Histamine2 antagonists in allergic rhinitis. Possible role in the control of cellular immune reaction.

We studied effects of monoclonal antibodies on lymphocyte subpopulations in patients with allergic rhinitis treated with histamine2 antagonists. The OKT4/OKT8 ratio after treatment showed a statistically significant decrease compared with initial values, the expression of a relative increase in the OKT8 subpopulation containing the suppressor fraction. In control patients no significant variations were observed. These results confirm our hypothesis that histamine2 antagonists act in allergic rhinitis by modulating the activity of T-suppressor lymphocytes reduced in atopic patients.

Stable isotope studies of zinc absorption and retention in young and elderly men.

Zinc absorption was determined with 67Zn and 70Zn, stable isotopes of zinc, in six young men and six elderly men who were confined to a metabolic unit for 12 wk. Their purified formula diets, supplemented with select food items, contained 15 mg of zinc per day. Zinc absorption was determined twice for each subject by combining zinc enriched with either 70Zn or 67Zn with the formula diet. Serum, urinary, and fecal zinc and zinc balance were determined by atomic absorption spectrometry. Zinc absorption averaged 17% in elderly men, significantly less than average zinc absorption of 31% in young men. Serum zinc was also lower in elderly men and increased in both groups during the course of the study. Zinc balance did not differ between groups, and endogenous zinc losses were less in the elderly than in the young men. The results suggest that while zinc absorption is less in elderly men than in young men, the lower absorption may reflect a lower requirement for absorbed zinc by the elderly. Alternatively, less efficient zinc absorption could result in decreased endogenous losses.

Long-term prognosis of survivors of Pneumocystis carinii pneumonia. Structural and functional correlates.

Limited data exist detailing the long-term sequelae of Pneumocystis pneumonia. Open lung biopsies were obtained in seven renal transplant recipients within 48 hours of the onset of respiratory failure. Biopsy specimens and simultaneous chest roentgenograms were graded without clinical information according to the severity of alveolar damage and pulmonary infiltrates, respectively. Evaluation of pulmonary function and exercise physiology were performed 15 to 21 months after their illness. Pulmonary function indices were normal except FRC (2.65 +/- 0.56 L or 77 +/- 16 percent of predicted) and Dsb (20.0 +/- 7.2 ml/min/mm Hg or 79 +/- 19 percent of predicted). Two patients developed arterial desaturation with exercise. Alveolar damage scores correlated with later exercise arterial desaturation (r = 0.88, p less than 0.05). Simultaneous roentgenographic scores correlated with later abnormalities of Dsb (r = 0.81, p less than 0.05). Mild residual abnormalities of pulmonary function were found in five of seven adult survivors of Pneumocystis pneumonia. These abnormalities correlated with pathologic and radiographic features of the acute illness.

Pirenzepine versus ranitidine in gastroprotection during antiblastic chemotherapy: a double-blind study.

Corticosteroids and cytostatic drugs possess a documented lesive action on upper digestive mucosa, making epigastric pain and/or pyrosis common complaints among patients on antitumor treatment. The selective antimuscarinic pirenzepine and the H2-receptor antagonist ranitidine were tested against the ulcerogenic action of antiblastic chemotherapy. Thirty-eight patients affected with malignant lymphoproliferative disorders were endoscopically examined and the endoscopic pictures were quantified by using an arbitrary score. According to a double-blind randomized sequence, 19 out of the 38 patients received pirenzepine 100 mg/die/p.o. and the other 19 received ranitidine 300 mg/die/p.o. along with antitumoral therapy for periods of 3-6 months. Seven patients died of hematologic complications before ending the treatment. In the 31 surviving patients (13 pirenzepine- and 18 ranitidine-treated) a second endoscopic examination was performed at the end of the treatment and the lesion score repeated. No significant difference was found between initial and final scores in both groups. The antisecretive action of the two drugs may account for their effectiveness, but other mechanisms such as cytoprotection cannot be ruled out.

Zinc, copper, and iron balance in elderly men.

Two-3-wk balance studies for zinc, copper, and iron were carried out in six elderly male subjects who were confined to a metabolic unit for a long-term study (12 wk) with constant dietary mineral intake. Average zinc balance was 0.1 +/- 0.05 (mean +/- SEM) and serum zinc increased in all subjects during the course of the study. Average copper balance was 0.06 +/- 0.06 (mean +/- SEM). There was no significant change in serum copper ceruloplasmin, but all were within normal ranges except for elevated serum copper in one subject who appeared to be in negative copper balance. Iron balance appeared to be negative, -0.44 +/- 0.16 (mean +/- SEM), despite improved blood iron parameters in five of six subjects. Results of this long-term balance study suggests that current dietary recommendations of 15 mg of zinc and 2 to 3 mg of copper are adequate for elderly as well as younger adults. However, balance data for individuals must be interpreted with caution, should be used only with other parameters, and should not be relied on exclusively as a basis for dietary recommendations. Additional data are needed to evaluate dietary iron recommendations for elderly men.

Beta-thalassemia in Brazil.

1. Clinical, hematological, genetic and peripheral blood globin synthesis studies were carried out on 17 symptomatic Brazilian thalassemics and their parents who live in the northeast of São Paulo State. The group included 8 beta zero-thalassemia homozygotes, 7 carriers of at least one beta + gene, one delta beta zero/beta zero-thalassemia double heterozygote and one beta zero homozygote also carrying the alpha-chain variant Hb Hasharon (alpha 2(47) His beta 2). 2. The mean non-alpha/alpha ratio for globin biosynthesis of the patients lacking HbA (beta zero homozygotes and delta beta zero/beta zero double heterozygotes) was 0.26 +/- 0.11 (mean +/- SD), which is not statistically different from the value of 0.32 +/- 0.06 obtained for the carriers of at least one beta + gene. In contrast, the mean non-alpha/alpha ratio for the thalassemia major patients (0.22 +/- 0.07) was significantly lower than that obtained for the milder cases (0.34 +/- 0.06) although the beta/alpha ratios for the parents of the two groups were similar. 3. The heterogeneity within this group of Brazilian patients having two thalassemic genes, i.e. 60% who are beta zero homozygotes and 40% who are carriers of at least one beta + gene, is consistent with the Italian origin of most of these patients.

Integrated excitatory response to microinjections of L-glutamic acid into the reticular system of the toad.

Microinjections of L-glutamic acid into the mesencephalic tegmentum and diencephalon of anesthetized toads evoked an excitatory response consisting of electrocephalographic and electromyographic activation, and a rise in arterial blood pressure. Electromyographic responses were reduced or absent in diencephalically injected toads. Except for this case, all other components of the response always appeared together and presented the same time course. L-glutamic acid was able to activate a group of neurons at different levels of the reticular system involved in an integrated response of ergotropic nature.

The diseases capable of giving rise to congenital or acquired small kidney are examined, together with their pathogenesis and radiological pictures. Various examples are presented of each form.
