Management of malignant pleural effusions.

Malignant pleural effusions (MPEs) represent a common complication of advanced malignancies. However, adequate palliation of this highly symptomatic accompaniment to cancer can be achieved in most patients by adopting the appropriate therapy. Several options are available for the treatment of MPE. Systemic therapy may control the effusion in patients whose underlying malignancy is sensitive to anti-cancer agents. Repeated thoracocentesis can be appropriate for patients with limited life expectancy or slowly recurrent effusions. In the majority of the remaining cases the treatment of choice is pleurodesis with sclerosing agents administered via tube thoracostomy. Controversy still exists as to which drug produces the best results: talc and bleomycin appear to be among the most cost-effective agents. The debate over the best agent to be used for pleurodesis refers to the difficulty in comparing results of studies using different eligibility criteria, response assessment and end-points. This article describes the various treatments which have been reported in the literature to play a role in the management of MPEs. It is also aimed at providing guidelines in allocating patients to appropriate treatments.

Systemic drug therapy of malignant pleural mesothelioma.

Malignant pleural mesothelioma (MPM) is an uncommon malignancy characterized by a rapid clinical course. Few patients are possible candidates for radical surgery. According to most reviews, radiotherapy has a limited role in the treatment of MPM. The role of chemotherapy in the management of pleural mesothelioma still remains uncertain. The available data indicate that although 10-20% of patients are known to achieve on objective response to a number of chemotherapeutic agents, the impact on survival appears limited and improvement in the quality of life remains uncertain. The results of combination chemotherapy are comparable to those of single-agent chemotherapy and no major difference is detectable among the various combinations. Prospective phase II trials are recommended for the identification of new active treatments while large-scale randomized phase III trials are needed to identify the best available treatment. In addition, new standard criteria for eligibility and response assessment are required. This paper reviews the available literature on the systemic drug therapy of MPM.

Thermal therapy of functional dyskinesias of the alimentary tract.

There are described the studies performed in Italy-especially at Montecatini-about activity of certain mineral waters, administered by mouth, in functional troubles of the digestive apparatus. The illnesses especially considered have been: idiopathic chronic constipation, irritable colon syndrome, biliary dyskinesias, correlated pathological conditions. The mineral waters analyzed had been: salso-sulphate-alkaline, bicarbonate, sulphate, bicarbonate-sulphate-alkaline, sulphate-bicarbonate waters and others. There are reported: research methods employed, the obtained results, the possible mechanisms of effect. The hydrological favourable influences about pathophysiology of digestive motor activity and in therapeutics of correlated diseases are demonstrated. The clinical results, and particularly their duration in time, are affermatif about therapeutic usefulness of thermal treatment by mouth. It's possible to suggest and to stress their better knowledge in Medicine and their increased employment in the treatment of functional dyskinesias of the alimentary tract.

Balneology

[Extension and features of parks and gardens around spas in Italy: explorative census; possible therapeutic usefulness II].

There are described the results of a census about parks and gardens own to 27 italian Spas. The considerable areas of green around Spas, the correct ratio whith the number of subjects in thermal treatment and other arguments represent favourable elements for italian medical Hydrology. In our census the found plants generally are in conformity with their correct geographical placing. Moreover they are in order to cause favourable effects for environmental Psychology and Therapeutics. The species of recommended plants for the increase and for improvement of green around italian Spas are mentioned. The importance of green around Spas in thermal Medicine and in Ecology is stressed.

[Parks and hydro-mineral springs in the classical world and in the contemporary era. Place in therapeutics].

Are briefly remembered the healing significances of woods and springs--together and separately--in ancient times. At first there were religions cults of them (health Goods, water and woods deities, temples, etc.). Subsequently there were their medicinal, lay uses especially of water. The Spas and Thermae of ancient Rome are well known. Importance in therapeutics of parks and gardens around Spas begins in XVIII century, in Europe. Greater care of parks close mineral waters springs was taken in XIX century, in North America (U.S.A., Canada). Here their importance in Ecology and Medical Hydrology was stressed. Today the use and diffusion of thermal parks can be proposed in order to environmental Psychology and to environmental Therapeutics.

[Observations on organic components of thermal mud: morphohistochemical and biochemical studies on lipid components of mud of the Terme dei Papi (Laghetto del Bagnaccio, Viterbo). Chemical bases of the interpretation of biological and therapeutic actions of thermal mud].

In previous findings the lipidic fractions extracted acc. to Folch from the mature muds of the majority of the Italian thermal springs hot baths was studied, with the aim to identify the organic substrates of their therapeutical activity. The organic components of the "mature" peloids are produced by the metabolism of the microphytozooplankton growing spontaneously in the clay-substrate, in contact with the hot water. The Popes thermal springs (Bagnaccio's Lake) are characterised by an unique environmental situation, because the muds are naturally matured in the hot thermal water, but not in artificial baths. The morphohistochemical aspects of thermal algae growing in the Bagnaccio's lake have been studied by means of Computerised Optic Probe Video-Microscopy, using a not contact zoom objective 70-400x. Peloid types, both the "white" and the "black" contains yellow pigments, fragments of hyphae, monocellular algae, Diatomeae, Cyanophyceae and few other species. The biochemical aspects of the muds extracts are characterised by the presence of Phospholipids (PC, PE, PS, SP), a series of Hydrocarbons ranging from C30 to C38, Phytosterols (beta-sitosterol, Campesterol, Stigmasterol and traces of Cholesterol), Free Fatty Acids (Palmitic, Palmitoleic, Myristic, Stearic, Oleic and Linoleic, heptadecenoic and heptadecanoic) and Terpenes (beta-amirrhyne, 24-methylene-cyclo-arthanole). In our opinion, the therapeutic effects of the mature muds are related to its organic components, with special regards to Phospholipids, Phytosterols and Terpenes. The richness of these components in the Popes Thermal springs seems to be great interest in the dermatological and cosmetic applications, other then the traditional use.

Animals

Alternating bolus and continuous infusion 5-fluorouracil: a strategy to overcome resistance to this fluoropyrimidine in advanced colorectal cancer patients.

Focusing our effort on the importance of FUra scheduling we have tested the hypothesis that pulse and continuous infusion (CI) of the fluoropyrimidine have different mechanisms of cytotoxicity. Our initial approach was to compare the mechanism of resistance of a cell line resistant to a short term exposure to FUra (HCT-8/FU4hR) to that of a cell line resistant to a prolonged exposure to the fluoropyrimidine (HCT-8/FU7dR). Cytotoxicity studies showed that HCT-8/FU4hR cells were still sensitive to FUra given as a 7-d exposure, suggesting different mechanisms of resistance. Indeed, rapid recovery of TS activity after drug removal was evident in the HTC-8/FU7dR cell line while HCT-8/FU4hR cells were similar to the parental cell line with regard to both the degree of in situ TS inhibition by FUra and duration of inhibition after FUra removal. In contrast, labelling studies with [3H-6]FUra(4 h exposure, 100 microM) showed that the incorporation of the fluoropyrimidine into RNA is significantly decreased in HCT-8/FU4hR cells as compared to parental HCT-8 cells. Given the lack of cross resistance between the two schedules in vitro, a pilot trial was done on patients with colorectal cancer refractory to bolus FUra. On 15 patients failing after FUra+LV or FUra alone 1 PR, 3 MR, 3 SD and 8 P were observed, confirming a certain degree of activity of CI FUra in patients clinically resistant to bolus FUra. Based on this rationale, a phase II trial of schedule-oriented biochemical modulation of FUra in advanced colorectal cancer patients was conducted, employing a hybrid regimen of 2 biweekly cycles of FUra bolus (600 mg/sqm), preceeded by (24 h interval) methotrexate, 200 mg/sqm (in order to maximize the RNA effect of the drug) alternating with Fura continuous infusion, 200 mg/sqm daily for 3 weeks, modulated by leucovorin, 20 mg/sqm weekly bolus (in order to maximize the DNA effect). Thirty-three consecutive patients (median ECOG PS 1) with advanced measurable colorectal cancer and no prior therapy for metastatic disease entered the study, from February 1992 to August 1993. Three complete and 13 partial responses were obtained among these 33 patients (RR = 48%, 95% confidence limits, 31-66%). After a median follow-up time of 23 months, 16 patients are still alive. The median progression free survival and overall survival were 9.6 and 20.8 months, respectively. No toxic deaths or grade 4 toxicity occurred. The incidence of grade 3 toxicity per patient in any cycle was: mucositis 6%, diarrhea 3% and vomiting 3% for the bolus part and 21%, 3% and 6% respectively, for the continuous infusion part of the regimen. Hand-foot syndrome occurred in 27% of the patients treated with the continuous infusion regimen. In conclusion, this experimental and clinical project has generated a novel regimen of schedule oriented biochemical modulation that is twice as active and half as toxic compared to bolus FU+LV given with either the daily x 5 or the weekly schedule. This high clinical activity is very encouraging, especially considering that 1) consecutive patients were entered, 2) the responses were independently reviewed, 3) the progression free survival and survival were much longer than those actually reported for this disease, 4) the toxicity of the program, in particular the bolus regimen, was relatively low allowing further intensification.

Adenocarcinoma

[Motility of the gastric antrum and the gallbladder following oral administration of sulfate-bicarbonate].

The authors evaluate the influence of a sulphate-bicarbonate mineral water on gastric emptying and cholecystic motility. Nine healthy volunteers drunk at settled intervals and with same procedures equal doses of mineral water and tap water on alternate days. Ultrasonographic evaluation of the gastric emptying and cholecystic volumes was performed before and after mineral water and top water drinks at settled intervals. The mean values of the gastric and the cholecystic volumes were calculated and compared at the single times. The results showed a significant colecystocinetic effect of the mineral water compared with tap water. No difference was observed in the gastric emptying with the mineral and tap water. Our work contributes to the knowledge of the pharmacodynamic effects of the mineral water employed and confirms its usefulness in the syndromes based on alterations in bile secretion and bile excretion in duodenum.

[Ecology and thermal parks in the context of the environment. Psychological and possible therapeutic aspects: trends in medicine].

Thermal parks are considered whether on account of their favourable effects on human well-being or as possible patterns of qualificated areas. Information on italian so called "thermal" ecology are given. In according to viewpoint of environmental psychology, the various natural side effects are remembered; the influences exerted by wood and park lands--also as meteorological factors--on human beings are stressed. The possible trends in the treatment of some psychosomatic, cardiovascular, digestive and metabolic diseases are pointed.

Ecology

[Feto-maternal transfusion. Description of a case].

Feto-maternal transfusion is an uncommon cause of neonatal anemia due to the passage of fetal blood in maternal circulation, in an amount greater than 150-180 ml. The onset of anemia may be acute or chronic in relation to the speed of blood loss. The case that we report has intermediate features between the acute and chronic form.

[Behavior of various enzymes and serum electrolytes in long distance runners during training. Effect of a bicarbonate-sulfate-calcium water of medium mineral content].

The above study was intended to assess the effect of a bicarbonate sulfate alkaline water of medium mineral contents in long-distance runners during training. Several parameters relating to water-electrolyte and lactate-pyruvate metabolism, and to the patterns of serum enzymes of variable cellular localization were determined. Altogether twelve indices were taken into account. No significant changes were observed fron the start to the end of mineral water treatment. This goes to show that training was well balanced and did not impair the athletes' biochemical homeostasis. At the end of the treatment period, non negligible fluctuations of CCK and LDH were observed. This finding is not devoid of interest and deserves further study.

Adult

Rapid and simple high-performance liquid chromatographic determination of tricyclic antidepressants for routine and emergency serum analysis.

An isocratic reversed-phase high-performance liquid chromatographic procedure is presented for the simultaneous detection of desipramine, nortriptyline, imipramine, amitriptyline and clomipramine in serum. Drugs are extracted after sample alkalinization and separated from each other on an octyl reversed-phase with n-butylamine as mobile phase modifier. Detection is achieved at 254 nm. The recovery of tricyclic antidepressants (92-110%) has good precision, with a relative standard deviation of less than 5%. Being rapid and simple, the method is suitable for the emergency clinical laboratory.

Penetration of cefonicid into serum, salpinges, cervix and vaginal tissue.

Cefonicid is a new cephalosporin that, given in adequate concentrations is able to kill some pathogens such as Gram-positive cocci and many Enterobacteriaceae. In this study we have evaluated 40 patients receiving 1 g of cefonicid i.m. at various times before abdominal or vaginal hysterectomy. The results of the present study indicate that a single dose of 1 g of cefonicid given intramuscularly is useful as prophylaxis and treatment of gynecological infections.

[Perinatal listeriosis. A report of a case with a hydrocephalus outcome].

Perinatal listeriosis is a disease whose incidence has increased constantly in recent years. Like neonatal infection determined by B hemolyticus streptococcus, the disease appears in two forms: the first is the septicaemic form with early onset, the second form is meningoencephalitic with late onset. We describe a case of listeriosis with immediate onset in which, in spite of the prompt therapy and the normality of rachiocentesis, developed a purulent ventriculitis with outcome in hydrocephalus.

Combined Modality Therapy

[Formation and excretion of bile. An example of possible thermal "detoxification"].

The detoxifying activity of fraction III of the bile (anion choleresis) is stressed. The stimulation of bile formation by the drinking of mineral waters is apt to enhance this fraction indirectly. It is therefore thought that thermal therapy facilitates the secretory function of bile and the organic detoxification connected with it.
