Urine laminin P1 assessment discriminates between invasive and noninvasive urothelial cell carcinoma of the bladder.

In a cross-section study the levels of urine laminin P1 expressed as a laminin P1:creatinine ratio were determined in 25 patients with superficial (noninvasive) transitional cell carcinoma (TCC) of the bladder, 12 patients with invasive TCC, 15 patients with inflammatory bladder diseases and 50 healthy controls. The mean of laminin P1:creatinine ratios in invasive TCC patients (4.83 +/- 3.05 U/mol creatinine) significantly differed from those of superficial TCC (2.60 +/- 1.07 U/mol creatinine), inflammatory bladder disorders (2.38 +/- 1.74 U/mol creatinine) and controls (2.47 +/- 0.86 U/mol creatinine) (p < 0.03). At the individual patient level 7 out of 12 patients with invasive TCC, but only 1 out of 25 patients with superficial TCC, showed laminin P1 urine levels above the normal range (0.75-4.18 U/mol creatinine), thus giving a 58% sensitivity and a 96% specificity of laminin P1 urine assessment in the discrimination between noninvasive and invasive disease. Given a 20-24% prior chance of invasive disease at initial diagnosis detection of elevated urine laminin P1 in TCC patients raises the posttest chance of invasive disease to 87.5%.

The relationship between cause of death of the kidney donor and the presence of ischemic lesions in the kidney.

Chronic ischemic lesions in the donor kidney amplify the nephrotoxic effects of cyclosporine A. With increasing age, the presence of chronic ischemic lesions in the kidney increases, and data concerning the fate of kidney grafts from older donors are conflicting. Kidney from donors with an intracerebral bleed do less well compared to kidneys from other donors. Systematic data on the relationship between donor age, cause of death and severity of chronic ischemic lesions are lacking. This study was performed to investigate this relationship.

Value of serum laminin P1 as a diagnostic and monitoring parameter in transitional cell carcinoma of the bladder.

In a combined (cross-sectional and longitudinal) study, the serum concentration of laminin P1 was measured by radioimmunoassay in 16 patients with benign inflammatory bladder disorders and 47 patients with transitional cell carcinoma of the bladder. The results were compared with the serum laminin P1 values in 50 healthy control subjects. In the cross-sectional study, the mean value of serum laminin P1 was significantly higher in bladder cancer patients than either the controls (p < 0.0001) or patients with benign inflammatory bladder disorders (p < 0.001). Similarly, the mean values of all different stages of grades of the tumor were significantly higher than either the controls or patients with benign inflammatory disorders. Progressive increase in the mean values of serum laminin P1 could also be found with deterioration of the stage or grade of the tumor. However, the difference between the mean values of the different cancer stages or grades did not reach statistical significance. In the longitudinal study, no significant difference could be detected between the mean values of patients with superficial tumor recurrence and those with remission of the disease (p > 0.5). Nevertheless, in the invasive cancer group, the levels of serum laminin P1 were directly proportional with progression of the disease (Z = 2.94; p < 0.01).

Quality of dental restorations and dental treatment in Dutch schoolchildren.

The quality of dental restoration and treatment need have been defined and investigated in a survey of primary schoolchildren in the Netherlands. Measurements were made using respectively a quality of dental restorations index and a dental treatment index. A statistical analysis was carried out on these data. Six criteria were used to classify the technical quality of restorations. Quality was analysed in relation to the child's region of residence and the type of dental professional regularly visited. Three other variables were also considered to be relevant in the analysis; the size of the restorations (one or more surface fillings), the child's age, and whether the restorations were polished (finished) or not. An overall index to measure the intrinsic quality of dental restorations (QDRI) based upon five qualitative criteria was constructed. A proportional dental treatment index (DTI) was developed on the basis of sets of fillings placed and fillings needed.

Amplification of the nephrotoxic effect of cyclosporine by preexistent chronic histological lesions in the kidney.

In contrast to its importance in improving renal graft survival, nephrotoxicity is the most serious side effect of cyclosporine. Dosage reductions of CsA mitigate its nephrotoxic effect. However, CsA nephrotoxicity is still a problem in clinical transplantation. In this study the relationships among preexistent chronic histological lesions, donor age, and renal function after transplantation in 100 CsA treated renal allograft patients are described. Multivariate data analysis revealed a significant statistical effect of the presence of preexistent chronic histological lesions on nonimmediate function (-.42, less than 0.0001) that was relatively twice as large as the effects of cold ischemia time (-.21, less than 0.05). Also in predicting creatinine clearance at 1 and 3 months, preexisting chronic histological lesions had the most important effect (-.39, less than 0.0001; -.57, less than 0.0001, respectively), whereas the presence or absence of rejection episodes was less predictive (.24, less than 0.001; .20, less than 0.01, respectively). Preexistent chronic histological lesions had also significant effects in predicting conversion to azathioprine and prednisolone, because of CsA nephrotoxicity (-.42; less than 0.0001). The degree of CsA nephrotoxicity therefore varies according to the presence of preexistent chronic histological lesions. Because of the increase of these lesions with age, one has to be aware of an exaggerated CsA toxicity, especially in the older donor age group and in older patients treated with CsA for various reasons.

Antibiotypes of Bacteroides gingivalis assessed by antimicrobial disks and multivariate analysis.

Antibiograms with 20 different antimicrobial disks were studied for antibiotyping of Bacteroides gingivalis isolates. The stability of the antibiotypes was tested by passage in mice. Several B. gingivalis isolates of the same subject were used to investigate the presence of different antibiotypes in one individual, while isolates from different subjects were used to investigate individual differences. The antibiotypes were found to remain stable after animal passage. All tested strains of different origin represented different antibiotypes. The isolates from one subject all belonged to the same antibiotype. Principal component analysis of the data showed that two factors were important in the discrimination of the strains of B. gingivalis. One included beta-lactam antimicrobial agents that affect the cell wall. The other included antimicrobial agents that inhibit synthesis of protein and nucleic acid. Both principal component analysis and discriminant analysis proved to be of great use in the reduction of the amount of data and the visualization of the relations between different antibiotypes of B. gingivalis in a linear map. Among the investigated subjects, different antibiotypes of B. gingivalis were found, indicating that in the mouth of an individual, one antibiotype of B. gingivalis predominates and that different persons harbor different antibiotypes of B. gingivalis.
