[Continuous monitoring of mixed venous oxygen saturation during mechanical circulatory support of the left ventricle in heart surgery patients. Pathophysiologic principles and personal experience].

The clinical use of mechanical supports for cardiocirculatory function assistance has permitted to treat a higher number of low output syndromes insensitive to pharmacological therapy. One of the most important problems arising during mechanical circulatory assistance, is the ability to know if the performance of the artificial device is adequate to match metabolic tissues demands. Continuous monitoring of mixed venous O2 saturation (MSvO2) has been proposed as the most complete and reliable tool to verify this correlation. The Authors analyse the problems connected with this kind of monitoring during left ventricular assistance with centrifugal pump in cardiosurgical patients and present 6 cases taken from their clinical experience.

Sobrerol enantiomers and racemates: solid-state spectroscopy, thermal behavior, and phase diagrams.

The characterization of the solid state of sobrerol enantiomers and racemates has been accomplished by a number of techniques on solid phase such as thermal analysis (DSC) and spectroscopy (IR, 13C NMR, and X-ray diffraction both on powders and on single crystal). Experimental and theoretical binary phase diagrams of cis- and trans-sobrerol enantiomers and their mixtures have been drawn and are discussed. Thermal analysis allowed, moreover, the detection of cis racemate polymorphism. Finally, the quantitative analysis of the cis racemate as an impurity of the trans racemate by means of microcalorimetric determinations is reported.

Two brothers with idiopathic membranous nephropathy and familial sensorineural deafness.

Genetic factors could play an important role in the pathogenesis of idiopathic membranous nephropathy, and a few cases of familial membranous nephropathy have been described: an increased incidence of some HLA antigens as DR3 and others has been reported. We present two brothers with idiopathic membranous nephropathy and sensorineural deafness. HLA typing was performed in the two patients and in the members of the family, and it showed the absence of linkage of an HLA antigen with the renal disease in the family.

[Peridural anesthesia and narcosis with propofol in thoracic surgery].

Twelve patients undergoing elective thoracic surgery because of lung cancer, have been studied. The anesthetic management included: continuous epidural anesthesia with bupivacaine (T6-T7), continuous infusion of propofol and vecuronium, mechanical ventilation with an oxygen/air mixture. We evaluated the perioperative analgesia and the cardiovascular side effects of two groups of patients differing for the position (supine or lateral) selected to administer the local anesthetic. We also analysed the most important characteristics of the recovery from anesthesia (degree of analgesia and consciousness, respiratory function and cooperation with physiotherapeutic manoeuvres). The authors conclude that, although the number of patients studied is limited, there are no significant differences between the two groups for intraoperative analgesia and hemodynamic imbalance; the anesthetic technique employed is a reliable alternative to classic balanced anesthesia, because seems to reply very well to the mayor purposes of thoracic surgery. Moreover it makes the operative room free from pollution caused by volatile anesthetics.

Preformulation studies on suppositories by thermal methods.

The application of differential scanning calorimetry (DSC) is described for investigating the interaction between trimethoprim and sulfamethoxypyridazine in suppository formulations containing fat bases (Suppocire, Novata, Witepsol). The thermal behavior of suppositories at various storage times is deeply influenced by the fat base and by interaction between active ingredients. Liquefaction time and dropping temperature were also evaluated for comparison and control purposes.

Fine needle biopsy of hepatocellular carcinoma: improvement in diagnosis by microhistology.

Percutaneous liver biopsies with fine aspirating and cutting needles were performed under ultrasound guidance to diagnose hepatocellular carcinoma in 112 cases. All the specimens were examined partially with cytologic methods and partially with microhistology. Cytology established the diagnosis of hepatocellular carcinoma in 83 cases and of generic malignancy in 12, with 9 false-negatives and 8 inadequates. Microhistology established the diagnosis of hepatocellular carcinoma in 92 case and of generic malignancy in 2, with 3 false-negatives and 15 inadequates. In all, the combined retrieval rate was 98% with only 2 cases of inadequate material and the diagnosis of hepatocellular carcinoma was possible in 105 cases (93.7%).

[Study of diet-induced thermogenesis using telethermography in normal and obese subjects].

It is well established that food ingestion induces increased thermogenesis, but the site in man of this increased heat production is still not known. Recent data suggest that brown adipose tissue or the liver may have an important role in this phenomenon. Since recent research provides some evidence to suggest that activation of thermogenesis in a specific organ or tissue may determine increased skin temperature in the surrounding region, we have studied skin temperature in the dorsal interscapular and in the liver regions using infra-red thermography in groups of normal and obese subjects before and after administration of oral glucose. In a separate group of normal subjects we studied oxygen consumption before and after glucose administration. While oral glucose produced a clear increase in thermogenesis, no ascertainable variation was observed in body temperature and, therefore, of heat dispersion in any of the regions studied in either normal or obese subjects.

IgA mesangial nephropathy associated with renal cell carcinoma.

The authors report the occurrence of proliferative glomerulonephritis with mesangial IgA deposits in a 47-year-old man, heavy smoker, affected with renal cell carcinoma at stage I. The relationship between neoplastic diseases and IgA glomerulonephritis is unresolved. It is possible that IgA mesangial nephropathy may occur as a paraneoplastic disease, but in this patient its association with renal cell carcinoma may be merely fortuitous.

Fine aspiration versus fine cutting needle, and comparison between smear cytology, inclusion cytology and microhistology in abdominal lesions.

Two hundred patients underwent ultrasound guided percutaneous fine needle biopsy of focal solid abdominal lesions using 22 gauge aspiration and cutting needles. The material obtained by aspiration needle was treated by smear cytology and by inclusion cytology, and that obtained by cutting needle by microhistology. The retrieval rate was 89% for aspiration needle (smear cytology = 89%, inclusion cytology = 83.5%) and was 83% for cutting needle; the combined diagnostic accuracy was 98%. The typing accuracy was 76% for smear cytology, and was 84% for inclusion cytology and microhistology. We conclude that: 1. to obtain the highest retrieval rate (98%) both aspiration and cutting needles are necessary, because the aspiration needle is more likely to secure necrotic or soft tissue, and the cutting needle, fibrous or hard tissue; 2. histologic treatment of the samples yields a higher typing accuracy: 84% vs 76%; however, smear cytology remains essential because it permits a much faster evaluation of the adequacy of the sample and because it may avoid histologic methods in 76% of cases; 3. the smear cytology + microhistology combination seems to be the best solution (retrieval rate = 97.5%), but the costs are much higher because the cutting needle is somewhat expensive. The best solution would be to use the combination smear + inclusion cytology (retrieval rate = 89%) and to reserve the cutting needle for when aspiration needle material proves to be inadequate.
