Corneal ulcers in Nepal.

In Kathmandu, Nepal, 133 cases of corneal ulcers were examined and scraped by the investigators during 1981. Corneal ulcers were seen to be more common in females during the active years of life, occurring more frequently during monsoon and autumn seasons. Trauma by vegetative matter was the common antecedent. Organisms were grown in about 50.0% of the cases. Bacteria were seen to be responsible in about 75.0%, and fungi in about 25.0% of growth-positive cases. Sensitivity tests in vitro showed that carbenicillin, chloramphenicol, cephaloridine, methicillin, and lincomycin were most effective against bacteria and clotrimazole was most effective against fungi.

[Ophthalmologic findings and problems in West Nepal].

On the basis of a screening of 6226 patients in western Nepal the most common causes of disease were established. Refraction anomalies and cataract were both found in over 20 percent of the patients, conjunctivitis/dacryocystitis and corneal opacities each in more than 10 percent. Lid conditions, trachoma, pterygium, vitamin A deficiency, glaucoma, iritis, diseases of the fundus and phtisis bulbi were each seen in more than 1 percent of the patients. Problems of treatment are described.

Scintiphotographic studies for long-term results in carotid cavernous sinus fistulae.

Scintiphotographic studies as a method for analyzing the brain perfusion in carotid cavernous sinus fistulae before and after surgical treatment with different techniques are presented. Twelve patients have been reexamined long after their operation to prove the sufficient crossover circulation through the circle of Willis while the homolateral carotid artery is impatent. Sufficient perfusion of both hemispheres could be well-documented by this radionuclide method. No neurological deficit was found in clinical examination either.

Ocular findings in leprous patients. A report of a survey in Malunga/Nepal.

A total of 116 patients in Nepal with advanced leprosy were examined in an ophthalmological survey. All patients were older than 20 years and none had suffered from leprosy for less than 10 years. Abnormal ocular findings occurred in 69.8% of all patients and 9.3% of the eyes were blind. The results are compared with those of other authors.

[Ocular findings in leprosy: results of a survey in the Green Pastures Leprosy Hospital, Nepal (author's transl)].

One hundred and seventy leprous patients underwent ophthalmic examination; of those with tuberculoid leprosy, 49.3% had ocular pathology; in patients with lepromatous leprosy the corresponding figure was 61.4%. 4.7 percent of all eyes examined were blind. Early chemotherapy prevents further complications in tuberculoid leprosy, but not in ;the lepromatous type. The results are compared with those of other authors.

Ocular findings in leprosy patients in an institution in Nepal (Khokana).

A total of 466 leprosy patients in Nepal, some advanced cases, were surveyed for ocular lesions. 74.2% were found with ocular features, and 12.7% of the eyes were blind. The patients were classified in tuberculoid, borderline-borderline, and lepromatous groups. Lepromatous leprosy is responsible for major ocular complications and blindness.

Influence of untreated chronic plastic iridocyclitis on intraocular pressure in leprous patients.

The intraocular pressures of a total of 1015 eyes of leprous patients who never had ophthalmological care or local eye treatment were measured. The patients were categorised according to the type of leprosy they had the eyes were categorised as without or with chronic plastic iridocyclitis. In patients with the tuberculoid and lepromatous types of leprosy the intraocular pressure was significantly lower in eyes with chronic plastic iridocyclitis than in unaffected eyes. It has been shown that chronic plastic iridocyclitis which remains untreated for many years results in significantly lower intraocular pressure.

Brunner's gland adenomas associated with high-output congestive heart failure.

Brunner's gland adenoma in the third portion of the duodenum is rare and only two such cases have been reported previously. A 35-year old man presented with high-output congestive heart failure. Profound iron deficiency anemia was corrected by transfusion, allowing detection of a duodenal tumor, which proved pathologically to be a Brunner's gland adenoma.

Interleukin-1 alpha and interleukin-1 beta in preterm and term human parturition.

Interleukin-1 (IL-1) has been implicated in the mechanism of human parturition in the setting of infection. The purpose of this study was to determine the effect of labor (term and preterm) and microbial invasion of the amniotic cavity on amniotic fluid (AF) concentrations IL-1 alpha and IL-1 beta. AF was retrieved by transabdominal amniocentesis from the following groups of women: midtrimester genetic amniocentesis (16 to 18 wk) (N = 15), preterm labor with intact membranes (21 to 36 wk) with or without infection (N = 72), preterm premature rupture of membranes (PROM) (N = 88), and term not in labor or in active labor with or without infection (N = 58). AF was cultured for aerobic and anaerobic bacteria as well as Mycoplasmas. IL-1 was measured with a commercially available immunoassay validated for AF (sensitivity: IL-1 alpha, 157 pg/ml; IL-1 beta, 50 pg/ml). All women at midtrimester had undetectable AF IL-1 alpha and IL-1 beta. Among women in preterm labor with positive AF cultures, IL-1 alpha and IL-1 beta were detectable in the AF in 86.6% (13/15) and 100% (15/15), respectively. In contrast, all women with negative AF cultures without labor (N = 36) had undetectable AF IL-1 alpha concentrations and 52.7% (19/36) had undetectable AF IL-1 beta concentrations. Histopathological chorioamnionitis was present in 92.8% (13/14) of patients who had positive AF cultures and detectable IL-1 in the AF. IL-1 was significantly higher in patients with preterm PROM, labor, and positive AF cultures than in the other subgroups of patients with preterm PROM.(ABSTRACT TRUNCATED AT 250 WORDS)
