[Double-blind evaluation of the effectiveness of cimetidine in the arrest of hemorrhages due to gastroduodenal peptic lesions].

40 patients with an endoscopic diagnosis of bleeding gastric or duodenal lesions have been examined in a double blind study. The haemorrhages were subdivided into slight and serious on the basis of objective parameters. Cimetidine (1.2 g/die i.v.) or placebo in a preparation indistinguishable from the drug were administered to patients. Arrest of the haemorrhage was evaluated on the basis of clinical and endoscopic criteria, and the study was pursued up to 96 hours from the start of bleeding. The effect of cimetidine proved greater than that of the placebo in serious haemorrhage, particularly where due to gastric lesions, but no differences were seen between drug and placebo in the slight haemorrhages.

[Postoperative treatment of liver injuries in the surgical intensive care unit].

Personal experience in the postoperative treatment of patients with liver traumas is reported, and a comparison is made between those treated in ordinary wards and those admitted to intensive care units. The latter had shorter stays (24 days as opposed to 40) and fewer postoperative complications, assuming a comparable picture of preoperative shock and associated lesions. The difference is attributed to the possibility offered by such units of providing closer surveillance and more rational management.

[A case of leiomyosarcoma of the rectum].

Leiomyosarcoma of rectum is a rather rare malignant tumour. Generally, it manifests late when pain, tenderness, rectal bleeding and obstruction occur or ulceration through the rectal mucose with bleeding or discharge occur. Rectal examination leads to the diagnosis of malignancy. The histological report confirms the type of tumor. A case of leiomyosarcoma of the rectum has been reported. Radical resection (Miles) is the treatment of choice at present. The prognosis is poor.

[Splenic artery aneurysm. Considerations on 2 cases].

Two cases of splenic artery aneurysm, their etiopathogenesis, complications and surgical treatment are presented. When serious technical problems are present the necessity of distal pancreatectomy, is underlined. Postoperative course is normally satisfactory.

[Catheterization of the subclavian vein by the supra- and subclavicular routes. Retrospective study in 105 patients].

Central venous catheterisation via the supraclavicular and subclavicular route was performed in 61 and 44 cases respectively at the department's intensive therapy unit. A statistical assessment was made of the modalities of application, complications, time of indwelling, and reasons for removal. A large number of more serious complications (pneumothorax, haemothorax, arterial puncture) were noted with the supraclavicular technique, whereas subclavian catheterisation permitted longer residence times and was rarely responsible for serious disturbances, even though there were more instances of poor positioning. A preference is expressed for the second method. Mention is also made of employment of the internal jugular in recent cases, though these are not yet sufficiently numerous to enable any conclusions to be drawn.

Estrogen receptors in colorectal adenocarcinomas and in other large bowel diseases.

Seventy-two patients with tumor and ten with non-neoplastic colon disease were studied for the presence of estrogen receptors (ER) by three different methods. Only seven specimens (six primary adenocarcinomas and one recurrent cancer) had an ER concentration above 3 fm/mg of cytosolic protein, with no sex, age and tumor stage correlation. Our results suggest that the large bowel does not contain a cytosolic receptor for estradiol.

[A rare case of Merkel's tumor with intestinal metastases].

A rare case of intestinal metastasis of a neuroendocrine carcinoma of the skin (Merkel Cell Tumor) is reported. The primary tumor was excised from the left arm two years earlier and followed an aggressive course with lymph-nodes, tonsillar and intestinal spread. Histologically, the intestinal lesion was similar to the primary tumor with small, undifferentiated cells. Undifferentiated small cell carcinoma and lymphomas of the gastrointestinal tract are to be considered in the differential diagnosis. Merkel Cells Tumor usually affects head and neck (44%) and extremities (48%), and is a highly malignant neoplasia, with early recurrence and distant metastases. Wide surgical excision of the primary lesion is the treatment of choice, while controversial is the role of prophylactic regional lymphadenectomy. Adjuvant radio- and chemo-therapy is frequently associated.

[Use of the computer in intensive care. A program for instruction and diagnosis of changes in acid-base equilibrium].

A method for assessing the acid-base balance in patients admitted to the Intensive Care Department, making use of the Graphic Display option of a minicomputer is presented. Patient condition can be plotted on present graphs whose coordinates are the pH and PACO2 measured at successive moments. It is thus possible to visualise the behavior of acid-base balance over a period, as well as the extent of renal or respiratory compensation. The data relating to a group of patients are reported by way of illustration.
