[Methods of isolation and determination of urinary 4-thio-6-hydroxypyrazolo(3,4 d)pyrimidine in patients treated with thiopurinol].

Oxithiopurinol, [4-thio-6-hydroxypyrazolo(3,4-d)pyrimidine] has been caracterized in urines of patients treated with Thiopurinol : the principle involved is the precipitation of both oxithiopurinol and uric acid in combination with cupric hydroxide. The urinary oxithopurinol content is determined by means of anion and cation exchange resins. 56 to 70 per cent of the daily dose of Thiopurinol are eliminated in urine as oxithiopurinol.

Soluble complexes and antiglobulin factors detection by immunofluorescence and immunoadsorption in rheumatoid sera.

In this report is was demonstrated that in rheumatoid factors (RF) positive sera, 19 S IgM rheumatoid factor can form soluble complexes with different proteins (IgG, albumin) of sera. In these complexes the antiglobulin activity of IgM is not inhibited. When immunofluorescence and immunoadsorption procedures are used for the detection of antiglobulin activities of rheumatoid sera, the proteins which are bound to IgM rheumatoid factor, even if they are devoid of any antiglobulin character, may be revealed simultaneously with IgM. Moreover in some cases the detection of IgM may be hindered, while the linked proteins remain detectable. In these conditions, these complexes in RF positive sera may give false negative results for IgM rheumatoid factor, and may give rise to artefactual appearance of IgG and other proteins (albumin antiglobulin-like activities. This paper points out that before investigating IgG and IgA antiglobulin activities, IgM rheumatoid factor should be previously eliminated, for example by immunoadsorption.

[Treatment of hemophilic arthropathies by medical synoviorthesis].

The effects of medical synoviorthesis were studied by the authors in the haemophiliac. The use of radioisotopes was limited in view of the young age of the patients but a good result was achieved for the 5 elbows treated and for the 4 knees (yttrium 90) the results were 2 good, 1 fair and 1 failure. Osmic acid was injected in 44 knees. The results were good and fair in 51% of 39 knees. The results can be improved by a second injection. Synoviorthesis should be kept for joints with very few destructions because of the danger of worsening the osteocartilaginous lesions, specially with osmic acid.

[Paget's disease and vertebral blocks (apropos of 7 cases)].

The authors observed 10 blocks of from 2 to 8 vertebrae in 7 patients whose spines were affected by Paget's disease. The morphological characteristics of these blockages were for the most part similar to those of acquired synostoses, the corresponding articular apophyses appearing to be fused in the majority of cases. Even without a radiograph taken before the formation of the blocks, the fact that there had been progression of the synostosis between certain vertebrae indicates the probability of an acquired origin. The prevalence of these vertebral blocks in a group of 76 patients suffering from Paget's disease of the spine (12 percent, or 9 patients with 13 blocks) was significantly higher than that found in a population of 200 persons not suffering from Paget's disease, (3 percent, or 6 patients with 6 blocks) and among the 13 blocks discovered in the patients with Paget's disease only 3 were formed of vertebrae unaffected by deforming osteitis. These facts suggest that the formation of a vertebral block may be favoured by Paget's disease localized in the vertebral column. This is a little recognized idea even though it was formulated in Schmorl's work at the beginning of this century.
