[Clinical and immunological studies in patients with prostatic tumors].

The authors examined the cellular and humoral immune reactivity in patients with carcinomas of the prostate gland and determined several glycoproteins. The blastic transformation of lymphocytes was found significantly decreased. However, the concentrations of the immune globulins were not changed. Significantly changes were the glycoproteins belonging to the group of the "acute phase reactant proteins" and the complement titre in the serum. On account of the decreased immune reactivity the authors propose an immune therapy in the carcinoma of the prostate gland.

Diagnostic ultrasound in urology.

Diagnostic ultrasound has been used over the last eight years for the demonstration of various space-occupying lesions of the urogenital tract, for the estimation of the prostate size and for the identification of intrascrotal processes and tumours. 918 patients were studied by this method. In 595 (87.3 per cent) of the 682 surgical cases in this series ultrasonic diagnosis was confirmed by the surgical finding. False-positive and false-negative results totalled 17.7 per cent. The method is particularly suited for the differential diagnosis of renal cysts from tumours without the need for other procedures involving discomfort to the patients. This procedure, which is free from any iatrogenic hazard, provides accurate information on the size and structure of space-occupying lesions.

[The value of rectal examination in the diagnosis of prostatic cancer (author's transl)].

Rectal examination is the basic step in the diagnosis of carcinoma of the prostate. However its accuracy is limited because cancerous nodules are often difficult to differentiate from certain benign conditions which cause areas of induration. Needle biopsy is necessary for establishing the diagnosis of cancer. 699 men with palpable lesions of the prostate underwent transperineal biopsy. Out of 251 patients with advanced lesions only 171 (68.1%) had a histologically proved carcinoma of the prostate. Out of 119 men with an isolate firm nodule 50 (42%) had a positive biopsy whereas in only 62 (18.8%) of 267 biopsies of glands with changes in consistency and indurations histological examination confirmed the diagnosis of prostatic carcinoma.

[Experiences with ultrasound in the diagnosis of acoustically homogeneous resistances].

For the differentiation of acoustically homogeneous resistances the ultrasound method was used in 562 cases. In the present paper is reported on the possibilities in the differential diagnostics of renal cyst and cyst-like formations given by the echo method. The new diagnostic method gives an essential help for making the exact diagnosis, taking simultaneously into consideration the findings of the traditional examination methods.

[Frequency of asymptomatic significant bacteriuria in bile duct and colon inflammations].

When investigating the asymptomatic significant bacteriuria in cholepathias and disease of the colon was stated that the asymptomatic significant bacteriuria is a frequent concomitant appearance of acute cholecystitis, of chronic cholangitis and ulcerous colitis. The asymptomatic significant bacteriuria does not mean pyelonephritis; but is a sign for the fact that the kidneys are in a condition endangered by pyelonephritis (potential pyelonephritis), that's why an aimed antibacterial treatment and subsequently bacteriological controls of the urine prove necessary.

[Primary pyelonephritis (the focal problem)].

On different experimental models of pyelonephritis in dogs and rabbits an experiment for the classification of the pathogenesis of this disease with the special questioning of the possible development of the so-called primary pyelonephritis was performed. A series of experiments consisted of 3 groups: 1. Injection of an E.-coli-suspension into the musculature of the urinary bladder of dogs and rabbits. 2. Injection of an E.-coli-suspension into the testicles of rabbits. 3. Injection of an E.-coli-suspension into the seminal bladders of rats. The experiments lasted 2 to 15 days and 16 to 110 days, respectively. In the second series of experiment in rats and rabbits an E.-coli-suspension was injected into the gall-bladder of rats and rabbits. Bacteriologic blood cultures and histologic investigations of the kidneys, the ureters, the wall of the urinary bladder and the adjacent lymphatic and fatty tissue as well as of the gall-bladders were performed. The results showed that the pyelonephritis developed secondarily after the infection of the other infected organs by haematogenic or interstitial dissemination of the pathogenic agents. Conclusions are drawn: 1. Every pyelonephritis, also the haematogenic, is a secondary process. 2. Therefore all must be done to find out the primary inflammatory change in the organism and to cure it. 3. Severe forms of the pyelonephritis may, on the other hand, effect as focus and evoke in inflammation of other organs on lymphogenic and haematogenic way. 4. Taking into consideration the facts mentioned perhaps the great number of pyelonephritiides having become latent might be reduced.

[Indication for operation in acute kidney obstruction caused by urinary calculi].

We like to discuss the comment of our clinic: In case of an acute stone-conditioned renal blockage complicated by fever we immediately perform a decompression by means of elevation of the ureter catheter and the antibiotic therapy. A shivering attack accompanied by fever we regard as a threatening sign of pyelonephritis apostematosa and possibly after the first or second shivering attack we perform an opening. Apart from emphasizing the importance of the shivering attack we still might refer to the fact that for the establishment of the therapy of choice the complex evaluation of the clinical picture is necessary.

Ultrasonography in the differential diagnosis of hydrocele.

In addition to the conventional examination methods, ultrasonography was applied in the diagnostics of testicular lesions in 18 cases. This harmless method differentiating hydrocele from testicular tumour and inguinal hernia is recommended.

[Problems of infectious hospitalism in the Hungarian urology].

1. In more than half of the urological basis disease an asymptomatic significant bacteruria is present. 2. The asymptomatic bacteruria must be treated, since it is a sign for endangering by pyelonephritis (potential pyelonephritis). 3. In our clinical material the distribution of the pathogenic agents has changed and that in favour of bacteria resistant to medicaments. 4. This appearance could be observed above all in patients who were hospitalised for a longer time and in the older age groups. 5. With all remedies at our disposal must be tried to reduce the resistance.
