Maternal kidney stones during pregnancy and adverse birth outcomes, particularly congenital abnormalities in the offspring.

OBJECTIVE: The possible adverse birth outcomes, particularly congenital abnormalities (CAs) in pregnant women with kidney stones (KS) previously have not been evaluated; therefore, we decided to study this possible association. METHODS: The population-based data set of the Hungarian Case-Control Surveillance of CAs, 1980-1996, was used for this analysis. RESULTS: Of 22,843 newborns or fetuses with CAs, 69 (0.30%) had mothers with KS during pregnancy. Of 38,151 matched control newborns without any abnormalities, 147 (0.39%) had KS during pregnancy. KS were associated with an adjusted prevalence odds ratio (POR) with 95% CI of 0.8, 0.6-1.0 for CAs. A higher prevalence of maternal KS during the first trimester of pregnancy was not found in any CA group. There was no higher rate of preterm birth and low birthweight in the newborns of pregnant women with KS. CONCLUSIONS: There is no higher risk for adverse birth outcomes particularly CAs in the offspring of mothers with KS and related drug treatments during pregnancy.

Maternal urinary tract infection and related drug treatments during pregnancy and risk of congenital abnormalities in the offspring.

OBJECTIVE: The association between urinary tract infection (UTI) of pregnant women and preterm birth/low birthweight is known, but the possible association between UTI and congenital abnormalities (CAs) was evaluated rarely. Only one study showed an association with atrial septal defect, thus we decided to check this possible association. DESIGN: The population-based large data set of the Hungarian Case-Control Surveillance of Congenital Abnormalities (HCCSCA); most maternal UTIs were based on medically recorded data. SETTING The HCCSCA, 1980-1996, contained 22 843 newborns or fetuses with CAs and 38 151 matched controls, i.e. newborn infants without any HCAs. POPULATION: Hungarian informative offspring: live births, stillbirths and prenatally diagnosed malformed fetuses. METHODS: Case-control pair analysis. MAIN OUTCOME MEASURES: Twenty-five CA groups. RESULTS: A total of 1542 (6.75%) mothers in the case group had UTI during entire pregnancy compared with 2188 (5.74%) mothers in the control group (adjusted prevalence odds ratios [POR] with 95% CI: 1.15, 1.06-1.24). We did not find a higher prevalence of UTI during the second and/or third months of pregnancy in total case group (adjusted POR with 95% CI: 1.1, 0.9-1.2) and in any group of CAs including atrial septal defect type II. CONCLUSIONS: No evidence for the teratogenic effect of maternal UTI and related drug treatments during early pregnancy.

ErbB-2/HER-2 protein expression, serum tumour necrosis factor-alpha (TFM-alpha) and soluble tumour necrosis factor receptor-2 (TNFR-2) concentrations in human carcinoma of the uterine cervix.

The expression of erbB-2 protein (by immunohistochemistry), serum TNF-alpha, soluble TNF-receptor 2 (sTNFR-2, ELISA) concentrations and mitogenic (LPS, ConA, PHA) induced TNF-alpha production of the peripheral blood mononuclear cells (PBMNC) were studied in 91 (UICC Stage 1: 39, Stage 2: 33, Stage 3: 14, Stage 4: 5) patients with carcinoma of the uterine cervix. During a follow-up period of seven years 30 patients died (Stage 4: 5, Stage 3: 12, Stage 2: 11, Stage 1: 2). ErbB-2 protein expression was significantly more frequent in patients with UICC Stages 3-4 (14/19), and in those with fatal outcomes (14/30, p < 0.0001, chi-square test). Serum TNF-alpha (2.70 +/- 0.69 pg/ml) and sTNFR-2 (3.85 +/- 1.05 ng/ml) concentrations were significantly lower in cancer patients (p < 0.0001, Mann-Whitney test) as compared to 64 age-matched control women (TNF-alpha: 4.32 +/- 0.36, TNFR-2: 4.85 +/- 0.82). The mitogenic induced TNF-alpha production of PBMNC was also significantly less with all the three mitogens applied (LPS: 35.24 +/- 8.84, ConA: 26.28 +/- 7.81, PHA: 20.48 +/- 7.04 pg/l million of cells/24 hours, p < 0.0001) as compared to the controls (LPS: 65.33 +/- 8.82, ConA: 51.00 +/- 8.87, PHA: 41.80 +/- 9.01). Serum TNF-alpha, sTNFR-2 concentrations and the mitogenic induced TNF-alpha production of PBMNC was significantly decreased in patients with erbB-2 positivity as compared to those with negativity. In conclusion the expression of the oncoprotein and the lower levels of the members of the TNF system seem to be poor prognostic parameters in patients with carcinoma of the uterine cervix.

Role of tumour necrosis factor-alpha in insulin resistance during normal pregnancy.

OBJECTIVE: Role of tumour necrosis factor-alpha (TNF-alpha) was studied in insulin resistance during pregnancy. STUDY DESIGN: Serum TNF-alpha (ELISA) and fasting C-peptide (Cp) (RIA) concentrations were measured in 40 healthy pregnant women (15, 12 and 13 of them in the 1st, 2nd and 3rd trimesters, respectively) and in 25 healthy non-pregnant women in a case-control study. RESULTS: TNF-alpha (X+/-S.D.: 5.33+/-0.46 pg/ml) and Cp levels (3.37+/-1.30 ng/ml) were significantly higher in the 3rd trimester as compared with matched healthy controls (TNF: 4.07+/-0.26, Cp: 1.05+/-0.36) and to the pregnant women in 1st (TNF: 4.04+/-0.26, Cp: 1.34+/-0.59) and 2nd (TNF: 4.35+/-0.32, Cp: 1.11+/-0.35) trimesters. Significant positive linear correlation was calculated among TNF-alpha, Cp, Cp/blood glucose ratio (indirect parameters of insulin resistance) and body mass indexes (BMIs) of pregnant women (P<0.01). CONCLUSION: TNF-alpha may contribute to the insulin resistance during the course of normal pregnancy.

New aspects in the pathology of the preneoplastic lesions of the larynx.

In our 5-year biopsy study (1991 95) 51% of keratoses and 16% of adult-onset papillomas contained certain degrees of dysplasia. Koilocytosis associated with HPV infection was prominent in papillomas and the majority of keratosis but negligible in cancer. Using mouse monoclonal antibody against human p53 we detected by immunohistochemistry mutated p53 overexpression not only in cancer but in some cases of keratoses and papillomas. CD44 is a multifunctional integral cell membrane glycoprotein and is found in many mesenchymal, epithelial and tumor cells. CD44 has a great number of splice variants, isoforms supposedly implicated in development, progression and metastasis formation of tumors. Using monoclonal antibody to CD44 our study suggests that the expression of pan CD44 at successive stages of human laryngeal carcinogenesis gives preliminary information about the selection advantage to the tumor cells.

[Combined effect of CO2 and Nd:YAG laser on the rat tongue].

Authors fixed the handpieces of a CO2 and Nd:YAG lasers together so, that the two laser beams had a common focus. It is histologically proved in this paper, that the combined effect fulfills all the known advantages of either of the two lasers, while disadvantages are largely eliminated. Based on these results the device is planned to be introduced in oral surgery.

Therapies available for head and neck tumours.

Treatment modalities of head and neck tumours such as cancers of the skin, lips, oral cavity, sinuses, epi-, meso- and hypopharynx, larynx and cervical soft tissues are discussed. Cervical cysts, thyroid tumours and salivary gland alterations are also examined from the therapeutic point of view. Therapeutic advices are given for the treatment of regional metastases.

Preliminary evaluation of the use of the pectoralis major (PM) myocutaneous flap.

The modified PM myocutaneous flap was applied in 75 cases between 1982 and 1986 for the closure of major defects of the oral cavity, meso- and hypopharynx and the neck region. Notwithstanding almost all of the patients had stage IV tumour, we had a good aesthetic and functional result. There was no total flap necrosis. In three cases the partial necrosis spontaneously regressed. The details of the operation techniques are compared with other reconstructive methods, the advantage of the PM flap is discussed.

[CO2-laser surgery of rhinophyma].

Authors are the first in Hungary to report on a CO2-laser dermabration performed for the treatment of rhinophyma. They describe the mode and results of the laser intervention justifying the application of the CO2-laser.

Separate and combined use of Nd:YAG and carbon dioxide lasers in oral surgery.

The experience obtained with 789 laser interventions used in oral surgery is summarized. In the first group, 756 patients were operated on by CO 2 laser, in the second, 17 were treated with Nd:YAG laser, and in the third, 16 were treated simultaneously with both types of lasers. Among the outpatient interventions performed under local anesthesia were 233 precancerous conditions, 491 benign tumours, and 65 other lesions. Following CO 2 laser interventions, the zone of the irreversible tissue damage did not exceed 300 mum in the instance of high energy cutting; it was almost 1 mm following Nd:YAG laser cutting, and it was 500 mum when cut by a combined laser beam. The precancerous conditions can successfully be treated by CO 2 laser, especially by evaporation. The Nd:YAG laser leaves a broad necrotic zone. The combined application, however, is highly advantageous to treat vascular lesions and markedly hemophilic structures/hemangioma, epulis, tongue resection, and so forth. In oral surgery the combined use of the CO 2 and Nd:YAG laser beams is extremely beneficial due to their excellent hemostatic effect.

[Possibilities of lymphoscintillography in head and neck surgery (II): results of clinical evaluation].

In order to determine the value of lymphoscintigraphy the clinical results were compared to the lymphoscintigraphic findings. The latter were further compared to the histological results and to the isotope activity of single lymph nodes. The examination was performed in 31 cases of head and neck malignancies before neck dissection (in 29 cases radical neck dissection was done because of palpable lymph nodes; in 2 further cases elective neck dissection was performed on the NO neck). Lymphoscintigraphy yielded evaluable results in 28 cases. Lymphoscintigraphy correlated with the clinical results in 21 cases. Lymphoscintigraphy, histopathology and clinical examination showed correlation in 19 cases. Further application of head and neck lymphoscintigraphy is warranted.

Malignant teratoma of the left colon associated with chronic ulcerative colitis.

An autopsy case of a malignant teratoma of the descending colon is presented, the first observed in this site. A 41-year-old woman had suffered from chronic ulcerative colitis and was operated because of an abscess forming a mass in the left colon. However, the tumour proved to be inoperable and the patient died from septicaemia. Histologically, the tumour was a highly metastasizing malignant teratoma containing mature tissue elements (hair, cartilage, adipose tissue, squamous epithelium, bronchial, endometrial and cervical tissues). The relationship between the ulcerative colitis and the teratoma is obscure.
