[Small bowel obstruction and laparoscopic treatment in children].

From 1992 to 1996, 15 children with an acute small bowel obstruction were treated by laparoscopy. There were 10 boys and 5 girls between 6 to 15 years. They had been formerly operated in the neonatal period (4 cases), for appendectomy (9 cases), or ureteronephrectomy for Wilm's tumor (1 case), and caudal pancreatectomy after a traumatism (1 case). Thirteen times it was the first obstruction and twice the second or more (but never treated by surgery). Conversion in laparotomy was necessary in 4 cases. There were no operative complication. We noted a quick recovery (12 hours), an early discharge (3.5 days), and a good postoperative comfort. Laparoscopic viscerolysis for acute small bowel obstructions seems to be a good treatment in children.

Auxiliary liver transplantation for fulminant and subfulminant hepatic failure.

We report the first series of 9 auxiliary liver transplantations performed as a bridge to recovery in 8 patients with fulminant and subfulminant hepatic failure. Hepatic failure was due to hepatitis A virus (n = 3), hepatitis B virus (n = 1), hepatotoxic drugs (n = 2), autoimmune disease (n = 1), or it was of unknown origin (n = 1). The donor liver was reduced to a left lobe (n = 2), a left liver (n = 4), or a right liver (n = 3), and was implanted in an orthotopic position beside the native liver after it was resected by a left or a right hepatectomy. Conventional immunosuppression was used to prevent rejection. Six patients regained normal consciousness within 2 weeks, without any sequelae. Two patients had persisting encephalopathy due to graft initial dysfunction, one of whom showed portal vein thrombosis, which was successfully cleared. The other one showed hepatic vein stenosis and was retransplanted at day 15. Five of eight patients had to be reoperated because of a surgical complication. Five patients showed rapid regeneration of their native liver, but one died at day 45 from severe herpes virus broncholitis. The auxiliary grafts were removed (n = 3) or left to atrophy by tapering immunosuppression (n = 1). One patient developed cirrhosis of the native liver and died of infectious complications at day 42. The native livers of the two remaining patients are still atrophic, one at 4 months and one at 1 month posttransplant. Finally, 6 of 8 patients are alive with a follow-up of 1 to 17 months. Four of them have permanently stopped their immunosuppressive therapy. Our experience demonstrates that auxiliary orthotopic liver transplantation (1) is feasible in children and adults, using either a left or a right liver graft, (2) is efficient in providing adequate liver function, and (3) gives a real chance to the native liver to regenerate, offering these patients a future free of immunosuppression.

Spontaneous regression of fetal pulmonary sequestration.

The prenatal diagnosis of pulmonary sequestration can usually be made by the third trimester of pregnancy, from the combination of an intrathoracic mass and indirect signs such as cardiac deviation, fetal hydrops, pleural effusion and polyhydramnios. We describe four cases in which pulmonary hyperechogenicity was detected before 26 weeks' gestation. In three cases the hyperechogenic mass was isolated. In all cases it had mostly regressed during the pregnancy. A review of the cases of isolated pulmonary sequestration that have been diagnosed during the antenatal period is presented. Antenatal evolution was found to be unpredictable regardless of the type or severity of the case at the first diagnosis. We propose a classification to define more clearly the optimal management of pulmonary sequestration.

Accessory diaphragm--review of 31 cases in the literature.

The authors report a case history of a male newborn at term, presenting with an accessory diaphragm in the right hemithorax, and an associated lung hypoplasia, revealed by a transient respiratory distress. The review of 31 cases published in the literature confirm the rare occurrence of this malformation. The diagnostic difficulties are discussed. The advantages of both MRI and CT scanning are compared. Pulmonary, vascular, or diaphragmatic abnormalities were due either to the isolated diagnosis or to its possible associated malformations. Operative management is often considered in view of the progressive respiratory complications. Excision of the accessory diaphragm does not necessarily solve the problem of a coexisting lung hypoplasia.

Mediastinal stabilization by an expansion prosthesis in postoperative congenital diaphragmatic hernia with severe pulmonary hypoplasia.

The authors illustrate a case of right neonatal congenital diaphragmatic hernia (CDH) of Bochdalek with major pulmonary hypoplasia and postoperative massive mediastinal displacement to the right, leading to tracheal compression and recurrent respiratory distress at every attempt to wean the infant from the ventilator. Mediastinal stabilization was obtained by placing an expansion prosthesis (of the cutaneous type) in the right hemithorax, to prevent mediastinal obstruction of the main airways. This procedure may prove to be an original and efficient solution for the critical postoperative respiratory obstruction due to severe pulmonary hypoplasia and mediastinal mass effect in the newborn with CDH. However, this may not prevent progressive scoliosis, a serious problem frequently seen in the absence of one lung.

[Gastroschisis. Management. 50 cases].

OBJECTIVE: Demonstrate the need for a multidisciplinary antenatal approach to laparoschisis. MATERIAL AND METHODS: A retrospective series of 50 children who underwent surgery for laparoschisis between 1975 and 1994 in the Infant Surgery Department of the Strasbourg University Hospitals. Postoperative outcomes were evaluated as a function of the stage of laparoschisis and surgical technique. Follow-up was also examined. RESULTS: Depending on the periods evaluated, ultrasound diagnosis varied from 16 to 92% and was made at 21 weeks gestation on the average. Lesions observed were atresia of the bowel (10%), complete intestinal involution (2%), and perivisceritis (88%). Complete parietal closure was achieved after bowel emptying in 58% of the newborns. Postoperative mortality, usually in vere severe forms, fell from 46% to 15% over the 20-years study period. CONCLUSION: Antenatal echography can confirm the diagnosis of laparoschisis. It can be used to define risk factors (size, bowel dilatation(s), mesenteric blood flow) allowing planned extraction in an Infantile Surgery Unit. This multidisciplinary attitude should help to improve overall outcome.

[True benign non-parasitic cysts of the spleen: from diagnosis to treatment. Apropos of 5 new cases].

True non parasitary splenic cysts are uncommon. They are characterized histological by an epithelium on the inner surface of the cyst wall. This study presents, about 5 new cases of splenic cysts, the clinical presentation, the difficulties of the diagnosis based on the ultrasound exploration and computed tomography scan investigation, and the different possibilities of management of these cysts. The principal of the treatment is the resection of the totality of a cyst with a diameter higher than 5 cm. A partial splenectomy seems to be the best treatment, whenever possible, especially in children.

Antenatal diagnosis and treatment of fetal hydrops secondary to pulmonary extralobar sequestration.

Bronchopulmonary sequestration with associated non-immune hydrops has been previously reported with generally poor prognosis for the neonate. The vast majority of sequestrations are discovered postnatally, and less than half are correctly identified as sequestration before birth.Until recently, treatment for pulmonary sequestration was limited to postnatal respiratory support and resection of the sequestered lung. But aggressive respiratory support does not succeed in salvaging infants with underlying pulmonary hypoplasia. In our report, we describe the antenatal treatment of pleural effusions complicated by hydrops fetalis secondary to a bronchopulmonary sequestration, successfully managed with a transthoracic catheter placement. This therapy resulted in the resolution of pleural effusion, polyhydramnios and hydrops, with a good fetal outcome.

[Video-assisted thoracic surgery of hydatid cysts of the lung in children].

10 children between 3 and 9 years old, have been operated on for an echinococcosis cyst of the lung, by a thoracoscopic procedure. It was a vomited cyst in 6 cases, a pyopneumocyst in 1 case, and a jung noncomplicated and univesicular cyst in 3 cases. The surgical procedure is described. We brought into being the operative conditions such it is nowadays an excellent alternative to the classical thoracotomy.

Transverse mucosal preputial flap for repair of severe hypospadias and isolated chordee without hypospadias: a 350-case experience.

Three hundred patients with hypospadias and 50 patients with isolated chordee without hypospadias were operated on and followed by the same surgeon. A very low postoperative urethrocutaneous fistula rate (4.8%) was related to complete cutaneous coverage of the neourethra. The incidence of stenosis, noted in 8% of the cases, has progressively decreased with improvement of technical details and with growing experience. A total of 75% of the patients were corrected with a single operative procedure. Good disponibility of penile skin rendered a second intervention always easy.

Post-traumatic pancreatic pseudocyst: non-operative conservative management--report on 3 cases.

The authors report on 3 cases of post-traumatic pancreatic pseudocysts in children. Complete healing occurred with non-operative conservative treatment and total parenteral nutrition. Daily follow-up with clinical evaluation, abdominal ultrasound and lab exams are detailed under definite criteria of selection and follow-up. This mode of treatment might be considered as an alternative to exploratory laparotomy and external drainage.

Qualitative assessment of thermal and evaporative adjustments of human scrotal skin in response to heat stress.

To study scrotal thermoregulation and its efficacy to work against heat accumulation, five subjects were exposed to four experimental conditions under which core and skin temperatures and sweat evaporative responses of various skin surfaces--chest, abdomen and scrotum--were compared. The temperature response of the scrotal area exhibited the largest inertia, and this observation is likely to be the consequence of heat exchange via the vascularization of testes and scrotum which is more efficient than in other parts of the body in limiting local heat storage, thus alleviating heat stress of the testis. The pulsatile nature and the synchronous pattern of the scrotal evaporative heat loss indicate that scrotal sweating takes place, although the gradient response appeared to be less marked than elsewhere in the body. Relatively low and inert scrotal temperature can partly explain this poor local drive for sweating.

[Cantrell syndrome or the "shagreen skin" pentalogy. Apropos of 3 cases].

Three patients with incomplete forms of Cantrell syndrome underwent surgical therapy at the Department of Infantile Surgery in Strasbourg, France, between 1979 and 1990. One patient had a giant omphalocele. Another had a ventricular septal defect. Two of the three patients are alive and doing well. The nosology and prognostic factors in Cantrell syndrome are discussed.

[A retrospective study of 54 children hospitalized in the Department of Pediatric Surgery for suspicion of child abuse].

Forty-five children admitted to the department of pediatric surgery at Strasbourg (France) from 1980 through 1989 were reported as being possible victims of abuse. The conditions of the hospital admission and nature of the injuries found are analyzed. On the basis of the data in the reports established by social workers and judicial authorities and in psychiatric expert evaluations, the social and familial characteristics of child abuse are defined. Although based on the demonstration of many elements pointing in the same direction, the diagnosis of child abuse remains fraught with difficulty.

Mesoblastic nephroma presenting as a haemorrhagic cyst.

Two cases of haemorrhagic renal cyst presenting in the neonatal period are described. Ultrasound examination, excretory urogram, and MRI, gave diagnosis in the second case. The remarkable histopathologic pattern of these two cases has led to propose correct diagnosis of pure cystic mesoblastic nephroma, an unusual variety of mesoblastic nephroma.
