[Contribution of ultrasonography to the study of retroperitoneal tumors in children (author's transl)].

Ultrasonography investigations of retroperitoneal tumors in children were conducted in 25 cases: 11 nephroblastomas, 11 neuroblastomas, 2 ganglioneuromas, and 1 rhabdomyosarcoma. Ultrasonographic exploration of such tumors should include analysis of the mass: echostructure, limits, interfaces and dimensions, and analysis of its extension. The latter should determine the position in relation to the median line, the condition of the large vessels (inferior vena cava and aorta), the homolateral renal cavities and the contralateral kidney, and the presence or absence of glandular spread and hepatic metastases. The semiology of each of these elements is described and discussed, to determine their individual diagnostic value.

Primary retroperitoneal teratoma in adults: presentation of two cases and review of the literature.

Retroperitoneal teratomas are rare in adults; they are malignant in 25.8% of cases. The authors report two benign cases in which radiologic examination was essential in establishing the diagnosis. Irregular calcifications, which are visible in 61.5% of reported cases, cannot be considered a sign of benignity, since 12.5% of calcified tumors were malignant. Because of the indeterminate course of the disease, surgical removal is indispensable.

[Ultrasonography appearance of recessive hepatorenal polycystic disease in children (author's transl)].

Six cases of recessive hepatorenal polycystic disease in infants and young children were explored by intravenous urography and ultrasonography. Sufficient specific ultrasonography findings for making a diagnosis were obtained. The kidneys are large and produce strong echos, with inversion of the normal strength of medullary when compared to cortical echos, and poor definition of the renal sinus. Strong echos are also obtained from the liver, especially in the juvenile forms, because of the large amount of fibrosis. Large intrahepatic biliary ectasias were discovered in a 3-year-old child.

[Cricopharyngeal achalasia, exceptional cause of dysphagia in newborn (author's transl)].

Dysphagia of the newborn and the infant presents with many varied causes, neurological, malformative, turmoral or functional. Among the latter, cricopharyngeal achalasia is an exceptional entity open to discussion. Suspected as early as the first month, presenting with difficulty in swallowing with choking episodes and coughing up of food without any obvious etiology. Profile roetgengraphic studies will allow the diagnosis to be made. The characteristic sign is a round and regular posterior narrowing on the posterior wall of the esophagus (C4 - C5 level) with enlargement of the hypopharynx. This narrowing is a constant finding. This pathological entity, an absence of relaxation of the cricopharyngeus muscle during the third period of swallowing, well-known in the adult patient, has only exceptionally been described among newborn children. In the following case study, cure has only been obtained by an extra mucosal myotomy of the esophagus at the age of seven months. The discussion, considering the five cases reported in pediatric journals, covers the diagnostic elements, the physiopathology of the disease, the risks presented during evolution without treatment. Therapy must be precocious and surgical.

[Renovascular hypertension in infancy treated by aortorenal by-pass (author's transl)].

The writers record a severe case of renovascular hypertension resulting from fibromuscular dysplasia which appeared in the only main left renal artery. The first clinical symptoms were noticed at five months. The failure of medical treatment led to exploratory surgery at seven months with renal revascularisation by an aortorenal by-pass using a hypogastric arterial autograft. The writers stress the scarcity of hypertension symptoms among infants of less than one year and conclude the impossibility of prognosis in spite of an apparent cure after six months.

[Broncho-alveolar spread in a case of juvenile laryngeal papillomatosis (author's transl)].

The authors report the case of a 30-year-old man suffering from juvenile laryngeal papillomatosis, recurrent since the age of 4. 23 years later, multiple round opacities were found in the lungs, progressing slowly to excavation. Bronchoscopy with broncho-aspiration provided histological confirmation of the diagnosis of broncho-alveolar spread of the papillomatosis, made on the basis of plain films and tomography.

[Exploration and removal in focal infections of the liver (apropos of 32 resections in a 4-year period].

The surgeon can no longer be satisfied today, with a diagnosis of focal lesion of the liver. It is also necessary to give the precise localisation of the nodular image compared with the segments and vessels of the gland. The lesional topography is usually determined by coeliac arteriography but, in certain cases, complementary cavography becomes necessary. On knowledge of this hepatic angio-cartography, depends the choice of approach and the nature of the surgical operation, for there are easy methods of approach where, for example, removal may reasonably be attempted and, other segments which are difficult or impossible to approach, e.g. segments VIII and IV posteriorly. The authors report their experience of surgery of the liver between 1971 and April 1974 for 14 abscesses, 15 hydatid cysts, 5 tumours and 9 cases of trauma. They carried out 32 resections, 22 of the right lobe of the liver, 10 of the left lobe. The immediate results are studied in relation to the indications and techniques used.

[Radiology of tumours of the region of the ampulla of Vater (author's transl)].

On the basis of 18 personal cases, the authors discuss the different radiological appearances of tumours of the ampulla of Vater region. After a review of the histopathological aspects, they attempt to separate the respective indications for the various endoscopic and radiological studies possible, stressing the importance of duodenoscopy. They give an illustrated classification of the different appearances seen.
