Limited diagnostic value of lymphocytic karyotype in primary amenorrhea with streak gonads.

Cytogenetic data represent a first line diagnostic aid in gonadal dysgenesis. Generally, the results of a peripheral blood examination reflect the genotypic alteration of the patient. Nevertheless, on occasion one may encounter cases in which clinical and hormonal evidence suggestive of dysgenesis is not accompanied by an anomalous chromosomal finding, upon cytogenetic analysis of the peripheral blood. In these cases, a cytogenetic alteration may be present in cellular components of the ovary and the cutis. In the light of the above, two patients presenting with primary hypergonadotropic amenorrhea, streak gonads and normal peripheral karyotype are described. In one patient presenting with phenotype alterations, ovarian wedge biopsy via laparotomy followed by cytogenetic analysis of ovarian tissue and tissue from the cutis revealed a 45,X/46,XX-type mosaicism. In the other patient, the ovarian cytogenetic findings were unremarkable. Extending chromosomal analysis to several tissues, beyond the peripheral level, in selected cases, is discussed.

Non-mosaic isodicentric X-chromosome in a patient with secondary amenorrhea.

An isodicentric X-chromosome idic(X) (pter----q26.1::q26.1----pter) was found in lymphocytes and ovarian tissue of a 40-year-old female patient with secondary amenorrhea. No mosaicism was observed. The phenotype-karyotype correlation of our case and of previously described non-mosaic cases of idic(X) (q::q) with different breakpoints is discussed.

Thymopentin (TP-5) therapy during lymphadenopathy syndrome (LAS/ARC): preliminary report.

Persistent generalized lymphadenopathy during HIV infection is now a well classified syndrome featuring multiple chronic lymph node enlargement frequently associated with further symptoms (diarrhea, nocturnal sweating, loss of weight, fever): it is invariably accompanied by serious alterations of the cell-mediated immunity including lymphopenia, reduced number of helper lymphocytes in circulation, inversion of the helper/suppressor ratio, lower proliferative response in vitro and deficient delayed skin sensitivity tests. Minor opportunistic infections are also more frequent, the most widespread being chronic oral candidiasis. Whenever several of these signs are associated in a single patient, especially if the immunitary deficit is severe, an immunomodulating treatment is indicated to improve the lymphocyte functionality and finally to modify any evolutive tendency. The Authors give the preliminary results of a pilot study carried out on 12 patients with LAS/ARC treated with Thymopentin at a dosage of 50 mg by intra muscular injection on alternate days for cycles of 30 days. Compared with 14 untreated patients, the subjects receiving therapy showed a more stable immunological picture, and improvement in subjective symptoms and a better therapeutic response to minor opportunistic infections.

A multicenter double-blind controlled trial comparing lidamidine HCl and loperamide in the symptomatic treatment of acute diarrhoea.

The efficacy of 1-(2,6-dimethylphenyl)-3-methyl-amidinourea hydrochloride (lidamidine HCl, WHR-1142 A) an aryl-substituted amidinourea recently synthesized, was compared with that of loperamide in 32 patients with acute diarrhoea. The results of the study show that lidamidine HCl and loperamide had comparable effects in the pharmacological treatment of acute non-specific diarrhoea. Lidamidine HCl was also shown to be well tolerated; side-effects were generally minor and self-limiting.

[Are HBsAg positive viral hepatitis and its fate caused by the genetically determined immune response to the infection?].

The Authors, after a review of the latest researches about the immunopathogenesis of hepatitis B, propose a new hypothesis that points out the importance of antigen-specific suppressor T-lymphocyte activity for the evolution of hepatitis-B-virus infection. It is possible to verify this working hypothesis by employing a modified migration inhibition factor test, using preparations of isolated T-lymphocytes.

Different mechanisms of "in vitro' lymphocytotoxicity against isolated rabbit hepatocytes during the course of acute viral hepatitis.

In order to characterize the effector mechanism of "in vitro' lymphocytotoxicity against isolated rabbit liver cells in acute viral hepatitis (AVH), the test was performed in 4 patients with type A, in 8 with type, B, and in 2 patients with non-A non-B AVH: (a) using peripheral blood lymphocytes; (b) after enzymatic digestion of the surface membrane immunoglobulins of these lymphocytes, and (c) as in b with addition of patients' sera. 78.5% of the patients had an elevated cytotoxicity which did not change when the lymphocytes were treated as in b and c. Only the 2 patients which progressed to chronicity had an elevated cytotoxicity which disappeared after enzymatic exposure and reappeared with the addition of patients' sera. The results suggest that "in vitro' cytoxicity in recovering AVH is not antibody dependent, but probably of spontaneous type. On the contrary an antibody-dependent cytotoxicity is present in the 2 cases of AVH who progressed to chronicity.

[Prostheses reimplantation with porcine xenograft. Our experience (author's transl)].

The Authors explain the experience about the porcine xenograft, the results of the application of 98 bio-prostheses implanted between January 1976 and November 1979. The distribution of the implantations is: 52 mitral valve reimplantation; 16 aortic valve reimplantation, 2 tricuspid valve reimplantation, 26 xenograft were implanted with the association of another prosthesis or aorto coronary by-pass graft. The patients were selected for clinical criteria (age, sex, geographic provenance) and haemodinamic criteria. Three early embolism were checked, one lethal. The total mortality is of 9 patients: 4 patients were submitted to multiple surgical operation. The running of porcine xenograft is good on the middle term.

[Congenital arteriovenous fistula between the subclavian artery and vein. (Discussion of a surgically treated case) (author's transl)].

The Authors report a case of a congenital arteriovenous fistula between the subclavian artery and vein in a 42 years-old woman. The clinical examinations are discussed for the exact classification and the topographic localisation. The surgical treatment (extirpation of the fistula) is necessary especially because arteriovenous fistulas have a tendency to progress, which may lead to severe cardiac insufficiency, aneurismatic dilatations and risks of endocarditis.

[Chronic persistent HBsAG positive hepatitis in children. I. Subpopulations of T and B lymphocytes].

T and B peripheral blood lymphocytes were studied in five children affected by chronic persistent hepatitis HBsAg-positive. The percentage of E rosette forming cells was found decreased while thymidine uptake after Phytohaemagglutinin stimulation reached values higher than that of normal controls. Null cells were found increased. In all the patients the percentage of IgG bearing lymphocytes was markedly decreased. Such alterations of T cells compartement are similar to those found by other authors in the aggressive form. The hypothesis of a derangement of T cell subpopulation regulatory activity is put forward.

[Chronic persistent HBsAg positive hepatitis in children. II. Effect of treatment with levamisole].

In five children affected by HBsAg positive chronic persistent hepatitis a treatment with Levamisole (LMS) modified several immunological parameters which had been found altered before treatment. In particular the percentage of E rosette forming cells increased while that of EAC decreased; B lymphocytes with SmIgG and the responsiveness to phytohaemagglutinin, which were significantly decreased and increased respectively, got normal values. Since the persistence of HBsAg was unaffected by LMS treatment and a light increase of transaminase serum levels (GPT and GOT) was observed during treatment, doubts are expressed about the opportunity of using LMS in children affected by such a form of hepatitis.

Three years of bacteriological monitoring in the Department of Paediatric Surgery.

The results of three years of bacteriological monitoring in the Department of Paediatric Surgery at the Ospedale Maggiore Ca'Granda in Milan are reported. The tests carried out involved the patients (faeces, urine, and pus cultures), the environment (operating theatre, incubators), and healthy carriers (upper respiratory tract of nursing and paramedical staff). The aetiopathogenesis of the post-operative infectious complications that occurred in 1974-1975 was investigated with the aid of surveys of the causative pathogens, the type of the surgical interventions, the atmospheric pollution in the operating theatre (mean indicative values), the number of operations per month, and the frequency of positive findings among the carriers. As a parallel investigation, between 1971 and the time of writing of this report the authors monitored the resistance to gentamicin of bacteria belonging to the family Pseudomonadaceae isolated from patients, in view of the constant threat posed by the vicinity of the burns department.
