[Retrospective study of 88 cases of cancer of the cervical esophagus and definition of a new therapeutic approach].

Eighty-eight cases of cancers of the cervical esophagus were treated at the Institut Gustave-Roussy (France) between 1968 and 1980. They included 67 epitheliomas of the esophageal opening excluding those of the hypopharynx extending into the esophagus, and 21 epitheliomas of the actual cervical portion of the esophagus. Whenever possible, basic therapy was by radiotherapy (64 cases). There were no survivors after 2 years, possible explanations for this poor result being the selection criteria for treatment, the choice of radiotherapy as basic treatment, and the doses employed. A new therapeutic approach is suggested following a pathological study and an analysis of the different therapeutic possibilities available. Pre-operative chemotherapy is followed by total laryngopharyngectomy with total esophagectomy without opening of the thorax, performed under certain well defined conditions. Continuity is reestablished by gastropharyngostomy and postoperative radiotherapy is applied systematically. This therapeutic approach involving major surgery is applicable only for selected patients, others having to be treated by combined radiotherapy and chemotherapy alone.

Gonadotrophin-producing seminoma: a distinct category of germ cell neoplasm.

Gonadotrophin-producing seminomas can be identified in two ways: detection of human chorionic gonadotrophin (HCG) in the serum or urine, or histological identification of syncytiotrophoblastic giant cells, which can be shown to contain HCG by means of indirect immunoperoxidase techniques. A review of the histories of 14 patients with such tumours shows that they are associated with a worse prognosis than that of 'typical' seminomas, and tend to present with a more advanced stage of disease. These tumours are thus distinct, clinically and biologically, from typical seminomas, and their treatment needs to be planned accordingly.

[Combination of radiotherapy and chemotherapy in the treatment of upper respiratory and digestive tract carcinomas (author's transl)].

The authors set out in detail the conditions for which adjuvant chemotherapy is used in the treatment of carcinomas of the upper respiratory and digestive tracts. The conditions are at the same time of a theoretical (natural history as shown by cell kinetics) and of a practical (general physical state) nature. They evaluate 34 randomised therapeutic trials and 14 phase II trials: chemotherapy given either before, during or after radiotherapy. They finally draw several conclusions concerning the use of adjuvant chemotherapy.

[The treatment by radiotherapy only of carcinomas of the margin of the larynx. Experience of the Institut Gustave-Roussy. 161 cases (author's transl)].

From 1955 to 1970, 161 squamous cell carcinomas of the épilarynx were treated at Institut Gustave-Roussy by radiation therapy alone. Most part of the patients were referred to radiotherapy for local extensions (21 cases), or medically poor conditions (73) forbiding surgery. Only 50 cases were really suitable to radiation therapy. Results are poor. 30% at 3 years, 15% at 5 years. Analysis of different groups (indications, size, of the tumor, nodes, mobility), gives a better appreciation of the possibility of radiation therapy. The results are in agreement with literature. Application of combination of surgery and radiotherapy permitted by improvement of anesthesia is probably better in same cases, but many of the patients with epilarynx are in so poor conditions, that besides patient suitable to radiation therapy (bulky tumors, type of epiglottis with normal mobility) many of the patients are in too poor conditions to receive surgery.

A retrospective study of 131 cases of carcinoma of the posterior pharyngeal wall.

Between 1958 and 1973, 131 patients with carcinoma of the posterior pharyngeal wall were treated at the Institute Gustave-Roussy (127 men, 4 women, mean age 61 years). Nine had a tumor of the oropharynx, 37 of the hypopharynx and 85 of the oro-hypopharynx. Nine had T1 tumors, 19 T2 and 103 T3 (TNM-UICC 1975). 122 patients were treated by irradiation: 83% were T3 lesions, 81% were more than 4 cm in diameter, 80% were ulcerated and 42% were staged N3. The survival rate of these patients was very poor (3% at five years). The nine patients who had been treated by surgical excision had a better survival rate and three are alive at 5 years, but the initial prognostic factors were better in this group. It seems that the indications for surgery should be widened to increase the survival rate in these poor prognosis cancers.

[Testicular choriocarcinomas masked by predominating seminomas].

Testicular choriocarcinomas may be disguised by the histological appearance of pure seminomas with apparently normal HCG levels. As demonstrated by the 5 cases reported here, a definite diagnosis by further histological specimens, more often by the development of abnormal HCG levels, may be delayed. Early chemotherapy appears to offer the best chance to such patients. Suspicious signs: HCG levels at the limit of normal before castration, suspect histology (marked hypertrophy of the interstitial gland, zones of necrosis, an unusual clinical appearance with large lymph node lesions, an unusual course with early lymph node or metastic recurrences, may suggest the presence of choriocarcinoma and lead to routine chemotherapy.
