Surgical castration combined to cyproterone acetate treatment in patients with advanced prostatic carcinoma, gives satisfactory clinical results in 75% of the cases, with a follow-up period of 12-59 months. Histological tumour grading, tissue dihydrotestosterone (DHT) concentrations and DNA distribution by flow-cytometry in the tumoral cells are significantly correlated with the clinical response to hormonal therapy, whilst cytosol androgen receptors are not. 17 cases with DHT levels greater than 1.8 ng/g tissue are in remission (COR, POR or OS), whereas 5 cases with DHT levels less than 1.8 ng/g are in progression. In the responder group diploid DNA pattern is found in 20 cases and aneuploidy in 7, whilst in the not responder group aneuploidy was found in 5 cases and diploidy in 2. In conclusion histological grading and measurement of DNA and DHT in tumour tissue may be complementary in the prospective identification of the hormonally dependent tumours, which may benefit of the combined endocrine therapy suppressing androgens of testicular and adrenal origin.
Clinical and experimental evidence indicates that ureterosigmoidostomy is associated with a high risk for the development of colonic cancer, while there is no reported evidence of increased risk in patients who undergo urinary diversion of other types. In the present study the histochemical and lectin binding characteristics of goblet cell mucin were investigated in biopsy specimens from patients who had undergone ureterosigmoidostomy and from patients who had undergone rectal bladder surgery. Specimens from transitional mucosa surrounding colonic cancers and from normal rectal mucosa were also studied. For histochemical studies the high iron diamine-Alcian blue method was used. FITC-conjugated Dolichus biflorus agglutinin (FITC-DBA) and Arachis hypogaea agglutinin (FITC-PNA) were used for the study of lectin binding characteristics. In contrast to the striking increase in numbers of sialomucin-containing goblet cells found in the patients who had undergone ureterosigmoidostomy, the mucin proved to be histochemically normal in the rectal bladder surgery group. Abnormal lectin binding patterns were observed in colorectal mucosa after urinary diversion of both types, with the abnormalities consisting of dramatic decreases in FITC-DBA labeling (compared with controls) and the appearance of substantial numbers of FITC-PNA-labeled goblet cells. These findings indicate that the pattern of mucin secretion is definitely abnormal in patients who have undergone urinary diversion. Whether this abnormality is an indicator of premalignant changes remains to be established. These data, however, confirm that endoscopic and histologic follow-up studies may be of value in assessing the risk for the development of cancer in these patients.
Photon induced X-ray fluorescence has been employed to determine trace elements in urinary calculi. Twenty-one urinary stones, classified in four groups, have been analysed. Besides Ca, elements Fe, Zn and Pb were detected, and Cr and Mn were also detected in some cases. No clear differences were found between the four groups.
Multiple agents have been suggested as urinary bladder carcinogens. Of these, certain aromatic amines and some tryptophan metabolites (Anthranilic acid, 3 hydroxy Anthranilic acid) have been proved to be carcinogenic. Others are suggested but are not conclusively proved as carcinogenic agents. Various animal species have been used as experimental models for the study of induction of cancer of the bladder and substantial progress has been made in the search for etiologic factors and pathogenesis of bladder tumors. A carcinogenic model of cancer bladder induction is proposed. However, knowledge of the tumorigenesis of cancer of the bladder is still limited and fragmentary. It appears, however, that Tryptophan dysmetabolism play a major role in this field and future experimental and epidemiologic studies should take this fact into consideration.
Three patients operated upon via median sternotomy for resection of bilateral lung metastases are presented. 2 show no evidence of disease 30 and 42 months after operation, one died 10 months after surgery. The literature on the subject is reviewed; median sternotomy is considered a quick, safe and reliable route, indicated when resection of bilateral lung metastases is to be carried out.
X-ray spectrophotometry shows undoubtful advantages versus conventional chemical methods employed in the analysis of kidney stones. In fact, it is possible to detect little crystals that are very important in the genesis and treatment of urinary calculosis.

The age-adjusted mortality rate for prostatic carcinoma in Italy between 1969 and 1978 was calculated, as was the regional distribution for this disease. An overall increase in mortality was found, but the increase of the age-adjusted data was not statistically significant. The regional distribution of age-adjusted mortality rate showed a difference between northern and southern Italy, even when errors that can affect these findings were taken into consideration. At present, however, the size of these errors is not known. Such factors should not, however, prevent development of useful information and indications, but they should not be overlooked in an analytical interpretation of the data.

The hypothesis of hormone dependence of human renal cancer, based on experimental and clinical data, has recently been supported by estradiol-receptor (ER) and progesterone-receptor (PR) studies. ER and PR, found in experimental renal cancer as well as in normal human kidney and in human renal cell carcinoma (RCC), have been measured in 27 RCCs from patients submitted to surgery and endocrine therapy, in an attempt to predict the response to progestational therapy . Of these 27 tumors, 59% were positive for ER and 59% for PR; 37% were positive and 19% were negative for both ER and PR. The follow-up of 23 patients so far investigated showed that progestational therapy, commenced in 18 patients, has given favorable results in 14 patients and negative results in 3 patients with ER-PR- renal cancer. Antiestrogenic therapy, started after nephrectomy in 1 patient with ER+PR- renal cancer and lung metastases, failed since the patient died 8 months after surgery.
