[Malignant pheochromocytoma].

The reported incidence of malignant pheochromocytoma varies from series to series. In this series 4 cases (7.2 p. 100) were observed out of a total of 55. In two cases the tumour progressed rapidly but in the other two cases, metastases were detected 3 to 12 years after the apparent cure of a histologically benign pheochromocytoma. The urinary levels of catecholamines and their metabolites gave no indication of the underlying malignancy. The diagnosis was only made from the clinical and radiological detection of metastases (2 hepatic, 2 bone). There is no satisfactory treatment and various therapeutic methods have to be used in succession; surgery for a single metastasis, radiotherapy and antiadrenergic agents to combat clinical manifestations. The natural history of this tumour is relatively long.

Thyroid function and blood pressure in two new strains of spontaneously hypertensive and normotensive rats.

1. The influence of thryoid function on the development of hypertension was studied in strains of spontaneously hypertensive (SH) and normotensive rats. 2. Surgical thyroidectomy decreased systolic blood pressure more markedly in SH rats than in normotensive rats. The effects of oral administration of 5 and 100 micrograms of thyroxine 24 h-1 100 g-1 were studied in the thyroidectomized animals. In the two strains the blood pressure returned to control levels only after administration of the larger dose. 3. The evolution of body weight, total plasma tri-iodothyronine (T3) and tetraiodothyronine (T4) concentrations were followed as a function of age in SH rats and normotensive rats from 5 to 21 weeks. At each age, SH rats showed significantly larger body weight and decreased T4 concentrations. Plasma T3 in SH rats was lower than in normotensive rats until 15 weeks of age, after which the difference was not significant. At 11 weeks, plasms free T3 and T4 concentrations were slightly lower in SH rats than in normotensive rats. 4. The more marked hypotensive effects of surgical thyroidectomy in SH rats cannot be related to increased thyroid function.

[Hyponatremia, a frequent, and at times a major and indicative sign in anterior pituitary insufficiency].

Thirty five patients with hypopituitarism have been studied. Nine had a mild hyponatremia (126-137 mEq/l) and five a profound hyponatremia (105-124 mEq/l). None of them had hemodilution (plasma protein, potassium were normal). Hyponatremia was present even in the absence of deficit in GH and/or TSH, and/or FSH-LH. All the pateints had a complete ACTH deficiency. There was a negative correlation between age and natremia (r = -0,39; p0.02). 2/5 patients with severe hyponatremia had mental confusion. These two patients had a complete TSH deficiency. Natremia increased with cortisone therapy in all cases but reached the normal range only in one case. Mental confusion disappeared and natremia became normal in all patients with therapy with cortisone and thyroid hormones.

Ophthalmic Graves' disease.

Ophthalmic Graves' disease is a peculiar clinical condition, characterized by ocular symptoms, without thyrotoxicosis. A survey of 22 observed cases is discussed. This condition is 25 times less frequent than classical Graves' disease. The eye symptoms include upper lid retraction, exophthalmos, and severe ophthalmopathy. Unilateral symptoms are frequent (16 out of 22). A negative T3 suppression test was found in half of the patients. LATS was detectable in 3 out of 17 of the cases. The stimulation of TSH secretion by TRH was normal in 3 out of 6 of the cases. When the tests were combined and repeated, a positive response was found in 8 out of 9 of the exophthalmic patients, and in 3 out of 9 of the patients with lid retraction. Appreciation of the course of the disease necessitates a prolonged follow-up; transformation into classical thyrotoxicosis have been observed. Our clinical studies bring into focus the relationship of ocular symptoms with Graves' disease and Hashimoto's disease.
