A tan induced by 8-methoxypsoralen-long wave ultraviolet light has proved an effective photoprotective sunscreen in 5 patients with long wave ultraviolet light-induced polymorphic light eruption.
A 33-year-old woman is described who suffers from an idiopathic loss of mid-dermal elastic tissue which leads to wrinkling of the skin and to discrete perifollicular protrusions. In accordance with Sheley & Wood (1977) we conclude that these findings represent a new entity for which we propose the term 'non-inflammatory dermal elastolysis'.
Hyperuricemia occurs frequently in patients with psoriasis. An increased purin breakdown due to the enhanced epidermal turnover was stressed as a reasonable explanation. To prove this theory serum uric acid, cholesterol, triglyceride levels and the average body overweight were determined in 318 untreated psoriatic patients and the parameters were correlated with the extent of psoriatic skin involvement. In more than 100 psoriatic patients treated by oral photochemotherapy (PUVA), uric acid serum levels were examined additionally after the PUVA clearing phase and during PUVA maintenance treatment. The present study demonstrates: (1) There is no relationship between the frequency of hyperuricemia and the extent of psoriatic skin involvement, indicating that the increased epidermal turn over may not play a role in psoriatic hyperuricemia. (2) The most reasonable explanation for elevated uric acid in psoriasis seems to be a combination of genetic predisposition and hyperalimentation. (3) No significant change was found in the incidence of hyperuricemia under the influence of photochemotherapy.
Nineteen patients with mycosis fungoides (m.f.), without involvement of lymph nodes and/or internal organs, were treated with oral photochemotherapy (PUVA). After four to five weeks of PUVA therapy (four irradiations/week) complete remission of erythematous and infiltrative plaques occurred; tumorous m.f. lesions also responded to treatment but required longer treatment times. After complete resolution of m.f. lesions the patients were controlled regularly, the observation periods ranging from 6 to 27 months. When recurrences occurred the initial treatment schedule was resumed. Recurrences, more often seen in the tumorous m.f. stage, responded to PUVA equally well as the initial lesions. PUVA therapy of m.f. is thus more effective than conventional UVB-irradiation and less problematic than treatment with cytotoxic agents or ionizing radiation. Present experience indicates that PUVA represents the treatment of choice in early stage m.f.

Experimental PUVA therapy in five different skin diseases showed excellent results in lichen planus and pityriasis lichenoides et varioliformis. Present experience indicates a favourable influence of PUVA also in pityriasis rubra pilaris and in lymphomatoid papulosis. In Darier's disease PUVA leads to acute exacerbation.
Serum levels of 8-methoxypsoralen (8-MOP) were determined 2 hours after oral administration and found to vary considerably among psoriatic patients. There was no definite correlation between the 8-MOP serum level, the dose of 8-MOP/kg body weight ingested, and the minimal phototoxic UVA dose.
It has previously been shown tha UV irradiation can give rise to fresh skin lesions in the uninvolved skin of patients with pemphigus. In this study no increase of acantholytic activity was found in an organ culture model system for pemphigus in vitro, after in vitro and in vivo irradiation under various experimental conditions. By immunofluorescence no increase in IgG binding in irradiated specimens was observed. From these in vitro findings, which contrast the effect of UV light in pemphigus in vivo, it may be concluded that the circulatory system of the skin plays a major role in the photo induction of acantholysis in pemphigus.

28 male patient with uncomplicated acute gonorrhoea were treated with a single intramuscular injection of 2.0 g spectinomycin. All of them were cured. The suitable alternative to penicillin in the treatment of uncomplicated gonorrhoea, but it will not cure simultaneously-acquired syphilis. Its use should, therefore, be restricted to specific indications.
