Aerodynamic size distribution, hygroscopicity and deposition estimation of beclomethasone dipropionate aerosol.

Aerodynamic size distribution and aerodynamic mass per dose of beclomethasone dipropionate aerosol were measured at 24 and 95% relative humidity. At high humidity, the count median aerodynamic diameter was unchanged, mass median aerodynamic diameter increased from 2.01 micrometer to 2.68 micrometer, particle number/dose from 41.3 x 106 to 78.3 x 106, and aerodynamic mass per dose from 23.7 to 60.0 microgram. The quantity of active ingredient estimated to be in the 23.7 microgram aerodynamic mass at low humidity was 19.7 microgram. From data previously available describing average deposition fraction as a function of aerodynamic diameter, 6.7 microgram or 13% of the total dose of 50 microgram produced by the metered dose canister would be expected to deposit in the lower respiratory tract.

Thrombotic thrombocytopenic purpura in carbon monoxide poisoning. Report of a case.

A young, previously healthy man was arrested for bizarre behavior outside his poorly ventilated hotel room. He was found to have carbon monoxide poisoning and thrombotic thrombocytopenic purpura complicated by respiratory dysfuntion. The occurrence of these previously unrelated diseases in the same patient may be more than coincidence.

Method for sampling airway gas.

We have devised an improved technique for sampling gas from endotracheal airways for mass spectrometer analysis. It is reliable and unobtrusive and is particularly resistant to occlusion of the sampling line by secrections. It has performed well during continuous monitoring of patients in a respiratory ICU.

Fatal measles (rubeola) pneumonia in adults.

Fatal measles (rubeola) pneumonia, confirmed by viral culture and electron microscopy, occurred in a previously healthy 19-year-old man. Autopsy disclosed measles pneumonia but no recognizable disease that would predispose to such an infection. However, the prolonged course of the measles infection and very low levels of serum antibodies to measles indicate that an immune deficient state existed. Nearly all other adults dying of measles pneumonia have had impairment of the immune system, typically due to a lymphatic or hematologic malignant neoplasm treated with chemotherapy. An immunodeficient state may be a precondition for death from measles pneumonia in adults. Enlargement of air spaces with fibrosis in the anterior portions of both lungs is suspected to be due to tissue necrosis and high mechanical ventilatory pressures.

Treatment of allergic aspergillosis with triamcinolone acetonide aerosol.

The successful management of a patient with allergic asperillosis with triamcinolone acetonide for one year is reported. This patient had been maintained previously on systemic attempt to withdraw systemic corticosteroids continuously for eight years. An initial attempt to withdraw systemic steroids after initiation of aerosol therapy was unsuccessful; however, following bronchoscopic removal of mucus plugs, the transfer to maintenance therapy with triamcinolone aerosol alone was uneventful.

Aerodynamic sizing of tobacco smoke particulate from commercial cigarettes.

Aerodynamic sizing of smoke particulate was performed on a variety of commercial cigarettes using a single particle aerodynamic relaxation time (SPART) analyzer. Cigarettes tested were rated as "medium," "low," or "ultra-low" tar by the Federal Trade Commission method. Aerodynamic size determinations were made with filters attached and with filters removed. Smoke particles from all cigarettes were less than 0.6 micron mass median aerodynamic diameter, and particle size was not affected by filters. Commercial filters reduced particle number concentration by 20-96%. Particle number concentration of smoke particles from some "low" tar cigarettes was found to be similar to some "medium" tar cigarettes. Particle number per puff increased as the cigarette shortened. These findings suggest that estimation of tar delivery to smokers from cigarettes should include puff number information. Also, the ultimate respiratory tract deposition site of smoke particles is not affected by filters since particle size was not altered by passing smoke through cigarette filters.

Regulation of the beta 2-adrenergic receptor and its mRNA in the rat lung by dexamethasone.

Glucocorticoids increase beta 2-adrenergic responsiveness and receptor density in the lung, but the underlying mechanisms have not been clearly elucidated. To determine whether changes in beta 2-adrenergic receptor gene expression are involved in vivo, we measured beta 2-adrenergic receptor mRNA levels and beta 2-adrenergic receptor density in lungs from Sprague-Dawley rats treated with a daily injection of dexamethasone (1 mg/kg subcutaneously) for 1, 3, or 7 days. Animals were sacrificed either 2 or 24 h after receiving the last injection. beta 2-Adrenergic receptor mRNA levels were significantly (p < .05) elevated compared to saline-treated controls in the lungs of animals sacrificed 2 h after dexamethasone injection for 1 day (174 +/- 12%), 3 days (236 +/- 18%), and 7 days (220 +/- 11%). Receptor mRNA levels measured 24 h after dexamethasone injection did not differ significantly from the control group. Induction of beta 2-adrenergic receptor mRNA by dexamethasone was transient, since no significant cumulative or sustained increase in receptor mRNA levels was observed during the study period. Treatment with dexamethasone increased beta 2-adrenergic receptor density as expected, but no significant increase in receptor density was detected until 24 h after the third daily injection of dexamethasone, when levels reached 2045 +/- 150 fmol/mg protein compared to 1292 +/- 34 fmol/mg protein in the control group. Receptor density then remained at this elevated level through 7 days of treatment. These results show that dexamethasone up-regulates both the beta 2-adrenergic receptor and its mRNA in vivo in the lung. The induction of beta 2-adrenergic receptor mRNA levels indicates that glucocorticoids may regulate receptor density in the lung through modulation of gene expression. However, the difference between the time course of induction for the beta 2-adrenergic receptor and its mRNA suggests that additional translational or post-translational mechanisms may also be involved.
