Non-Hodgkin's lymphoma and Sjögren's syndrome. An immunopathological study of 113 patients.

One hundred and thirteen patients with untreated non-Hodgkin's lymphoma were evaluated for Sjögren's syndrome (SS) using clinical and biological parameters, and a minor salivary gland biopsy. Concomitant immunophenotyping for T and B cells, and kappa and lambda chains was performed on both minor salivary gland and lymphomatous tissues. Patients with a positive focus score on examination of their minor salivary gland biopsy had a second biopsy, when possible, at the end of induction therapy. Fourteen of the 113 patients had SS according to the Greek criteria, and 4 of these 14 patients had an identical monotypic infiltrate in both their minor salivary gland and lymphomatous tissues. In all four cases this infiltrate disappeared from both locations when the lymphoma was in complete remission (CR). The 10 other patients with criteria for SS had a mixed infiltrate which persisted unchanged when the lymphoma was in CR. Among the 99 patients without SS, 12 had a positive focus score and an identical monotypic infiltrate in both their minor salivary gland and lymphomatous tissue. This infiltrate disappeared from both locations when the lymphoma was in CR. Among the remaining 87 patients, 82 had no minor salivary gland infiltrate, and 5 had a positive focus score with mixed lymphocytic infiltrate. Long term follow up and a large cooperative study are needed to better understand the immunopathologic lesions of patients of this latter type.

[Acute agranulocytosis induced by pyramidon or phenothiazines. Apropos of 31 cases].

The authors present 31 cases of acute agranulocytosis due to drugs. 27 resulted from amidopyrine poisoning and 4 were due to phenothiazines. The clinical picture in most cases, was that of severe septicemia with lesions of the mucosae. On blood count agranulocytosis was complete, associated on bone marrow examination with an appearance of selective hypoplasia of the granular series or an aspect of maturation blockage at the stage of the myelocyte or the metamyelocyte. The course was fatal in 6 cases. A search for leukocyte agglutinating antibodies, carried out in 29 cases, was positive in only 5 cases. These facts correspond, on the whole, to the, clinical and etiological data found in the literature. Although the agranulocytoses induced by phenothiazines, seem to be mainly of toxic origin, the agranulocytoses induced by amidopyrine associated both an immunological mechanism and a toxic mechanism. Among all the biological tests proposed, the most useful is the le ukocyte agglutination test, but the serological diagnosis is rarely conclusive.

[Therapy of bacterial infections occuring during aplastic phasis of acute leukemias (author's transl)].

On a three years' period, 122 patients with AL (45 children, 76 adults) received 158 treatments of induction, involving a severe medullary aplasia. In 48 cases (30.4%) none infectious complication was recorded. In the other 110 observations, infections syndrome (septicemia due to gram-negative rods above all) was treated by an early and intensive combination antibiotherapy (three successive protocols are employed, after failure of the precedent), in association with a careful medical reanimation and trasfusions of granulocytes in 16 cases. Regression was obtained in 94 cases (59,5%) but infection was lethal in 16 patients (10.1%). More than cytological variety of AL, principal pronostic element is age (15 deaths among 88 infections episodes in adults, one among 22 in children). Granulocytes rate at the time of aplasia (lower than 500 /mm3) also represents a factor of gravity. Among 94 infections episodes, antibiotherapy was successful alone in 81 cases (86.2%), illustrating the fundamental place of this treatment.

[Hypercalcemia and osteoclast activating factor (OAF) associated with acute lymphoblastic leukemia (author's transl)].

The occurence of hypercalcemia appears to be unusual in leukemia. The mechanisms are numerous: secretion of PTH of PTH like substance, prostaglandin E2 or vitamin D like sterols. The reported case is one of lymphoblastic acute leukemia and hypercalcemia associated with osteoclast activating factor secreted by leukemic cells. In this patient the serum levels calcium closely paralleled the course of acute leukemia.
