[The hemodiafiltration with infusion of acetate-free dialysis fluid can modify the inflammatory response in patients "high responders" to inflammatory stimuli?].

PURPOSE: This study aimed to verify the effects of paired hemodiafiltration on-line (PHF-AF) on inflammation in patients who were "high responders" to inflammatory stimuli: elevated C-reactive protein (CRP), genetic polymorphisms influencing a low transcription for interleukin-10 (IL-10) and a high transcription for IFN-gamma. METHODS: Ten patients selected as high responders for IFN-gamma and low responders for IL-10 were included in a crossover study to compare PHF-AF and standard bicarbonate hemodialysis (BHD). At study entry and before the start of each dialysis session the following examinations were performed: CRP, albumin, fibrinogen, ferritin, transferrin, prealbumin and serum levels of IL-6, IL-10, IFN-gamma, tumor necrosis factor-alpha (TNF-alpha). After the 1st and 3rd week of the study, the blood samples were also collected after the dialysis session. RESULTS: . There was a significant reduction in albumin and prealbumin in PHF-AF patients during the study; none of the other parameters were changed in both patient groups. CRP tended to be elevated after dialysis in both PHF-AF and BHD. While IL-6, IL-10 and IFN-gamma were unchanged during the dialysis session, there was a significant variation in TNF-alpha levels, which were increased in BHD (from 10.9 +/- 3.1 to 14.7 +/- 4.1 pg/mL; p=0.004) and reduced in PHF-AF (from 11.9 +/-2.8 to 6.3 +/- 2.2 pg/mL; p=0.0004). CONCLUSION: Although the cytokine levels were unchanged during the study in both BHD and PHF-AF, the modification of TNF-alpha during the dialysis session was considered as inflammatory significance.

[Clinical study of two families with late-onset autosomal dominant spinal-cerebellar ataxia linked with HLA. Preliminary results].

Two families with autosomal dominant spinocerebellar ataxia (SCA) of late onset were studied. These families originate in the same small rural area in a Southern Italian region (Calabria). We report the clinical study of 23 patients in different stages of the disease and neuropathological study in one patient. Linkage studies provided strong evidence for linkage of the SCA locus to the HLA loci (SCA1) in the subjects of these families. Our study allows to outline the clinical features of HLA linked SCA in order to trace a pattern of SCA1 phenotype thus making easier the identification of SCA1 heterozygotes in an early clinical stage.

[Leukoencephalopathy in the elderly. Binswanger disease?].

100 patients with cerebro-vascular disease, characterized by an acute episode, were explored by TC to examine the etiopathogenesis of cerebral damage of the periventricular white substance, correlated with Binswanger disease. Only 7 patients showed typical bilateral lesions of the periventricular white substance. All these patients, except for one, were over seventy. The same percentage was verified in patients more than 70 years old without cerebral vascular disease. In our series the only patient with mental impairment showed very serious lesions of the periventricular white substance.

The role of spinal cord MRI in the diagnosis of multiple sclerosis.

In this study we evaluated the effectiveness of high field thin slice surface coil MRI in demonstrating MS plaques in the cervical and upper thoracic spinal cord. MRI seems to be in our experience a sensitive indicator of spinal cord involvement in patients suffering from MS.

[Computerized tomography in the evaluation of neurologic complications of preventive therapy of acute lymphatic leukemia].

The authors examined by means of computed tomography 12 patients with ALL treated with radiotherapy combined with cytostatic chemotherapy and presenting clinical signs of CNS damage. The CT scan findings are correlated with clinical signs, mainly distinguishing between symptoms of either diffuse or focal damage. An attempt is made in order to evaluate the possible role of Methotrexate and X-Ray therapy in the pathogenesis of the CNS lesions.

[A case of giant hydronephrosis in a child, secondary to congenital stenosis of the pyeloureteral junction].

An examination of some anatomopathological and clinical aspects of hydronephrosis and its causes in childhood is followed by the description of a personal case marked by discrepancy between the anatomical and pathological picture of giant hydronephrosis and the complete absence of symptoms, resulting in delay in its clinical detection. Stress is laid on the fact that, although surgical reconstruction of the pyeloureteral junction is possible, a very great number of nephrectomies still have to be performed owing to the tardy discovery of these lesions.
