[Experiences with Depo-Provera (medroxyprogesterone) in the treatment of kidney tumors].

30 patients with renal tumours were treated with Depo-Provera (Medroxyprogesterone). Among the 21 patients operated on without metastases and with an parallel metastasis (fibrous capsule, adipose capsule, peritoneum) in the course of 6 to 15 months no one died of a tumour progression. Among the 9 patients with a distant metastasis in one case the regression of the metastasis could be achieved, whereas in another case progesterone stopped the progression of the intraoperatively observed liver metastasis.

Resection in renal tumour patients.

Nine cases (two of them bilateral) of renal resections for renal tumour are reviewed. The survival results of patients with bilateral renal tumours are considered particularly favourable, one of them has survived for 72, the other for 37 months. Of the patients with solitary renal tumour, one was lost 25 months postoperatively, after the development of a distant metastasis. The other patient has been free of recurrence and metastases after 31 months. The above results have proved the outdatedness of the old concept, i.e. to remove the tumorous kidney. As supported by the authors' cases, their patients have had a high survival rate after resection for renal tumour. The clear-cell tumours are well demarcated by a fibrous capsule and they can easily be resected. The granular-cell renal tumours are localized more centrally, they often infiltrate the kidney. In cases cautiously considered, the resection of the tumorous kidney can be performed under the present technical conditions by strict 6 monthly postoperative urographic, ultrasound, and angiographic examinations.

[Prevention of pyelonephritis].

The possibilities of the prophylaxis of pyelonephritis in the intrauterine life, in infancy and childhood, in internal medicine, gynaecology and urology are described. After explanation of the causes of pyelonephritis it is referred to the importance of the asymptomatic, significant bacteriuria and it is emphasized that there is no primary pyelonephritis, but every pyelonephritis is secondary.

[Hair loss following chronic hemodialysis].

Attention is drawn to the frequent occurrence of loss of hair in chronically dialysed patients. No examinations were undertaken to find the cause of this phenomenon. However it is possible that, besides insufficiently eliminated toxins, hypothyrosis is also involved.

Ten years of diagnostic ultrasound in renal disease.

Observations with diagnostic ultrasound in renal disease over a 10-year period are reviewed. The advantages of diagnostic methods involving the least possible discomfort to the patient are emphasized. Ultrasonography is regarded as a reliable non-invasive method for the diagnostic differentiation between renal tumours and cysts. It was used by the authors for this purpose in 740 cases. Renal tumours were found by this method in 622, and renal cysts in 118 cases. Of the 740 patients, 726 underwent surgery. The preoperative ultrasonic diagnosis of renal tumour proved to be correct in 91.8 per cent, that of renal cyst in 88.9 per cent of the respective surgical cases. Ultrasonography also lends itself to the follow-up of polycystic disease of the kidney, being suited for its early diagnosis, as well as to its identification in the stage of renal failure when the traditional diagnostic methods are no longer practicable.

[Clinical value of immunologic studies in patients with bladder tumors].

By means of mathematical calculations the authors tried to find correlations between the clinical and histological stages of vesical tumours and the results of immunological tests. Density calculation showed what parameters follow the degrees of malignity. Moreover the significance of the chain and basic proportional factors of the parameters calculated for diagnosis and follow-up is pointed out.

The investigation of cellular and humoral immune reactivity in patients with tumours of the bladder.

The authors investigated the cellular immune reactivity by measuring the lymphocyte blast transformation, immune globulins and some glycoproteins in the sera of patients with papilloma and carcinoma of the bladder. They found a change in the cellular immune reactivity, but none in immune globulin concentration. Among the glycoproteins the acute phase reactant proteins changed significantly.

Ultrasonic measurement of residual urine: a method requiring no catheter.

Residual urine was measured with the two-dimensional and the gray-scale equipments (Kretz-Technik) in 26 cases each. The results were checked by catheterization. The difference between the ultrasonographic and the catheter values was 20 to 40 ml for the two-dimensional, but only 5 to 10 ml for the gray-scale apparatus. Ultrasonography is regarded as a reliable and entirely harmless procedure, suited for the measurement of residual urine.

[Transurethral resection].

In five years 938 transurethral resections were performed in the Department of Urology of Semmelweis University Medical School. The indications were in 483 cases tumours of the bladder, in 179 adenoma of the prostate and in 182 cancer of the prostate; the indications of the remaining cases were miscellaneous. The indications, most important surgical parameters, postoperative complications and treatment of transurethral resections are discussed from the authors' point of view. The results prove the advantages of transurethral resection and confirm its importance as a basic surgical intervention in urological practice.

[Primary pyelonephritis (the focal problem)].

On different experimental models of pyelonephritis in dogs and rabbits an experiment for the classification of the pathogenesis of this disease with the special questioning of the possible development of the so-called primary pyelonephritis was performed. A series of experiments consisted of 3 groups: 1. Injection of an E.-coli-suspension into the musculature of the urinary bladder of dogs and rabbits. 2. Injection of an E.-coli-suspension into the testicles of rabbits. 3. Injection of an E.-coli-suspension into the seminal bladders of rats. The experiments lasted 2 to 15 days and 16 to 110 days, respectively. In the second series of experiment in rats and rabbits an E.-coli-suspension was injected into the gall-bladder of rats and rabbits. Bacteriologic blood cultures and histologic investigations of the kidneys, the ureters, the wall of the urinary bladder and the adjacent lymphatic and fatty tissue as well as of the gall-bladders were performed. The results showed that the pyelonephritis developed secondarily after the infection of the other infected organs by haematogenic or interstitial dissemination of the pathogenic agents. Conclusions are drawn: 1. Every pyelonephritis, also the haematogenic, is a secondary process. 2. Therefore all must be done to find out the primary inflammatory change in the organism and to cure it. 3. Severe forms of the pyelonephritis may, on the other hand, effect as focus and evoke in inflammation of other organs on lymphogenic and haematogenic way. 4. Taking into consideration the facts mentioned perhaps the great number of pyelonephritiides having become latent might be reduced.

[Indication for operation in acute kidney obstruction caused by urinary calculi].

We like to discuss the comment of our clinic: In case of an acute stone-conditioned renal blockage complicated by fever we immediately perform a decompression by means of elevation of the ureter catheter and the antibiotic therapy. A shivering attack accompanied by fever we regard as a threatening sign of pyelonephritis apostematosa and possibly after the first or second shivering attack we perform an opening. Apart from emphasizing the importance of the shivering attack we still might refer to the fact that for the establishment of the therapy of choice the complex evaluation of the clinical picture is necessary.
