Drug-triggered pemphigus in a predisposed woman.

A 31-year-old woman with three pemphigus-prone antigens in her HLA haplotype (B7, DR4, DQw7) developed the disease soon after taking a pyrazolone derivative, viz. feprazone. The pemphigus lesions persisted despite withdrawal of the drug and worsened appreciably when she used ceftriaxone (a new cephalosporin with three sulphur atoms) for a bout of acute pharyngitis. Thiol groups formed from the metabolic breakdown of ceftriaxone are thought to have promoted acantholysis via a biochemical route. Genetic predisposition alone ('the soil') may be essential, though not per se sufficient for outbreak of pemphigus; the intervention of exogenous, heterogeneous factors ('the seed') often seems decisive in triggering full-blown disease.

[The importance of direct immunofluorescence in the diagnosis of oral lichen planus. A clinical study and proposal of new diagnostic criteria].

A series of 40 lesions of various kinds as lichen planus are examined using samples stained with trichromic test. HLA-DR expression with immunoperoxidase technique and direct immunofluorescence using the standard techniques. The aim of the study was to discover the usefulness for diagnosis of direct immunofluorescence, above all in histologically uncertain cases. The results suggest the view that direct immunofluorescence is essential for diagnosis of lichen planus, in all clinical aspects. The author concludes with description of clinical proceeding for differential diagnosis between white and erosive lesions of mouth.

Adult

[Cyclosporin in a bioadhesive formulation in the therapy of oral erosive lichen planus. A clinico-experimental evaluation].

This study illustrates the possible topical therapy of OLP with cyclosporine A under bioadhesive gel formula. The treatment was carried out on 6 patients for 8 weeks in much lower (48 mg/day) than those used by other authors. The novelty of this study lies in the use of a bioadhesive formula with carbosymethylcellulose which allows CsA to remain on oral mucosa for about two hours. The remarkable results have shown a reduction ranging from 50 to 80% of oral lesions, thus proposing its specific use in all the forms resisting usual therapy.

Administration, Topical

[The immunological basis of vaccination against carious disease. Microorganism interactions. Specific host defense system. Immunological protection of the oral cavity].

The authors describe the very complex interactions of the microorganisms of the oral cavity and among these latter and the guest organism. There are described the mechanisms of aspecific defense of the oral cavity and give some outlines of the anatomy and of the physiology of the organs and of the glandular structures of the specific immune defence of the oral cavity through blood and saliva.

Bacterial Vaccines

[Prophylaxis of carious disease. Outline of dietetic and pharmacological prophylaxis].

The authors schematically describe the possibility of specialistic sanitary intervention in prevention, primary secondary and tertiary. They, furthermore, discuss of the real possibility of dietetical and pharmacological prophylaxis with special regard to the proper use and on the mechanism of action of fluorine and other chemical antiplaque agents.

Dental Caries

[Goldenhar syndrome].

The AA. outline the principals features about the etiology, pathogenesis, clinic and therapy of the oculo-auriculo vertebral dysplasia or Goldenhar's syndrome, with the best attention about the most interesting hypotheses on the pathogenesis, like focal necrosis for hemorrhage in development of face.

Adolescent

[The immunological basis of vaccine prophylaxis of carious disease. Maternal-fetal relations. Defense factors specific to the host].

The authors discuss of the relation between mother-phoetus from an immunological point of view and the real protection at birth because of the passing through the placental philter of maternal immunoglobulins. Then the Authors discuss of oral tolerance and of the possibilities of administering by oral-enteric way and the immunity of the guest.

Acrocephalosyndactylia

[Orofaciodigital syndrome].

The AA. behaving the orofaciodigital syndromes, describe the main peculiarity of the etiology, anatomo-pathology, clinical features, diagnosis to the light of the late acquisitions about these.
