. The patients were admitted to the study consecutively and nonparametric statistics were used throughout the study for the evaluation (Wilcoxon's matched-pairs signed test). The following aspects were examined: Continous reaction times (CRT), rating scales (a.m. Beecher), blood pressure, pulse, plasma concentration of DZ and of N-desmethyldiazepam, visual changes, amnesia, adverse reactions, recalling of duration of surgery, orientation in time, phlebitis, and final evaluation performed by the patient. The operative conditions were found to be excellent, and only a few adverse effects were noticed. The patients expressed an overall enthusiasm about the sedation. In the rating scales only the predominantly sedative items were affected and not the anxiolytic ones. Neither the heart rate nor the systolic blood pressure were significantly changed; on the contrary, the DZ-group was less affected than the PL-group when maximum increases in the systolic blood pressure were compared in the two groups. The continuous reaction times were significantly correlated to the plasma concentrations of diazepam, especially 1 hour after sedation. The reported recurrence of initial clinical symptoms 6 hours after sedation due to the N-desmethyldiazepam could not be demonstrated in this study. The CRT has proved itself to be of great value in discriminating between different kinds of cerebral affections; the patients with DZ-intoxication showed significantly faster CRT 1 hour after injection than did patients suffering from well-documented cerebral disorders such as organic brain damage or hepatic encephalopathia.
