Basal bone resorption in the rat fetus related to the hormonal status of the mother.

Fetal bone resorption was measured by an organ culture technique using fetuses from thyroparathyroidectomized (TPTX) pregnant rats to investigate the roles of fetal and maternal hormones in the regulation of basal bone resorption in utero. Thyroparathyroidectomized female rats were treated with thyroxine and/or metabolites of vitamin D3. Basal bone resorption was greatly increased in fetuses from TPTX mothers. Administration of 1,25-dihydroxycholecalciferol or 1,24,25-trihydroxycholecalciferol to TPTX female rats normalized the basal bone resorption of the fetus whereas injection of 24,25-dihydroxycholecalciferol was without effect. Treatment of the TPTX mothers with thyroxine was also shown to normalize basal bone resorption in the fetus.

Hidden anti-nuclear antibodies in rheumatic diseases.

Hidden anti-nuclear antibodies are demonstrated by immunofluorescence using smears of rat nuclei as substrate and rat liver section technique when sera are incubated with penicillamine. The non-detection of hidden anti-nuclear antibodies by tissue sections in the absence of a splitting agent may be due to the formation of high molecular weight complexes between rheumatoid factors and anti-nuclear antibodies. These high molecular weight complexes containing anti-nuclear antibodies do not have access to tissue nuclear antigens, but can react directly with free nuclei. It is postulated that anti-nuclear antibodies may represent the early pathway of both rheumatoid arthritis and connective tissue diseases. The demonstration of hidden anti-nuclear antibodies in seropositive sera indicates that rheumatoid factors may have a protective effect. It may explain dissimilarities observed in the clinico-immunological profile of rheumatoid arthritis and systemic lupus erythematosus. The splitting effect of penicillamine observed in vitro may be similar in vivo. It can explain clinical improvement and immunological side effects observed in rheumatoid arthritis patients treated with this drug.

[Rheumatic manifestations of erythema elevatum diutinum].

The authors report on a case of erythema elevatum diutinum developing at the same time as a seropositive rheumatoid polyarthritis. Disulone, moderately active on skin lesions, has little influence on the development of arthritis that have reacted well to treatment with D-penicillamine. A review of the literature makes it possible to identify the type of rheumatismal manifestations and immunological disorders accompanying the dermatosis. Three etiological circumstances must be looked for : focal infection, malignant lymphopathy, connectivitis.

Induction and inhibition of bone matrix resorption.

A method is described by which the action of hormones on cartilage and bone matrix in mice is ascertained by measuring 35S release from bone explants. Parathyroid extract was found to increase the rate of 35S release in vitro as well as in samples from pretreated animals, whereas calcitonin and 25-hydroxycholecalciferol were effective only in vitro.

[Humeral lytic lesions and circulating anticoagulant in secondary syphilis (author's transl)].

A case of secondary syphilis revealed by rheumatic complaints is described. Arthralgia and myalgia were associated with lytic lesions and periostitis of both humerus. A more diffuse skeletal involvement was detected by bone scanning. A genital ulceration 4 months before, diffuse lymphadenopathy and a strongly positive serological reaction ascertain the diagnosis. The association of a circulating anticoagulant is discussed. All manifestations cleared with penicilline treatment.

[Fourier transform analysis of glycolipid depots in Fabry's disease].

An application of the Fourier Transform Process to the Fabry inclusion study has been made. The study, which rests upon five tissues and eight persons, brings out a new low frequency stratification. Various degrees of freedom exist for the basic molecule association. The periods are given for the three fundamental directions of the inclusion.

[Lipid thesaurismosis rheumatism. Arthropathies in alpha galactosidase A deficiency (Fabry's disease). Ultrastructural study of the synovial membrane demonstrating microcrystals in the mitochondria of synoviocytes].

Articular manifestations may be the onset of genetic alpha galactosidase deficiency (Fabry's disease). Ultrastructural study shows typical osmiophilic lamellar inclusions of trihexosylceramides in synoviocytes, capillaries and adipocytes. Furthermore microcrystals identical to those seen in Gaucher's disease and type II hyperlipoproteinemia were observed in mitochondria and free in cytoplasm. These data suggest a microcrystalline pathogenesis of these arthropathies, as in gout and chondrocalcinosis, and what we have generally called crystallopathic arthropathies.

[Rheumatism caused by lipid thesaurismosis. Articular involvement in hyperlipoproteinemia type II : presence of microcrystals in the mitochondria of synoviocytes].

For 13 years polyarthritis with specific synovial involvement was observed in a case of type II hyperlipoproteinemia. Microcrystals similar to those described in Gaucher's and Fabry's disease were seen in synovial cytoplasm and mitochondria. These data suggest a microcrystalline pathogenesis for type II hyperlipoproteinemia arthritis as in gout and chondrocalcinosis.

Interactions between IgM antiglobulins and IgG antinuclear antibodies. Some aspects of D-penicillamine activities.

In this report we describe an in vitro masking action of IgM rheumatoid factor (IgM-RF) towards IgG antinuclear antibodies (IgG-ANA) which can be recovered by using D-penicillamine (DP) as an unmasking agent. The mechanism of this masking effect was elucidated by using smears of rat free nucleus as substrate, instead of the classical rat liver cryostat sections technique. It was postulated that the 'inhibition' of IgG-ANA by IgM-RF may be due to the formation of high molecular weight complexes (HMWC); this would be the same in vivo. Allowing the formation of HMWC which can be removed from the circulation, IgM-RF may have a protective effect by preventing or minimizing systemic lesions. Therefore, IgM-RF may be considered as a defence response against potentially noxious ANA or antigen-antibody complexes. Induced nephropathy and the high frequency of detection of ANA observed in rheumatoid arthritis (RA) patients treated by DP may be the result of the dissociating effect of this drug on HMWC, in which the activity of pre-existing ANA is hidden when rat liver sections are used for its detection.

[Phalangeal osteoid osteoma (13 cases)].

Thirteen cases of osteoid osteoma of the phalanxes are reported: 10 fingers and 3 toes. Any phalanx may be affected, with no preference for a particular site, in the young adult. Segmentary tumefaction of the soft parts near the lesion is a more consistent sign than pain, which is often quite minimal. Radiographies demonstrate the increase in the size of the phalanx, whose matrix is condensed. All radiographic techniques must be used to determine the exact location of the nidus, that is always very small (1 to 2 mm), and whose total removal is the only guarantee of cure.

[Criteria for the evaluation of the antirheumatic (antiinflammatory) drugs in man].

Comparison of 5 antiinflammatory drugs and placebo in a double blind, short term, cross-over trial conducted in patients suffering from rheumatoid arthritis. 15 measurements were used and studied. With classical unidimensional statistical tests it was not possible to discriminate active drugs between them. Multidimensional analysis (correspondance analysis and discriminant analysis) are more sensitive and allow us to distinguish active drugs with only 11 criterion.
