[Hereditary epidermolysis bullosa and laryngeal involvement].

Two cases of epidermolysis bullosa (EB) with laryngeal involvement are reported in 2 Algerian girls, aged 6 and 7 years. The first one with a junctional EB, received local treatment with good results. For the second, with an EB dystrophica, tracheostomy was necessary and remained so. Laryngeal involvement is rare in EB. It and remained so can be serious and life-threatening. Eight cases where previously reported. Their clinical and therapeutical particularities are discussed.

[Protracted otitis and subacute mastoiditis in children. A prospective study apropos of 118 cases].

Antibiotherapy has lead to a substantial decrease in the number of infants with acute mastoiditis or complications thereof. However, the non-negligible incidence of protracted otitis currently observed has caused one to raise several pathogenid hypotheses, among which subacute mastoiditis figures as one needing to be detected early. This prospective study was conducted on 118 children treated for protracted otitis in the ENT Service at Hopital Trousseau, during the period from january 1987 to december 1988. It allowed us to develop diagnostic and therapeutic strategies to cope with the difficult problem of protracted otitis, taking into account bacteriologic findings and risk factors, among other things.

[Otorhinolaryngologic involvement in cystic fibrosis].

ENT involvement is very frequent in mucoviscidosis, particularly rhinitis and sinusitis. This prospective study, which spanned from September 1st, 1988 to August 31st, 1989 anc included 27 children with cystic fibrosis of pancreas was carried out with the intent of determining the incidence of the various associated diseases, their bacteriologic profile, and the optimal therapeutic procedure. Less than a child in 5 is symptomless when the nose and sinuses are affected. Polyposis is found to occur in 6/27 cases, and all children have sinus radio-opacities. Ear disease is rare (1/4 of cases) and is manifested by asymptomatic tubal dysfunction with minimal audiometric repercussions. Bacteriologic specimens of sputum and sinus purulent discharge show simultaneous germ positivity (10 times out of 11). Our therapeutic recommendations are based on our own experience as well as literature data, and are, in our opinion, only relevant for children with clinical signs of disease, as opposed to advocating systematic treatment of sinus foci found on X-ray.

[The carbon dioxide laser in tracheobronchial diseases in children. A prospective study of 11 cases].

Carbon dioxide laser has been used in bronchology for over 15 years, primarily in adults and older children. With the recent development of appropriate bronchoscopes, one has now been able to extend its application to infants and small children. 11 patients, aged 10 months through 7 years, were treated by this technic at the ENT Service of the Hôpital Trousseau (Paris). Indications for broncholaser management at this age and the particularities thereof are specified, as well as the results obtained and the complications linked with its use. This new therapeutic tool appears to be reliable and effective in a certain number of cases, representative of which are circumscribed bronchostenoses, granulomas resulting from trauma or associated with tuberculosis, and benign tumors.

Bronchial Diseases↗

[Severe congenital laryngeal stridor. Endoscopic surgical treatment].

Congenital laryngeal stridor or laryngomalacia is a congenital disease causing an usually shrill and solitary inspiratory noise, sometimes associated with disorders of deglutition and dyspnea when crying. Most often, the symptoms spontaneously disappear before the age of two. However, some cases are very severe, with permanent dyspnea, leading to tracheal intubation or tracheotomy. To avoid the risks of prolonged tracheotomy in infants, a new surgical technique has recently been described, beside epiglottectomy and hyomandibulopexy, now abandoned. This is simple endoscopic section and resection of the ary-epiglottic folds. Preliminary results in 15 patients seem to show the superiority of this so-called epiglottoplasty technique. Except in one case with supraglottic oedema attributed to a major gastro-esophageal reflux, all patients recovered from their dyspnea with this procedure. Extubation usually was rapidly feasible and the post-operative period was uneventful. The patients are kept in hospital for 2 to 5 days, and an antibiotic and anti-reflux treatment is recommended. This procedure is advocated as a treatment of choice of "laryngomalacia" with severe dyspnea.

[Surgical protocol of velo-palatal and labio-nasal clefts].

Based on a series of 52 cases of velopalatine and labionarinal clefts treated according to the same principle of therapeutic chronology, the authors define their recent surgical protocol for the primary repair of these clefts. Satisfactory functional repair of the tongue and platysma muscles of the face determine the good anatomical results and, in particular, the development of the maxilla together with the results on speech and middle ear pathology. Pathophysiological elements argue in favour of staphylorrhaphy as the first operative stage in unilateral labiovelopalatine clefts associated, at the same time, with cheilorhinoplasty; passive orthodontia is generally performed until the secondary palatoplasty. The surgical repair technique is closely related to this therapeutic timing in terms of the muscular and periosteal detachments.

[Conservative surgery of cancer of the larynx. Cordectomy, hemilaryngectomy, supraglottic laryngectomy].

Of a total of 219 partial laryngectomies performed, 119 were cordectomies, 30 hemilaryngectomies and 70 supraglottic laryngectomies. Results of each type of operation are presented with emphasis on problems of local recurrences, evidence of validity of procedure selected and of its indications. Analysis of results, with the reservation that indications were sometimes wide but always reasonable, showed an extremely low incidence of recurrences, the quality of the resection (limited or insufficient) having finally little incidence on the latter, except perhaps in the supraglottic laryngectomies. It is suggested, therefore, that no attempt should be made to "recover" a histologically detected insufficient excision by complementary radiotherapy not required in principle. This assists follow up review and allows secondary therapy if an unfavorable course is noted.

[Functional surgery of cancer of the hypopharynx. Hemilaryngopharyngectomy, posterior pharyngectomy by bilateral cervical approach].

The authors report on 48 patients operated by hemi-laryngo-pharyngectomy for spinous carcinoma of the piriform sinus. They try to precise the indications by analysing the factors influencing survival and relapses. They compare surgery to the other methods. In a second part, they report 3 cases of posterior pharyngectomy by bilateral cervical incision.

[Functional radical neck excision. Evaluation of 10 years' experience. Apropos of 434 cases].

A total of 434 functional évidements of neck were conducted in 313 patients, taking into account all primary localizations. Details of the procedure used are described while emphasizing the need for an évidement tending towards one as complete as a radical operation. Surgery was usually elective (89% of cases) but in 11% considered as essential. A high proportion of lymph nodes classified as no were, however, found to be invaded (22.8%, including 27.5% of these with capsule rupture). Despite this, glandular recurrence on the side of functional évidement was extremely low (less than 7%) even when recurrence was epithelial or involved a second localization. These findings emphasize the validity of functional évidement and confirm published data, known to be difficult to analyze. Although the value of this operation with respect to the cancer is well established, the actual functional interest, particularly in relation, to external branch of XI th nerve is debatable.

Nasal fossa malformations and paramedian facial cleft: new perspectives.

Choanal atresia may be associated with other cranio-facial malformations, including various degrees of nasal fossa malformation, and may be a part of paramedian facial clefts (as described by Tessier et al. [1977]). We identified five such cases with combined clinical elements corresponding to Tessier's paramedian facial cleft, including eyelid coloboma, mild to severe choanal and nasal fossa anomalies, ethmoidal hypoplasia and anterior skull base malformation, sometimes with proboscis lateralis and half-nose hypoplasia. These observations incited us, first, to elaborate a conception which accounts for the likely embryological mechanisms involved; second, to propose a new classification based on anatomical and pathogenic embryological considerations; and last, to propose the use of transpalatal approach to restore choanal permeability, since endonasal laser therapy is particularly dangerous in such cases.
