[Laboratory tests applied to transfusion problems. Identification of dangerous universal donors and their frequency].

We studied 504 subjects, whose blood group wes O; they were periodic or occasional blood donors from Transfusional Center A.V.I.S., Policlinico "Umberto I", Roma. Our research was based on the principle stated by Gandini: he states that the natural isoagglutinin concentration equal or superior to 1:200 and the presence of hemolytic isoantibodies may make universal donors dangerous. Our results indicated that the percentage of this kind of donors is 27.75%. Sice 17.26% of our studied subjects showed hemolytic antibodies, we consider it is sufficient to identify dangerous donors through a dosage of hemolytic isoantibodies.

Prognostic significance of different biological markers (DNA index, PCNA index, apoptosis, p53, karyotype) in 126 adenocarcinoma gastric biopsies.

BACKGROUND: The different clinical evolution of gastric adenocarcinomas with the same clinico-pathologic characteristics prompted the authors to investigate the prognostic significance of different biological markers. METHODS: One hundred twenty-six preoperative cancer gastric biopsies, selected according the stage evaluated after following gastrectomy, were examined for DNA content by means the static cytometry, proliferating cell nuclear antigen (PCNA), p53 mutation, apoptosis by immunohistochemistry and karyotype using fluorescence in situ hybridization (FISH) techniques. RESULTS: The gastric cancers, ail belonging at stage III, were adenocarcinomas histologically typed as: 26 well differentiated (G1), 45 moderately differentiated (G2), 43 poorly differentiated (G3) and 12 of undifferentiated type with signet-ring cells. The tumours showed a prevalence of diploidy (68%), a mean PCNA-LI of 4.8%, trisomy of chromosome 7 in 40% of the cases, low presence of apoptosis (30%) and p53 mutation (17%). Only apoptosis was significantly correlated to histological diagnosis (p = 0.009). A multivariate analysis showed that the DNA content and PCNA-LI were the only independent prognostic markers for survival (p = 0.005 and p = 0.0002 respectively). CONCLUSIONS: The evaluation of these two biological variables, especially of the PCNA index, on gastric cancer biopsies may be useful in predicting the aggressiveness of each tumor and in identifying patients in need of additional perioperative therapies.

Comparison of intravenous nifedipine and sodium nitroprusside for treatment of acute hypertension after cardiac surgery.

BACKGROUND: The present study was designed to compare the hemodynamic effects of nifedipine and sodium nitroprusside, intravenously administered to control acute systemic hypertension in the immediate postoperative period after open heart surgery. METHODS: The study was carried out on 16 patients who developed acute systemic hypertension, defined as mean systemic arterial pressure (MAP) values above 90 mmHg, within the first hour after ICU admission, which followed cardiac surgery for valvular and coronary disease. After control measurements during the hypertensive status, each patient was treated by alternating infusions of sodium nitroprusside and nifedipine in order to obtain MAP values of about 80 mmHg with each administration. All patients had preoperative left ventricular ejection fraction above 45%. RESULTS: Nifedipine produced a superior and significant increase in cardiac index (CI) (+28%) and stroke volume index (SVI) (+30%), and a greater decrease in systemic vascular resistance index (SVRI) (-39%), when compared with sodium nitroprusside [CI (+5%), SVI (no change), SVRI (-27%)]. On the contrary the right and left ventricular filling pressures were reduced significantly by sodium nitroprusside [pulmonary capillary wedge pressure (-30%), central venous pressure (-20%)], while nifedipine induced small and not significant changes in the preload values. No ECG changes suggesting ischemic myocardial events were observed in any patient. CONCLUSIONS: On the basis of these results it has been concluded that nifedipine affects primarily the arteriolar resistance vessels without significant changes in venous tone, which is on the contrary markedly reduced by sodium nitroprusside. The better improvement in cardiac index and stroke volume index obtained with the administration of nifedipine, makes this drug a good alternative to sodium nitroprusside for treatment of acute hypertension after cardiac surgery in patients with a good preoperative left ventricular function.
