Absence of treatment-induced second neoplasms after ABVD in Hodgkin's disease.

One of the most serious complications of successful treatments for Hodgkin's disease is an increased incidence of acute nonlymphocytic leukemia (ANLL) and other malignancies. A retrospective analysis carried out on 1032 consecutive patients with Hodgkin's disease admitted to our Institute between 1965 and 1978 and treated with radiotherapy (RT) or chemotherapy or both modalities revealed that within 10 yr from initial therapy. ANLL was documented in 3% of patients, and over a comparable period of time 7.9% of patients developed other malignancies. ANLL was observed only in patients treated with chemotherapeutic regimens containing alkylating agents and/or procarbazine either alone (2.3%) or associated with RT (4%). Other second tumors were documented in patients given RT with or without chemotherapy. No second malignancies were observed in patients given ABVD (adriamycin, bleomycin, vinblastine, and dacarbazine) with or without RT. The incidence of ANLL was higher in patients given chemotherapy as salvage treatment upon relapse following primary irradiation (6.1%) compared to patients initially treated with combined modality (1.5%). The difference, however, failed to reach statistical significance. Since our analysis supports the evidence of a major role played by alkylating agents, procarbazine, and RT in inducing second malignancies, regimens not containing there drugs or their administration through treatments of different intensity warrant careful consideration.

Sinoatrial function in old age.

The purpose of our study was to investigate the effects of old age on sinoatrial function. We analyzed data obtained from 35 normal adults who were divided into 3 Groups: A (20-40 years, n=11), B (41-61 yrs, n=12). We evaluated: mean sinus node cycle length, sinus node recovery time, corrected sinus node recovery time, effective and functional refractory periods (EARP and FARP) and sinoatrial conduction time. EARP and FARP in Group C were significantly longer than in Group A (P less than 0.0025 and P less than 0.0005, respectively) and in Group B (P less than 0.005 and P less than 0.025, respectively). The differences were significant also when the values were expressed in percent of SCL (Group A vs Group C: P less than 0.005 for EARP, and P less than 0.005 for FARP; Group B vs Group C: P less than 0.025 for EARP and P less than 0.025 for FARP). These were the only significant differences observed between the three groups. Our data indicate that in normal adults the aging process does not affect sinus node automatism and impulse spread to the atrium, while atrial refractoriness is lengthened significantly.

Haemodynamic effects of carbochromen.

Carbochromen increases coronary flow and cardiac output. A previous study has advanced the hypothesis that the latter may be due to afterload reduction. Fourteen patients with coronary heart disease have now been studied by means of radionuclide angiocardiography. Gated blood pool angiocardiographic data were collected in basal conditions and, without moving the patient, 80 mg of carbochromen were administered i.v. Data were collected again, following infusion, during 3'-6'(1) and 7'-10'(2) periods. Changes in the following parameters have been evaluated: LV ejection fraction (EF), LV ejection rate (ER), system pressure (BP), heart rate (HR), cardiac output index (CO), stroke volume index (SV), LV end-diastolic volume index (EDV), systemic vascular resistance index (SVR), regional LV wall motion. During period 1 a significant decrease was observed in BP and SVR, the other parameters remaining unchanged. During period 2 there was a significant increase in CO, SV, EF, ER and a significant decrease in SVR. BP was unchanged. No changes were ascertained in HR and EDV. Eight patients, in basal conditions, showed asynergy in the LAO projection. Three of these patients showed improved wall motion during period 2. A possible central action of carbochromen should be pointed out. This conclusion can be drawn by observing the increase in the pump indexes, while BP and SVR show a decrease and EDV and HR no change. The left ventricular wall motion improvement observed in some of the cases confirms the possibility that carbochromen is capable of improving cardiac contractility. This effect may follow the regional myocardial perfusion increase.

[Atrial refractoriness and its dispersion in different physiopathologic conditions].

Previous studies have demonstrated a prolonged and non-uniform atrial refractoriness in patients with both sinus node dysfunction and supraventricular tachyarrhythmias. However, they have not been able to define separately the influence on atrial electrophysiologic properties of isolated "bradycardia", of sinus node dysfunction and of supraventricular tachyarrhythmias. Therefore we have measured the effective and functional refractory periods at three different sites of the right atrium (high, middle and low lateral wall) in 16 normal subjects (N), in 9 patients with chronic asymptomatic sinus bradycardia (AB), in 9 patients with sinus node dysfunction but without evidence of supraventricular tachyarrhythmias (SSS) and in 10 patients with paroxismal atrial fibrillation (PAF). The study was performed both in sinus rhythm and during atrial pacing (120 beats/min) utilizing twice threshold stimuli. Dispersion of atrial refractoriness (D) was determined from the range of refractory periods measured at the three different atrial sites as the longest minus the shortest refractory period. Refractoriness at the high site of the lateral wall (parasinusal zone), mean values of the refractory periods obtained at the three atrial sites, and dispersion were compared among the three groups. Refractoriness at the two rates (sinus rhythm and 120 beats/min) was also compared. During sinus rhythm SSS, AB and PAF showed a significantly higher refractoriness than N, while only SSS and PAF showed increased D. Atrial pacing reduced refractoriness but not D in all groups. At the same driven frequency refractoriness of SSS and AB, and D of SSS and PAF were still significantly higher than those of N. Finally, it is noteworthy that during paced rhythm, single values of mean refractoriness of SSS did not correspond with those of N. In conclusion, our data suggest that: 1) sinus node dysfunction and supraventricular tachyarrhythmias exert an independent influence on atrial electrophysiologic properties; 2) the presence of a less homogeneous recovery of atrial excitability should be considered as a possible concause in the genesis of atrial fibrillation; 3) chronic isolated sinus bradycardia seems to be characterized by a longer atrial refractoriness and not by an increased D; 4) contrary to results obtained in animal subjects, there does not seem to be a relation between D and cycle length in man.

Influence of conjugated oestrogens on circulating oestradiol, oestrone, LH, FSH and prolactin levels in postmenopausal women.

The effects on plasma 17-beta-oestradiol (E2), oestrone (E1), LH, FSH and prolactin (PRL) levels of 1.5 mg conjugated oestrogen, administered daily per os for 20 consecutive days, was investigated in six postmenopausal women aged 60-68. Both E2 and E1 increased progressively and significantly (p less than 0.001) from 18 and 28 pg/ml to 32 and 108 pg/ml, respectively, at the end of treatment; five days after the last dose both E2 and E1 had fallen to pretreatment levels (p greater than 0.05). LH and FSH decreased progressively and significantly (p less than 0.001) from 114 and 105 mlU/ml (before therapy) to 43 and 36 mIU/ml, respectively, after oestrogen administration. One week after interruption of treatment, both LH and FSH were significantly higher (p less than 0.001) than that obtained at the end of therapy. No significant variation (0.05 greater than p greater than 0.02) was observed for plasma PRL during and after oestrogen administration. Such results indicate that in postmenopausal women the specific enzymatic mechanism of oestrogen interconversion are maintained and that there is no increase of prolactin. IN this was, the possible effects on the development of breast cancer by elevated levels of this hormone, usually observed during long-term oestrogen therapy, would be avoided.

Electrophysiologic effects of carbochromene in man.

Sinoatrial and atrioventricular node functions were evaluated by the electrode catheter method in 13 patients with coronary artery disease before and 10 min after the i.v. infusion of 80 mg carbochromene. The drug had no effects on sinus node automatism, lengthened the atrial functional refractory period, and restored a 1 : 1 AV conduction at the same atrial rates at which a Wenckebach phenomenon occurred. These effects on atrial functional refractory period and AV conduction may be useful in the treatment of patients with coronary artery disease who present atrial or junctional arrhythmias.

Primary malignant neoplasms associated with chronic lymphocytic leukemia.

Over the past 2 decades there has been an almost exponential increase in the frequency with which cases of leukemia associated with another primary malignant lesion have been reported. In this study we reported the occurrence of a second primary neoplasm to 82 consecutive cases of chronic lymphocytic leukemia (CLL) admitted to the Istituto Nazionale Tumori of Milan from September 1962 to December 1978. In 16 of these (19.5%), an assoicated neoplasm was diagnosed subsequently (8 cases) or concurrently (8 cases) to CLL. Head and neck carcinomas and breast cancer had the highest incidence (5 and 3 cases, respectively). The results of this study further support the hypothesis that patients with CLL are prone to develop subsequent cancer. The defective cellular and humoral immunity in CLL may have an etiological role in the development of an additional primary malignancy. Although alkylating agents are known carcinogens in experimental animals and man, our results support the lack of a correlation between treatment with alkylating agents and incidence of second primary neoplasms, as demonstrated by Greene et al. (10).

[Clinico-immunological studies of 32 cases of bone tumors].

We have studied 32 patients with different bone neoplasias. 18 had osteogenic sarcoma. Clinical and immunological observations led us to consider our results as a general approach to the problem. No specific conclusions can be made. We describe 3 patients treated with levamisole therapy and two with transfer factor. In single patients some useful observations are connected about relationship between treated and non-treated patients.
