Adenocarcinoma of a neovagina constructed according to the Baldwin-Mori technique.

The Authors describe a case of cancerization of a neovagina constructed according to the Baldwin-Mori technique, occurring 39 years after the initial operation. Description of the clinical case is followed by a number of anatomo-pathological considerations. The risk of cancerization and the adverse events associated with this type of neovagina militate against the use of autologous transplant operations in neovagina construction.

[Rehabilitation of burned patients. II. Therapeutic exercise].

After illustrating some major ideas concerning disability and the role played by the Rehabilitation Medicine, with particular reference to burns, the Authors consider rehabilitation. The main therapeutical rehabilitation measures used in the various stages of the disease are described. The Authors conclude by underlying the importance of an early and adequate rehabilitation as well as the need for a multidisciplinary approach since patients suffering from burns present a variety of medical, surgical and rehabilitation problems.

Burns

Transabdominal cervico-isthmic cerclage: state of the art.

The aim of this study was to evaluate the long-term results of transabdominal cervico-isthmic cerclage according to the Vecchietti technique in patients with cervico-isthmic incompetency diagnosed by hysterosalpingography. The patient sample consisted of 36 women operated on in the Verona University Institute of Obstetrics and Gynaecology from 1975 to 1981 and 18 women operated on in the Clinic of Obstetrics and Gynaecology, Lyon. The results in the Verona study population show that in 28 cases (77.7%) the operation was crowned with success by the birth of live, viable neonates; intrauterine death occurred in 2 cases (5.5%), miscarriage in 3 (8.3%) and sterility in 3 (8.3%). The Lyon results show that in 15 cases (83.3%) the pregnancies resulted in the birth of live neonates, while sterility occurred in the other 3 (16.6%). Overall analysis of the 54 patients shows that the abdominal cerclage was successful in 43 cases (79.6%). Only in 11 cases were negative results obtained: 6 cases of sterility (11.1%), 3 miscarriages (5.5%) and 2 intrauterine deaths (3.7%). The data presented demonstrate the validity of the operation when performed in non-pregnant women in the presence of proven cervico-isthmic incompetency.

Abdomen

Modulated Mouchel incision in obstetric surgery.

The Authors present 10 clinical cases in which a modulated Mouchel incision was performed over the period from 1.4.1989 to 1.4.1990 to achieve caesarean section. The postoperative course was uneventful in 9 cases with no complications. The only complication observed was slight left parametric inflammatory infiltrate in one case only. The laparotomy wound healed uneventfully in all 10 cases. Thus, there were no complications contra-indicating the use of Mouchel's technique, which presents demonstrable advantages in the execution of caesarean sections. These consist in rapidity of execution, no need for extensive subfascial detachment, ease of fetal extraction, straightforward reconstruction and an aesthetically valid final result.

[Rehabilitation of burned patients. I. General aspects].

The authors discuss the general aspects of problems connected with pathology caused by burns from a rehabilitation standpoint. The motivations that render all the rehabilitation effort more and more important for assisting the patient suffering from burns are reported. Then, brief considerations follow regarding the physiopathologic aspects of the healing process evolution. The authors also report about the main functional deficiencies caused by the pathologic involvement of the motion as well as of the respiratory system.

Burns

Surgical management of lung cancer with solitary cerebral metastasis.

Between 1964 and 1986, 19 patients underwent resection of both a primary lung cancer and the associated brain metastasis. One patient underwent resection of 2 separate primary lung cancers and the associated metastases. The 12 men and 7 women ranged in age from 42 to 67 years (mean, 54.6 years). The cell type was adenocarcinoma in 12 tumors, squamous or adenosquamous cell in 5, large cell undifferentiated or anaplastic in 2, and malignant carcinoid in 1 tumor. The types of resection were as follows: lobectomy for 12 neoplasms, pneumonectomy for 5, bilobectomy for 2, and wedge resection for 1 neoplasm. Radiotherapy to the brain was given in connection with sixteen of the twenty craniotomies. The patient with 2 separate primary neoplasms survived 19 years before dying 5 months after the second craniotomy. The mean survival is 8.0 +/- 2.1 years (+/- the standard error), and the median survival is 1.67 years. Survival at 1 year was 65 +/- 10.7% and at 5 years, 45 +/- 11.1%. On univariate analysis, the following factors were found to correlate significantly with longer survival: a lung tumor in Stage I or II; negative mediastinal nodes; curative rather than palliative resection of the lung tumor; and age younger than 55 years. However, on multivariate analysis, only curative resection was a significant factor (p less than 0.01). We believe these results justify continued application of this combined surgical approach to patients having limited-stage lung cancer with a solitary brain metastasis.

Adenocarcinoma

Low doses of recombinant interleukin-2 enhance delayed type hypersensitivity to PPD in mice infected with Mycobacterium bovis-BCG and fed a diet supplemented with vitamin A acetate.

Mice infected intravenously with high doses of M. bovis, strain BCG, showed a marked impairment of delayed-type hypersensitivity to PPD evaluated in vivo as skin test and in vitro as splenocyte blast transformation. Furthermore, this state of unresponsiveness is partially reversed either by the intraperitoneal injection of high doses of IL-2 or by feeding mice a diet supplemented with vitamin A acetate (VAA) at least 6 weeks before the BCG injection. This paper shows that low doses of recombinant IL-2 together with a supplementation of VAA in the diet at the moment of BCG infection is sufficient to induce in mice a positive skin reaction and blast transformation to PPD.

[Algoneurodystrophy].

The authors consider the extensive literature concerning a quite frequently observed clinical picture, that has been named in different way by each researcher. Algoneurodystrophy is the name used in this analysis. They make a review of the most important works appeared in literature. It points the contrasting aspects of the research made till now. Therefore, in authors' opinion, it's necessary for them to investigate with further studies various aspects of the disease (etiological, pathogenetic, clinical, therapeutic ones, etc.) associated with a prevention of the secondary and tertiary damage the earliest as possible.
