[Color-coded Doppler sonography in thyroid diseases: initial experiences].

The thyroid gland was investigated in 43 persons (12 healthy volunteers, 31 patients with diseases of the thyroid gland) with a new colour Doppler system (angiodynography). In euthyroid persons (normal thyroid gland, diffuse or multinodular goitre) only a few vessels were detected. In cases with Graves' disease however, hypervascularization was evident. Autonomous adenomas had a hypervascular periphery, whereas our two carcinomas showed a high vascularisation in the nodule. Angiodynography might become an important diagnostic tool for the evaluation of thyroid glands. The number of radionuclide studies and punctures might be limited.

Photogrammetry of the muscles of facial expression.

The present study has attempted to determine the volume, square measure as well as length, width and thickness of the individual muscles of facial expression by photogrammetry. 15 fresh male head specimens were employed for a careful dissection of muscle layers. The volume was measured using the immersion procedure. The mean values of each parameter together with the standard deviation were summarized. The results suggest that this novelty in the representation of the mimetic muscles will facilitate the planning of corrective interventions in plastic surgery. Moreover, photogrammetry offers additional information on the required size and measurements of donor muscles.

[Needle electromyography with sonographic control of needle position].

High resolution ultrasound provides excellent imaging of mimic muscles. In contrast to clinical examination, single muscles can be well defined and by real time imaging be observed during function. Since an EMG-needle electrode is also visible the puncture of small muscles or muscle-segments can be achieved with great exactness under sonographic control. The result of the electromyographic studies can, therefore, be related to individual muscles or muscle-segments.

[Computerized tomography analysis of cement-free total hip endoprostheses].

16 patients with total hip endoprosthesis were investigated by high resolution computed tomography (CT) to analyse possible advantages of CT over conventional radiography. The quality of the examinations was good only in pure titanium systems: examinations of chromium-molybdenum alloys were severely deteriorated by strike artifacts. Positioning of implants is well defined in radiographs; CT provides better visualisation of the relation between implant and cortical bone. Evaluation of the contact of the shaft to the femoral corticalis is difficult in radiographs but facilitated in axial scans. Determination of relevance of CT analysis of total hip endoprosthesis remains subject to a future long-term follow-up study.

[Clinical imaging of the mimetic muscles using high resolution ultrasound].

In comparison to other techniques, high resolution ultrasound has proved to be a good method for clinical imaging of muscles. By free selection of the scan plane and multidimensional viewing it is possible to represent each muscle from origin to insertion and to observe them during function. Pathology produces different types of echogenity. By imaging an EMG needle electrode the exact position within the muscle can be demonstrated. The result of the EMG study can therefore be related to small individual muscles or muscle segments, which are below the size suitable for clinical examination.

Magnetic resonance imaging of the spine in multiple myeloma.

The lower thoracic and lumbar spine of patients with multiple myeloma was examined by magnetic resonance imaging (MRI), plain radiography, and bone scintigraphy. Three independent investigators evaluated the power of these diagnostic methods to detect bone lesions in 192 vertebrae from 18 patients and in 60 vertebrae from 7 controls. 41 foci with abnormal signal intensity were detected by MRI; X-ray films showed osteolytic lesions in 4 vertebral bodies; and bone scanning was positive in 2 cases. The superiority of MRI in detecting myeloma-associated focal bone lesions was statistically significant, and in one case the lesions were confirmed at necropsy. Deviations in shape and height of vertebral bodies were slightly more easily visible on radiographs. Early detection of imminent medullary compressions in 2 patients led to successful radiotherapy before symptoms appeared.

[Aseptic humerus head necrosis--tortuous path of a maker of scythes through expert medicine].

The case report is presented of a 50 year-old scythe smith who produced 1400 scythes daily in piece-work over a period of 17 years. He complained for the first time in 1977 about severe pain in his left shoulder. Six years after the onset of pain aseptic necrosis of the humeral head was finally diagnosed by reviewing old X-rays of 1977, but the patient had meanwhile absolved a troublesome journey through a variety of incorrect-medical diagnostic and therapeutic procedures. The patient's history, previous reports and the actual clinical findings observed in our department are presented in this paper. We consider that this patient is suffering from an occupational disease which should be legally recognised as such.

[Sonographic anatomy of the neck and its importance in lymph node staging of head and neck cancer].

Our experience with sonography of the neck (about 1,500 examinations) indicates that most of the structures that are of therapeutic importance can be demonstrated by ultrasound. The cervical nerves, however, cannot be differentiated sonographically from perivascular fat because there is insufficient impedance difference presented by the myelin sheaths. The typical sonographic anatomy of the anterior and lateral cervical muscles, of the major vessels, the thyroid and parathyroid and salivary glands has been analysed. The structures that can be shown by sonography have been related to dissections of the neck.

[Diagnosis of varices in liver cirrhosis: duplex ultrasound versus gastroscopy].

This study assesses the usefulness of duplex sonography in the diagnosis of oesophageal varices. 32 patients with liver cirrhosis were studied by endoscopy and duplex sonography. There was an excellent correlation in the case of the medium-sized and large-sized varices. Only small varices could not be detected by duplex sonography. On the other hand, 3 large-sized paraoesophageal varices, which were not identified by endoscopy, were easily seen by duplex sonography. The sensitivity of duplex sonography was between 0.8 and 0.9, the accuracy between 0.93 and 0.96.

[Diagnostic value of ultrasound in malignant melanoma].

High-resolution real-time sonography enables visualisation of the morphology of the cutis and of cutaneous tumours. Evaluation of 26 malignant melanomas showed that there is a high degree of correlation between the sonographically measured values of maximal tumour thickness with those determined postoperatively by histometry. As malignant melanomas have very few internal echos, they can be easily differentiated from benign tumours. High-resolution sonography is thus the only diagnostic imaging method which helps to evaluate preoperatively malignant melanomas.

[MR effects of x-ray contrast media].

Although Pantopaque has now been widely replaced by water-soluble contrast media for intrathecal application, retained residuals of oily contrast media are still a very common finding. Whereas other contrast media, e.g. barium in the GI tract, are inert in magnetic resonance imaging, the signal behaviour of Pantopaque may lead to false interpretations. Similar to fat, the T1 and T2 relaxation times of Pantopaque are short. Conventional radiographs of the spine are recommended in patients with intradural MRI findings similar to fat in case of previous myelography.

[Magnetic resonance tomography in the diagnosis of cerebral space occupying lesions].

197 primary and secondary cerebral tumours as well as tumour-like lesions are described. T2 and T1-weighted images were obtained. Magnetic resonance imaging (MRI) offers advantages in localisation and tissue characterisation. Localisation is improved by the ability to obtain axial, coronal or sagittal images and by the absence of artifacts due to bone in lesions in the posterior fossa. A significant advantage is the excellent contrast resolution of soft tissue. On the basis of our experience, we present an overview of the most common brain tumours using MRI.

[Sonography in cervical lymph node metastases].

The value of high resolution real time sonography for the detection of cervical lymph-node metastases in head and neck cancers was assessed in 118 histological proven cases. The accuracy of sonography (89%) proved to be superior to palpation (80%). In 9 patients a thrombosis of the internal jugular vein was detected. The ability of sonography to detect tumorous infiltration of the arterial vessel wall is discussed in 12 patients. Regarding possible artefacts, sonography is the only imaging modality which can solve this problem. Sonography should be performed preoperatively in all patients with malignant head and neck tumors for cervical lymph-node staging.

A new transducer technology--the annular phased array: comparison with a conventional mechanically rotating sector transducer: the upper abdomen.

During a 3-month period, 285 patients underwent ultrasonographic examination of the upper abdomen. Scanning was done with a new annular phased array transducer, and a conventional rotating sector transducer. Our study indicates that the annular phased array is more effective for outlining boundaries of the liver, spleen and pancreas in the near and far field. Owing to improved lateral resolution, the new technology permits US imaging of parenchymal organs, even in cases of severe bowel gas obscuration.

[Surface coils in magnetic resonance diagnosis of spinal space-occupying lesions].

190 patients underwent MRI of the spine. In 119 cases pathological changes were detected. 82 of these were located in the extradural compartment, 14 were found in the intradural-extramedullary space and 23 were situated inside the myelon. Routinely, spin echo sequence were performed in sagittal direction (T1-weighted images as well as T2-weighted images), in about 20% combined with transverse sections. All patients were investigated using surface coils of 20 X 40 or 15 X 45 cm; Using a field of view of 350 mm, spatial resolution was 1.3 mm per pixel. The quality of images obtained in this way was clearly superior to techniques using the body coil.
