Body fluid compartments in hypertension.

BACKGROUND AND OBJECTIVES: There is a correlation between the fluid and ionic homeostasis and blood pressure but it is not known if these body fluid changes represent the cause or rather the effect of the blood pressure rise. We have estimated the compartmental distribution of body fluids by means of the Bioimpedance Spectroscopy (BIS) analysis in a hypertensive cohort compared to control subjects. MATERIAL AND METHODS: We have enrolled 28 hypertensive patients (14 females, 14 males, mean age 47 +/- 5) and a sex- and age-matched control group of 37 healthy subjects (17 females and 20 males, mean age 45 +/- 8). They underwent anthropometric measurements, then extracellular (ECW) and intracellular water (ICW) were assessed using BIS. RESULTS: Both mean weight and BMI of hypertensive patients resulted significantly higher than of the control group (p < 0.05). We found higher ICW values in hypertensive compared to normotensive subjects. This difference was proportional to the difference of mean blood pressure values, reaching significance only as regards the stage II hypertensive subgroup (p < 0.03). DISCUSSION: Our data confirm that the blood pressure increases are associated to TBW, and caused mainly by ICW increases. The BIS, a simple, reliable, non invasive and cost effective methodical approach, estimating the distribution of body fluids, offers new possibility of the management of the hypertensive disease, to establish a more appropriate antihypertensive treatment. Moreover, the BIS, estimating the volume restoration of the different body compartments, may be helpful in evaluating the effectiveness of the pharmacological treatment.

[Cardiovascular and respiratory reflex responses due to stimulation of muscle nerve receptors and experimental physical exercise].

The cardiovascular and respiratory reflex effects brought on by the infection into the femoral artery of bradyquinine and of hypertonic sodium chloride solutions and by rhythmic muscular work and muscular work of tetanic type induced by low and high frequency electrostimulation in the muscles have been investigated in the rear extremity anaesthetized rabbits. Intra-arterial injection of bradyquinine and rhythmic muscular contractions trigger off cardiovascular reflux effects of inhibitory type represented by a reduction in arterial pressure, heart frequency and vascular resistances of the rear extremity controlateral to the one stimulated. Intra-arterial injection of hypertonic NaCl and tetanic contractions cause opposite effects, namely of decidedly excitatory type. Such responses are not substantially altered by complete removal of the skin layer of the stimulated extremity. The neuromuscular block obtained with curare-like substances does not affect chemical stimulation responses, whereas it eliminates similar effects caused by experimental muscular work. The experiments carried out show that injection into the muscular arteries of chemical agents of various type produces varying cardiovascular reflex effects as a result of activation of chemosensitive nervous receptors located in the muscular tissue, and also that during physical exercise similar reflex modifications are produced, very probably caused by stimulation of receptors present in the contracting muscles.

[Treatment of pathological obesity biliopancreatic bypass operations: one-year follow-up of 10 cases (author's transl)].

A biliopancreatic bypass operation has been used to treat obese patients since June 1978, and 10 women have been followed-up for over a year. Biliopancreatic shunts reduce food absorption by eliminating enzymatic and biliary secretion activity in the intestinal lumen. Bodyweight loss was satisfactory, was proportional to excess weight, and was evaluated as being a mean of 33.1 kg. The constant and sometimes dramatic diarrhea provoked by conventional jejuno-ileal bypass operations was not observed. Hepatic steatosis, frequently present to varying degrees, was never made worse : in one case it even disappeared completely, while in another case biological tests conducted one year after operation showed considerable regression. Neither severe complications nor mortality were reported in this series.
