A 48-year-old-woman had simultaneously a peri-arteritis nodosa (PAN) and a Waldenström disease. PAN's symptoms were quickly regressive, neuropathy excepted (with slowness) under treatment by cyclophosphamide and prednisone. Hemopathy is now stabilized. Such a pathological association is exceptional (2 other cases in literature). Is it accidental? No cryoglobulinaemia or circulating immune-complexes or HBS antigen were found. Immunofluorescence study of arteriolar lesions in muscle showed a polyclonal fixation in inflammatory granuloma and monoclonal (IgM lambda) on endothelial side. Passive deposit is not excluded, because research of specific activity of IgM lambda (against fibrin or endothelial cells) was negative. Waldenström's disease has probably facilitated apparition of necrotizing angeitis but mechanism is not proved.
The skin trauma due to the surgical incision used to get at the middle ear can produce an epidermic cyst. This thesis is backed up by four cases of epidermic cysts which appeared several years after an operation on the middle ear, and in three cases it was in the temporal region and in one case in the parotid region. It is therefore quite probable that such cysts can be mistaken for relapsing cholesteatoma. For the same reason, one can never exclude a priori an epidermic cyst in the middle ear or around the ear in a patient who has been operated, and in whom no epidermic tissue was seen during the operation.
From 1964 to 1992, 56 patients with cardiac myxoma have been operated on. Sixty seven operations have been made during which 85 myxomas have been excised. Five patients had been reoperated from recurrent myxoma: 4 patients 2 times, 1 patient one time. In this population we observe 7 cases of multiple myxomas implanted 7 times in many cavities. The study of this series displays a lot of risk factors: implantation outside the left atrium, multiple myxomas in one or several cavities, true recurrence of myxoma, young age of the patient, associated lesions as part of the Carney's complex. In this series, 13 patients, representing 5 families, have one or more of these risk factors. in two of them, a familial character has been demonstrated. Analysis of these special kinds of cardiac myxomas displays very closed analogy between familial myxomas and high risk group. These findings suggest that recurrent and/or multiple and/or familial myxoma represent the same entity.
Blood, hairs and sperm are tissues currently used in forensic science to carry out DNA fingerprints in the field of individual identification. Our study aims at showing that dental pulp can also be an exploitable DNA source. A quantitative and qualitative DNA investigation on a yield gel with sizing extracted from this tissue shows that the molecule appears to be in a great quantity and of a high molecular weight. The last characteristic allows the use of the restriction fragments length polymorphic technique to make DNA fingerprints, based on the individual variations of the restriction sites disposition, within the DNA molecule, for a given enzyme. Thus, for an enzyme, the length of the restriction fragments obtained and revealed by a probe, is visualized by autoradiograph. According to this technique, we compared the DNA fingerprints obtained on 8 persons with the probe MS43A, from both pulpal and nucleated blood cells. During the extraction of the third molar of each sample, a small quantity of blood was taken in the tooth socket. In the same time DNA was recovered. For a same person, the autoradiograph showed the same pattern for both blood and tooth sample. The DNA profile similitude confirms the possibility of using the tooth for genetic analysis. The interest in using dental tissues as a DNA source of individual identification falls within the particular character of resistance of this organ towards physical or chemical exterior aggressions. The tooth which until now was used in Anthropology and Forensic Science for its morphological aspects could be used this way for genetic study.
