[Intestinal subocclusion caused by lymphoma of the small intestine].

Primitive gastrointestinal lymphomas constitute a rare pathological event, may involve any part of the gastrointestinal tube, and normally belong to the non-hodgkin variety. They do not present characteristic symptoms as compared with what can be found in other gastrointestinal lesions whether neoplastic or inflammatory. Radiologically and endoscopically they do not evidence any particular aspects, so that they can be confused with other diseases (gastroduodenal ulcer, benign tumours, epithelial malignant tumours). For such primitive forms, radical surgical treatment constitutes the first treatment and undoubtedly the fundamental form, with which radiotherapy and chemotherapy may be associated. In addition to a review of the literature, the Authors examine a case of intestinal lymphoma complicated by a picture of subocclusion.

[Actinic lesions of the ileum].

After an examination of the literature regarding the causes of lesions from rays, their most frequent sites, the diagnostic difficulties and the various types of treatment, the paper draws attention to the increase in such lesions in relation to the use of megavoltages, and the advisability of exploratory laparotomy in previously irradiated subjects even in cases apparently labelled as neoplastic relapses.

[Comparative observations on the course of different types of necrosis].

Histological comparison between cryonecrosis and thermonecrosis has shown that cryonecrosis in generally better delimited and surrounded by a layer of finer and more regular granulation tissue than thermonecrosis. Moreover, the course of the cold lesion is faster and is less open to phenomena of suppurative septic and oedematous phenomena.

[An unusual case of digestive hemorrhage caused by gastric lipoma].

Gastric lipoma responsible for repeated haemorrhage was observed in a 50-yr-old woman. The relevant literature is reviewed and reference is made to the rarity of this form. Its main clinical signs include a latent course, haemorrhage, and invagination of the tumour in the duodenum. It is stated that surgical management is necessary, owing to the high percentage of diagnostic uncertainty, and the serious haemorrhagic and occlusive complications mentioned.

[Cryosurgery. Current status. Problems and future prospects].

As matters new stand, cryosurgery has certain definite indications in various fields, either alone or in association with other forms of treatment. It is probably the therapy of choice for 1st-stage haemorrhoids and a series of skin lesions. It is of palliative value in the treatment of tumours and/or an addition to conventional management. The same is true of neoplasm in various sites and stages. As far as the immunological response is concerned, the data available are inconclusive. Further investigation is therefore justified, both experimentally and in man, bearing in mind all the parameters that can be used to assess a possible immune response.

Environmental air pollution in an intensive care unit for nephrology and dialysis.

The quality of indoor air depends on external pollutant concentrations and on internal sources, such as heating and air conditioning systems, building materials, ventilation, cleaning products, personnel and their activity. This study assessed environmental air pollution in an intensive care unit (ICU) for nephrology and dialysis. Air-dispersed particulate pollution was measured using a gravimetric method and spectroscopic photocorrelation. Microbiological pollution was evaluated by passive and active collection. Particulate concentrations exceeded recommended limits in some of the environments. There was a prevalence of small particulates, which are the most harmful type of all. An overall evaluation of bacterial pollution showed low levels of contamination in some of the rooms. In none of the environments we were able to detect pathogens such as Aspergillus fumigatus, methycillin-resistant Staphylococci or toxin-producing fungi.

[Cryotherapy of hemorrhoids. Analysis of 40 cases].

An appraisal is made of the results of cryotherapy in 40 carefully selected cases of haemorrhoids. It is shown that the method is effective if practised in accordance with the correct technique and where specifically indicated. The incidence of possible complications and recurrences is also discussed. Points in favour of the method include its greater tolerability, the possibility of using it in out-patient management, and the fact that it permits an earlier return to work. Attention must be paid to the narrowness of its indications and the anorectal diseases that may often be present.

[Common bile duct calculosis and stenosis of Vater's ampulla. Results of surgical treatment of 307 cases].

A systematic account is offered of a personal series of patients operated for calculosis of the common bile duct, with or without stenosis of Vater's ampulla, over a period of 15 yr. Reference to the clinical and anatomopathological features of these cases is followed by a description of the criteria followed in the choice of the surgical approach. Recent and earlier results are illustrated and some comments are made with regard to the methods employed: ileal choledochotomy, transduodenal papillosphincterotomy, and choledochoduodenostomy). A preference is expressed for papillosphincterotomy in all cases where the correct indications are present.
